[Update on Iatrogenic Creutzfeldt-Jakob Disease].

Recent progress in understanding of iatrogenic Creutzfeldt-Jakob disease (CJD) has revealed pathological and biochemical features specific to patients with iatrogenic CJD Based on the pathological and biochemical characteristics, we have identified iatrogenic CJD cases among presumed sporadic cases; in a neurosurgeon and in a patient with a medical history of neurosurgery. In addition, incidents of suspected CJD exposure via contaminated neurosurgical instruments are occasionally noted. Therefore, improvement of CJD case recognition and tracking of neurosurgical instruments are necessary to pr

Do pneumatic tube transport systems transmit potential pathogens? A hygienic risk assessment in a university hospital.

BACKGROUND: Prompted by an outbreak of vancomycin-resistant enterococci (VRE) in a medical facility, this study examined a pneumatic tube transport system (PTS) as a potential transmission channel. METHOD: Samples from the receiving station and entry racks were gathered via smear technique. Sponges used for PTS decontamination were soaked with 0.89% NaCl and transported through the channel. Micro-organisms were recovered from the tubes and cleaning sponges using a wash-away technique. Air sampling was performed at the receiving station in order to detect any airborne contamination. Tubes were

Endoscope drying and its pitfalls.

Inadequate drying of endoscope channels is a possible cause of replication and survival of remaining pathogens during storage. The presence during storage of potentially contaminated water in endoscope channels may promote bacterial proliferation and biofilm formation. An incomplete drying procedure or lack of drying and not storing in a vertical position are the most usual problems identified during drying and endoscope storage. Inadequate drying and storage procedures, together with inadequate cleaning and disinfection, are the most important sources of endoscope contamination and post-endos

Risk of Contamination in Assembled vs Disassembled Instruments in Hip Arthroplasty Surgery.

BACKGROUND: Periprosthetic joint infection (PJI) is one of the most common causes of revision total hip arthroplasty (THA) and associated with higher costs, prolonged pain, and worse clinical outcomes. Many factors have been linked to increased infection rates, one being the operative equipment and instrumentation used during the surgical procedure. With few arthroplasty instruments designed for complete disassembly and increasingly complex instrument designs, this study seeks to understand the effect that instrument disassembly plays on infection using disassembled and assembled standard femo

Evaluation of the retrograde contamination guard in a bacteriologically challenged rabbit model.

We compared a newly developed closed urinary drainage system incorporating a retrograde contamination guard (RCG) with the conventional closed drainage system. The new system contains a solid bactericide (povidone iodine) pellet enclosed in a porous cartridge at the drain port of the urine collection bag. A catheterised rabbit model was used. The urine drainage bags were challenged daily for 8 days with an auxotrophically marked uropathogenic strain of Escherichia coli at the outlet tube. The bag urine was infected with the marked organism in 10 of the 11 rabbits in the control group (conventi

Early onset fulminant neonatal sepsis and multiple serous effusions caused by Pantoea agglomerans in an extremely preterm neonate.

Pantoea agglomerans is a Gram-negative bacterium commonly isolated from environmental sources such as plants and soil. Infections in humans are usually reported following penetrating wounds or in hospital settings due to contaminated medical equipment. Neonatal sepsis caused by this organism is rare. Late onset sepsis in neonatal intensive care units due to contaminated parenteral nutrition solutions has been reported. This case report describes early onset sepsis due to P agglomerans in an extremely preterm neonate, with no known maternal risk factors. The patient had a fulminant clinical cou

Virus elimination during the recycling of chromatographic columns used during the manufacture of coagulation factors.

Various chromatographic procedures are used during the purification and manufacture of plasma products such as coagulation factors. These steps contribute to the overall safety of such products by removing potential virus contamination. Virus removal by two affinity chromatography procedures, i.e. monoclonal antibody chromatography and metal chelate chromatography (immobilised metal ion affinity chromatography), used during the manufacture of the high purity factor VIII (Replenate®) and factor IX (Replenine®-VF), respectively, has been investigated. In addition, as these columns are recycled a

Characterization of exposures among cemented tungsten carbide workers. Part II: Assessment of surface contamination and skin exposures to cobalt, chromium and nickel.

Cobalt, chromium and nickel are among the most commonly encountered contact allergens in the workplace, all used in the production of cemented tungsten carbides (CTC). Exposures to these metal-containing dusts are frequently associated with skin sensitization and/or development of occupational asthma. The objectives of this study were to assess the levels of cobalt, chromium and nickel on work surfaces and on workers' skin in three CTC production facilities. At least one worker in each of 26 work areas (among all facilities) provided hand and neck wipe samples. Wipe samples were also collected

[Respiratory and Facial Protection: Current Perspectives in the Context of the COVID-19 Pandemic].

The new SARS-CoV-2 pandemic is an example of an international public health emergency, which is associated with considerable social and economic challenges. At the healthcare level there is the risk that nosocomial outbreaks can be local amplifiers. Adequate infection control practices are of critical importance, which include proper use of personal protective equipment. This equipment must be appropriate to the pathogen transmission route that, in the case of SARS-CoV-2, occurs through droplet and contact routes. The infected individual, when talking, coughing or sneezing, spreads droplets co

A Survey of Cystic Fibrosis Center Recommendations for Nebulizer Cleaning and Disinfection.

BACKGROUND: Despite advancements in cystic fibrosis (CF) therapeutics, the persistence of chronic infections necessitates continued use of nebulized therapies. Though the Cystic Fibrosis Foundation recommends well-defined cleaning and disinfection of nebulizers to mitigate pathogen exposure risks, discrepancies between Cystic Fibrosis Foundation guidelines, manufacturers' instructions, and variability in center recommendations contribute to confusion and non-standardized practices. METHODS: A digital survey was distributed to directors, associate directors, and care coordinators of CF centers
