Rotationplasty--a unique surgical procedure with a functional outcome.

ROTATIONPLASTY is a surgical procedure that may be appropriate for children with severe congenital femoral deficiency or children with malignant tumors of the femur or proximal tibia. OSTEOTOMIES AND RESECTION of the femur and tibia are performed, allowing the patient's foot to be rotated 180 degrees and reattached while preserving the nerves, muscles, and blood supply. THE POSTERIOR-FACING FOOT functions as the patient's knee joint in a specially fitted prosthesis. Although the outcome is visually unusual, for some children, regaining function and physical capabilities outweighs cosmetic conc

[Telangiectatic osteogenic sarcoma].

The authors treated 6 children with sarcoma telangiectaticum (SOT). Their average ages was 7.8 years (ranging from 4 to 15 years of age). In 3 children the tumor was located in the proximal epiphysis of the femur and in the remaining 3 cases in the distal epiphysis of the femur. Clinical symptoms were present on average 8.6 weeks (ranging from 2 to 20 weeks) before the final diagnosis was made. In 3 children the final diagnosis was made after performing 1 biopsy, while in the remaining 3 cases a second biopsy was required to confirm the diagnosis. All cases were given pre-op chemotherapy (Cisp

Tibial turn-up for long distal femoral bone loss.

Since 1990, seven patients have been treated by the authors with the tibial turn-up procedure. They have ranged in age from 8 to 37 years; four were skeletally mature adults and three were children. All seven patients were faced with a clinical situation that required surgical removal of a long portion of their affected distal femur. Three of the adult patients initially were treated for osteosarcoma with long distal femoral resections and allograft arthrodesis of the knee. The allografts ultimately failed, two because of aseptic failure and one because of infection. One patient required dista

Long-term followup of patients with autogenous resection arthrodesis of the knee.

Arthrodesis of the knee may be indicated for the reconstruction after resection of tumor around the knee. Since the introduction of this technique, resection arthrodesis using segmental autogenous grafts has been the principal method of reconstruction. From August 1967 to February 1985, 73 patients underwent resection arthrodesis using autogenous grafts. All procedures were performed for malignant or potentially malignant lesions. Ten-year followup was available on 40 patients. The reconstructive procedure was performed using an intramedullary rod and hemicortical femoral or tibial grafts with

Desmoplastic fibroma of bone. A report of eight cases and review of the literature.

Desmoplastic fibroma of bone is a rare benign tumor consisting of thin, wavy fibroblasts set in an abundant matrix of collagen fibers. At times it is difficult to distinguish desmoplastic fibroma from other fibrous lesions, especially low-grade fibrosarcomas. Fewer than eight cases have been previously reported. We have reviewed the diagnostic and therapeutic findings of eighty additional cases. Six patients had the lesions located in an extremity and two had an axial lesion. The average age of the patients was twenty-five years (range, twelve to fifty-six years) and all of the patients had mo

[Peridural use of buprenorphine. Case report].

This case-report of a 78-year old patient, suffering from extreme pain caused by metastatic formation and pathological bone fractures, shows, that the epidural injection of buprenorphine is a good alternative compared to other means of pain-treatment. During almost four days heart rate, blood pressure and breathing frequency, as well as the general reactions of the patient, were observed. Few side effects and little discomfort make the method advisable for patients in poor general condition.

Appendicular skeletal tumours coined by odontogenic terms.

Two intriguing tumours in the appendicular skeleton are coined by odontogenic terms: the "adamantinomas" and "cementomas" (or "cementifying fibromas"). Both are extremely rare, mainly the latter. Eight verified observations are presented here: five adamantinomas and three cementomas. Whereas in adamantinomas, the localization and radiographic picture was very typical in all cases, only one cementoma was found in the metaphysis of a long tubular bone in our survey. Diaphyseal tibial localization in one, and metacarpal in the another patient are the first two atypical localizations, described fo

Radiographic aspects of massive bone osteoarticular allograft transplantation.

Experience with 56 cases of osteoarticular allograft transplantation of the extremities is presented. Cadaveric allograft including a joint surface and a variable amount of shaft is used in the treatment of aggressive benign and low-grade malignant tumors involving a long bone near a joint. The preoperative radiographic evaluation is outlined. Thirty-four patients had an uncomplicated course. Twenty-two patients had complications including aseptic fracture, infection, and tumor recurrence. The radiographic spectrum of both complicated and uncomplicated cases is illustrated.

Diffuse bone marrow uptake [123I]MIBG in neuroblastoma: an "MIBG super scan" case report.

We present the case of a 3-year-old child with neuroblastoma metastatic to the bone marrow. [123I]MIBG total body scintigraphy revealed a diffuse pattern of intense fixation throughout the skeleton. This was analogous to the "super scan" described with 99mTc-MDP bone scintigraphy when extensive metastatic or metabolic bone disease is present. Essentially, a high bone-to-soft tissue ratio was found without uptake in the liver, kidney and bladder, owing to a "steal" mechanism caused by the avid bone metastases.

[Chondromatous tumors in MRT: clinical picture in relation to localization and histopathology].

19 patients with histologically verified cartilaginous tumours (chondromas, chondrosarcomas) were examined with magnetic resonance imaging (MRI) in order to assess differences regarding the MR appearance of these tumours which would provide information on the biological behaviour of these lesions. MR findings were then correlated with histopathology. With regard to the signal behaviour of cartilaginous tumours a good correlation with histopathology could be established insofar as the relatively hypocellular tumours composed of hyaline cartilage exhibited a high signal level on T2-weighted imag

Primary lymphoma of bone in children.

Primary lymphoma of bone (PLB) occurs infrequently in children. Between January 1962 and November 1988, 395 children with non-Hodgkin's lymphoma (NHL) were treated at St. Jude Children's Research Hospital. Eleven of these patients (2.8%) presented with a bone primary, usually in the femur (eight of 11 patients). The median age of these seven boys and four girls at presentation was 13 years (range, 5.5 to 19 years). Seven patients had one or more additional bones involved. All patients had high-grade lymphomas based on the National Institutes of Health (NIH) Working Formulation. The histologic

[Secondary malignant fibrous histiocytoma of bone 11 years after radiation exposure].

We report about a secondary malignant fibrous histiocytoma of bone which is most frequently found in distal femur. It appears as a malignant form of a giant-cell tumor after the exposure to radiation. The typical symptoms given in literature publications: pain, swelling and the pathological fracture were present. After amputation there was no metastasis for 15 months.

Cortical bone metastases.

The data on 26 patients with solitary metastatic lesions arising in cortical bone were studied. Nineteen patients were over 50 years of age. In 19 patients, the cortical metastasis was the first indication of the presence of a primary malignant condition. In seven cases, cortical metastases developed in patients with a known primary tumor. The primary tumors involved were eight renal cell carcinomas, six bronchogenic carcinomas, two carcinomas of the gastrointestinal tract, one osteosarcoma, one neuroblastoma, one melanoma, one hepatoma, one carcinoma of the breast, and one thyroid carcinoma.

Orthopaedic management of bony metastases of renal cancer.

Pain and loss of function of the skeleton are the main symptoms of patients with bony metastases of renal cancer. Experience with 57 patients (40 of them had 52 operations) shows the surgical possibilities for early restoration of function and pain relief. Metastases were mainly localized in the femur, vertebral column, and humerus. Surgical management after nephrectomy consisted either in curettage and stabilization with plates and bone cement in case of multiple metastases or in resection and endoprosthetic treatment for solitary lesions. Thus the quality of life could be improved in every c

[Clubbed fingers and arthralgia as a reversible paraneoplastic syndrome (Pierre-Marie-Bamberger syndrome) in non-small-cell bronchial carcinoma].

HISTORY AND CLINICAL FINDINGS: During chemotherapy for a non-small-cell bronchial carcinoma with metastasis to the right femur (previously locally excised), a 34-year-old man suddenly developed severe, lasting joint pain in the ankle, knee, elbow and wrist without signs of increased warmth or swelling of these joints. At the time of diagnosis clubbed fingers had been noted. INVESTIGATIONS: Radiography of the hands showed bilateral periosteal hyperostoses. Computed tomography of the thorax revealed tumor progression. DIAGNOSIS, TREATMENT AND COURSE: The triad of clubbed fingers, periosteal hy

[Coincidence of bone infarct and aggressive bone metastasis].

A 42-year-old male patient presented with a history of persistent pain in the right femur without trauma of 2 months, duration and an episode of bloody stools 3 months earlier with no clinical findings upon examination. X-rays and CT scan revealed a circumscribed lesion with sclerosis and periostal reaction in the right proximal femur. A three-phase bone scan showed a massive hot spot in this area. Primarily differential diagnoses included a Brodie's abscess and/or a tumor. An excisional biopsy of the area was performed and revealed the coexistence of a bone infraction and the metastasis of an

Trousseau's syndrome caused by malignant degeneration of a choledochal cyst.

The patient described in this report represents the first reported case of Trousseau's syndrome caused by a malignancy arising in a choledochal cyst. The clinical features were classic, with thrombophlebitis migrans, resistance to anticoagulation, and an occult, mucin-producing adenocarcinoma. The increased recognition of the malignant potential of choledochal cysts should encourage clinicians to consider these unusual anomalies in patients with jaundice or other abnormalities of hepatic function. When possible, the cyst should by excised to minimize the risk of malignant degeneration.
