Exome-wide association study of blood lipids in 1,158,017 individuals from diverse populations.

Rare coding alleles have crucial roles in the molecular diagnosis of genetic diseases. However, the systematic identification of these alleles has been challenging due to their scarcity in the general population. Here we discovered and characterized rare coding alleles contributing to genetic dyslipidemia, a principal risk for coronary artery disease (CAD), among 1,158,017 multi-ancestral individuals. Testing 2,997,401 rare coding variants, we identified 800 exome-wide significant associations (176 predicted loss of function (pLoF) and 624 missense variants). Associated alleles are enriched in…

Comprehensive Germline Profiling of High-Grade Serous Ovarian Cancer Using Whole-Exome Sequencing.

While ovarian cancer screening is not recommended in the general population, attention has shifted to screening women with elevated hereditary risks. Although germline BRCA 1/2 pathogenic variants account for 40% of inherited ovarian cancer risk and family history (FH) remains important, known germline variants alone do not fully explain familial ovarian cancer risk. Whole-exome sequencing (WES) was performed on blood samples taken from 231 individuals, including 39 patients with high-grade serous ovarian cancer (HGSOC) and 192 healthy controls (HCs) stratified by FH. We analyzed pathogenic or…

Incremental yield of genome sequencing after exome sequencing for nonimmune hydrops fetalis spectrum.

OBJECTIVE: Exome sequencing (ES) detects a monogenic disorder in approximately 30% of pregnancies with unexplained nonimmune hydrops fetalis spectrum (NIHFS), defined as the presence of one or more pathologic fetal fluid collection. Since genome sequencing (GS) can identify additional genetic variants not detectable by ES, we sought to determine the proportion of cases with new reportable findings on GS following nondiagnostic standard of care testing, including ES, for NIHFS. STUDY DESIGN: Cohort study of pregnancies with NIHFS that had nondiagnostic results of karyotype and/or chromosomal m…

Exome Sequencing in Patients with Loin Pain Hematuria Syndrome and Persistent Isolated Hematuria.

INTRODUCTION: Loin pain hematuria syndrome (LPHS) is characterized by chronic loin pain and hematuria without a urological cause. We hypothesized that rare genetic variants in genes impacting the glomerular filtration barrier (endothelial cells, glomerular basement membrane, or podocytes) may contribute to LPHS. METHODS: Exome sequencing was performed in 29 consecutive patients; 17 with LPHS, and 12 with persistent painless isolated hematuria (IH). We first evaluated a virtual gene panel of 130 hematuria-linked genes curated from the literature followed by an exome-wide analysis. Variant inte…

Spontaneous spinal CSF leaks: a rare variant exome sequencing study and functional analysis.

BACKGROUND: Spontaneous spinal CSF leaks are associated with connective tissue diseases including Marfan syndrome and Loeys-Dietz syndrome, which are caused by mutations in genes that influence the content and integrity of the extracellular matrix. Patients with spontaneous spinal CSF leaks without a defined connective tissue disease diagnosis can show subtle or non-specific connective tissue disease manifestations, suggesting that mutations in extracellular matrix proteins might contribute to more common presentations of this condition. By doing a whole-exome sequencing study, we aimed to elu…

Whole-Exome Profiling of Epstein-Barr Virus-Positive Neuroendocrine Carcinoma of the Nasopharynx.

Epstein-Barr virus (EBV)-positive neuroendocrine carcinoma (NEC) of the nasopharynx is a rare malignancy with poor prognosis and lacks squamous markers, rendering it biologically distinct from nasopharyngeal carcinoma (NPC) of squamous epithelial origin. However, its molecular features remain largely undefined, and the entity has not been formally recognized as a nasopharyngeal carcinoma subtype, substantially limiting advances in its diagnosis and treatment. In this study, we performed whole-exome sequencing on seven EBV-positive nasopharyngeal NECs. These tumors exhibited a high tumor mutati…

Whole exome sequencing uncovers rare variants associated with PCOS susceptibility in Indian women.

Polycystic ovary syndrome (PCOS) is a complex polygenic endocrinopathy affecting 5-20% of reproductive-age women. Familial studies, candidate gene studies, and GWAS have identified multiple PCOS-associated genetic loci. This study aims to identify the functional variants associated with PCOS. We applied whole exome sequencing (WES) to identify functional variants among eighty-five well-characterized women with PCOS. The annotated variants were filtered based on minor allele frequency and in-silico pathogenicity prediction. We found a significant association of 234 rare pathogenic nonsynonymous…

Deciphering the digenic architecture of congenital heart disease using trio exome sequencing data.

Congenital heart disease (CHD) is the most common congenital anomaly and a leading cause of infant morbidity and mortality. Despite extensive exploration of the monogenic causes of CHD over the last decades, ∼55% of cases still lack a molecular diagnosis. Investigating digenic interactions, the simplest form of oligogenic interactions, using high-throughput sequencing data can elucidate additional genetic factors contributing to the disease. Here, we conducted a comprehensive analysis of digenic interactions in CHD by utilizing a large CHD trio exome sequencing cohort, comprising 3,910 CHD and…

Trio Whole Exome Sequencing in Chinese Childhood-Onset Lupus Reveals Novel Candidate Genes.

OBJECTIVE: Systemic lupus erythematosus (SLE) is an autoimmune disease in which rare and common gene variants contribute to pathogenesis. Severe sporadic disease in children is often explained by "de novo" variants that can be uncovered by trio sequencing. METHODS: Whole-exome sequencing was performed in 50 Chinese trios with childhood-onset SLE (cSLE). Rare coding variants in SLE-associated genes and all de novo variants were investigated. Gene pathway and expression analysis and interferon-β (IFNβ) luciferase assays were used to predict contribution to disease. RESULTS: Each proband carrie…

Expedited Exome Reanalysis Following Deep Phenotyping and Muscle Biopsy in Suspected Mitochondrial Disorder.

BACKGROUND: Exome sequencing (ES) is a useful tool in diagnosing suspected mitochondrial disease but can miss pathogenic variants for several reasons. Additional testing, such as muscle biopsy or biochemical testing, can be helpful in exome-negative cases. METHODS: We report a patient who presented with repeated episodes of lactic acidosis and failure to thrive. RESULTS: ES and mitochondrial sequencing were initially negative but clinical suspicion for mitochondrial disease remained high. After muscle biopsy showed evidence of mitochondrial dysfunction, the ES was reanalyzed and revealed nov…
