The fetus of the diabetic mother: growth, malformations and methods of surveillance by ultrasonography--the Beilinson Medical Center experience.

Some important issues continue to plague the perinatologist caring for the pregnant diabetic. These issues deal principally with fetal surveillance to prevent macrosomia, idiopathic prematurity and multiple congenital anomalies. In this report particular emphasis is placed upon the role of transvaginal and transabdominal ultrasonography in monitoring fetal growth and in early detection of congenital anomalies. New modalities, such as Doppler flow measurements and nuclear magnetic resonance imaging, are described in addition to our experience at the Beilinson Medical Center in monitoring diabet

Early onset fetal growth restriction.

Fetal growth restriction remains a challenging entity with significant variations in clinical practice around the world. The different etiopathogenesis of early and late fetal growth restriction with their distinct progression of fetal severity and outcomes, compounded by doctors and patient anxiety adds to the quandary involving its management. This review summarises the literature around diagnosing and monitoring early onset fetal growth restriction (early onset FGR) with special emphasis on optimal timing of delivery as guided by recent research advances.

Antepartum cardiotocography--an audit.

Between 1981 and 1986, 9,840 women were monitored by antepartum nonstressed cardiotocography (CTG). A satisfactory fetal reserve pattern was detected in 91%, a reduced reserve pattern in 8% and a critical reserve pattern in 1%. The incidences of fetal growth retardation, Apgar score less than 6 at 1 minute, perinatal mortality and Caesarean section all increased significantly (p less than 0.001) as the degree of cardiotocographic fetal reserve worsened. Intrauterine growth retardation and/or low urinary oestriol excretion was associated with a highly significantly increased incidence of abnorm

The value of cardiotocography in abruptio placentae. Case report.

The main indication for Caesarean section in cases of placental abruption is fetal distress. We report a patient in whom clinical examination, including auscultation of the fetal heart, suggested a mild haemorrhage, which would normally have been managed conservatively, but continuous fetal heart rate monitoring showed severe asphyxia, necessitating emergency Caesarean section at which a large concealed haemorrhage was found.

Intrapartum assessment of the postdate fetus.

Continuous monitoring of fetal heart rate (FHR) and routine sampling of fetal scalp blood were utilized to evaluate the intrapartum performance of 82 fetuses of postdates pregnancies. A comparison was made between this group and 260 term pregnancies. The incidences of abnormal FHR patterns were not different between the postdates group and the control group. All of the postdates neonates with low 5 minute Apgar scores had passed meconium. The mean pH values of the postdates fetuses with meconium were significantly lower than those of the control group at each sampling interval (i.e., early lab

[Human fetal breathing movements: basic analysis and clinical relationship to fetal well-being (author's transl)].

By means of real time B-scan, human fetal breathing movements (FBM) was basically and clinically investigated on 244 observations in 215 pregnancies. The results obtained were as follows. (1) FBM was frequently observed during active sleep in which FHR variability and fetal movement increased. (2) The incidence of FBM increased rapidly from 12.5% at 23-27 weeks to 77-100% at more than 35 weeks gestation, but reduced to 44% at delivery. (3) FBM became more regular and deeper with advancing weeks of pregnancy, especially after 36 weeks of gestation. The frequency of FBM in normal pregnancy was s

[Obstetric management and delivery in diabetics].

A review of the obstetrical management of diabetic pregnancies is presented. The best way to avoid complications is to achieve the best possible diabetic control. Ultrasonics are able to determine gestational age in the first weeks of pregnancy, to detect fetal malformations and to follow fetal growth. At the end of pregnancy, fetal heart rate monitoring is the golden standard to judge fetal well being. Delivery at term is of paramount importance. The presence of fetal macrosomia is an important parameter of the decision of the mode of delivery.

Unrecognized maternal heart rate artefact in cases of perinatal mortality reported to the United States Food and Drug Administration from 2009 to 2019: a critical patient safety issue.

BACKGROUND: Maternal heart rate artefact is a signal processing error whereby the fetal heart rate is masked by the maternal pulse, potentially leading to danger by failure to recognize an abnormal fetal heart rate or a pre-existing fetal death. Maternal heart rate artefact may be exacerbated by autocorrelation algorithms in modern fetal monitors due to smooth transitions between maternal and fetal heart rates rather than breaks in the tracing. In response, manufacturers of cardiotocography monitors recommend verifying fetal life prior to monitoring and have developed safeguards including sign

The importance of the learning process in ST analysis interpretation and its impact in improving clinical and neonatal outcomes.

BACKGROUND: Intrapartum fetal heart rate monitoring was introduced with the goal to reduce fetal hypoxia and deaths. However, continuous fetal heart rate monitoring has been shown to have a high sensitivity but also a high false-positive rate. To improve specificity, adjunctive technologies have been developed to identify fetuses at risk for intrapartum asphyxia. Intensive research on the value of ST-segment analysis of the fetal electrocardiogram as an adjunct to standard electronic fetal monitoring in lowering the rates of fetal metabolic acidosis and operative deliveries has been ongoing. Ectopic Atrial Beats May Cause a ZigZag Pattern at Intrapartum Recording of the Fetal Heart Rate Using Fetal Scalp Electrode.

INTRODUCTION: Fetal scalp electrode (FSE) is considered the gold standard for the intrapartum monitoring of the fetal heart rate (FHR) being associated with the lowest rate of signal loss and artifacts including the recording of the maternal heart rate. FSE acquires a fetal electrocardiogram and evaluates the time intervals between successive R waves. As such, it allows the recording of the beat-to-beat fluctuation of the FHR. However, due to the precise estimation of the inter-beat interval, FSE may also demonstrate recurrent atrial ectopic beats and register a highly oscillatory FHR pattern

Use of peripartum ST analysis of fetal electrocardiogram without blood sampling: a large prospective cohort study.

OBJECTIVE: Fetal peripartum surveillance with ST analysis of fetal electrocardiogram (STAN) alone or in combination with fetal blood sampling (FBS) is a worldwide debate. STAN monitoring without FBS support was implemented in 2000 in the authors' department when it took part in a European multicentre project. The aim of this study was to evaluate neonatal outcomes associated with peripartum STAN monitoring without FBS support in a large prospective cohort of patients at high risk of peripartum fetal asphyxia. STUDY DESIGN: This prospective cohort study included all consecutive high-risk women

Development of standard definitions and grading for Maternal and Fetal Adverse Event Terminology.

OBJECTIVE: Adverse event (AE) monitoring is central to assessing therapeutic safety. The lack of a comprehensive framework to define and grade maternal and fetal AEs in pregnancy trials severely limits understanding risks in pregnant women. We created AE terminology to improve safety monitoring for developing pregnancy drugs, devices and interventions. METHOD: Existing severity grading for pregnant AEs and definitions/indicators of 'severe' and 'life-threatening' conditions relevant to maternal and fetal clinical trials were identified through a literature search. An international multidiscip

Undetected Severe Fetal Myelosuppression following Administration of High-Dose Cytarabine for Acute Myeloid Leukemia: Is More Frequent Surveillance Necessary?

BACKGROUND: Cytarabine use during pregnancy carries a 5-7% risk of neonatal cytopenia. We report two cases of fetal myelosuppression following high-dose cytarabine administration for acute myeloid leukemia (AML). CASE 1: A 36-year-old G9P6 diagnosed with AML at 21 weeks was monitored for fetal anemia weekly and growth monthly. At 33 weeks (after 2 cycles), BPP was 2/10 and MCA PSV was elevated at 1.51 MoM. Urgent cesarean section was performed. The infant had an initial pH of 6.78 and pancytopenia (hematocrit 13.3%, platelets 3 K/UL, and white blood cell count 2.0 K/UL). Initially transfusion

Prevention of stillbirths associated with umbilical cord abnormalities: A clinico-pathological overview.

Stillbirths affect millions of pregnancies every year worldwide, and obstruction of the umbilical cord blood flow is one of the main causes of fetal death. This review provides a clinico-pathological overview of cord abnormalities potentially associated with antepartum stillbirth, describing the mechanism determining the death and addressing suggestions about the possibility of stillbirth prevention. Fatal effects on the fetal circulatory system can occur due to various umbilical cord abnormalities, both in cases of acquired anomalies (i.e., multiple tight nuchal cords and true knots) or conge

Power-MF: robust fetal QRS detection from non-invasive fetal electrocardiogram recordings.

Objective.Perinatal asphyxia poses a significant risk to neonatal health, necessitating accurate fetal heart rate monitoring for effective detection and management. The current gold standard, cardiotocography, has inherent limitations, highlighting the need for alternative approaches. The emerging technology of non-invasive fetal electrocardiography shows promise as a new sensing technology for fetal cardiac activity, offering potential advancements in the detection and management of perinatal asphyxia. Although algorithms for fetal QRS detection have been developed in the past, only a few of

Fetal Arrhythmia Diagnosis and Pharmacologic Management.

One of the most successful achievements of fetal intervention is the pharmacologic management of fetal arrhythmias. This management usually takes place during the second or third trimester. While most arrhythmias in the fetus are benign, both tachy- and bradyarrhythmias can lead to fetal hydrops or cardiac dysfunction and require treatment under certain conditions. This review will highlight precise diagnosis by fetal echocardiography and magnetocardiography, the 2 primary means of diagnosing fetuses with arrhythmia. Additionally, transient or hidden arrhythmias such as bundle branch block, QT

[Limit of antenatal management of intrauterine growth retarded fetus].

Recent advances in perinatal medicine contributed to the survival of extremely premature infants. Nonetheless, prognosis of the fetus with intrauterine growth retardation (IUGR) especially of preterm IUGR is not satisfactory with respect to both neonatal and long term neurodevelopmental outcome. As in utero pathophysiology of the IUGR has been revealed by noninvasive fetal monitoring technique along with invasive cordocentesis to determine fetal blood gas parameters, most of the neonatal morbidity of IUGR is thought to be originated prior to birth. Excluding the IUGR due to chromosomal abnorma…
