BACKGROUND: The application of bronchoscopy in infants and small children is still restricted in most institutions because of their limited airways. The purpose of this study was to evaluate the usefulness of a modified mini (2.1 mm OD) flexible fiberoptic endoscope (FFE) in diagnosis and therapy of suspected pediatric airway problems.

A case of 59 year old woman with a 21 year history of recurrent pneumonias of the upper left lobe is presented. When she at last agreed for the diagnosis her chest X-rays revealed atelectasis and cirrhosis with bronchiectases of the left lung. OBJECTIVE: This study compares the use of standard overhead fluorescent phototherapy units with the BiliBlanket a woven fibreoptic pad which delivers high intensity light with no ultraviolet or infrared irradiation in the treatment of jaundice in preterm infants.
METHODOLOGY: We chose to study infants between 800 and 2500 g, with strict criteria for commencing and ceasing phototherapy. Serum bilirubin levels were followed at 12-24 h intervals until 24 h after cessation of phototherapy. Infants were allocated at random to receive either conventional phototherapy or the BiliBlanket.

The role of the fiberscope in the management of difficult and failed intubations has been well established and the importance of learning this valuable skill has been emphasized. Although rarely considered in series of lower airway foreign bodies, endobronchial sutures can cause chronic cough or hemoptysis years after thoracic surgery. Eight endobronchial sutures were found in six patients who had undergone surgery four to 30 years prior to admission. Over a three-year period, fibreoptic transbronchial lung biopsy (FTLB) was performed in 28 adult Nigerian patients with chronic infiltrative lung disease after sputum examination, haematological, biochemical and serological tests failed to provide a definite diagnosis. Several types of intra-uterine lesions can be destroyed endoscopically by coagulation with laser Nd-YAG. The use of a small diameter malleable fibroscope makes it possible to direct the energy accurately so that it is easy to use throughout the whole procedure. Operative hysteroscopy using laser has been performed in 12 cases of persistent metrorrhagia. Using this technique it has been possible to avoid open surgery such as hysterectomy or myomectomy. The procedure is quick and painless. It can be used on outpatients.

Flexible fiberoptic sigmoidoscopy more and more replaces sigmoidoscopy with a rigid tube. In a retrospective series of 152 rigid sigmoidoscopies we analysed depth of endoscopy and findings. The findings in an unsuspected case of Strongyloides stercoralis infestation coexisting with adenocarcinoma of the lung are presented.
