A rare case of symmetrical open bimalleolar fractures with bilateral dislocation of ankles is described. A 30-year-old man sustained indirect trauma working in squatting position with a heavy log of wood falling on his knees with ankles in inversion, and sustained symmetrical open III B bimalleolar fractures with dislocation of bilateral ankles with the distal fibula coming out of the skin. The patient underwent operative management on bilateral ankles and the post-operative course was uneventful. The patient had no complaints at 4 years' follow-up and had a full, pain-free function. The proba

BACKGROUND: Osteofibrous dysplasia is a congenital, non-hereditary benign bone disease characterized by localized bone protrusion and replacement of normal bone cancellous by proliferating abnormal bone fibers. For this case, there is no unified treatment standard for internal fixation reconstruction or replacement, which is mainly based on comprehensive evaluation of each patient's clinical history and imaging findings.
CASE PRESENTATION: We report a case of systemic multiple osteofibrous dysplasia complicated by left femoral bone fracture, left proximal-femur shepherd's crook deformity, and

The Bosworth fracture dislocation is a rare type of ankle fracture and is usually unrecognized on initial radiographs, therefore early open reduction is recommended. This study reports a 51-year-old female with uncommon imaging and clinical features. Preoperative X-ray, computed tomography, and magnetic resonance imaging of the ankle showed posterior detached dislocation of the intact distal fibula, the loose osteochondral fragments located in the tibiotalar joint, and the rupture of the deltoid ligament. Due to the initial infected wound on the severely swollen ankle, delayed arthroscopy-assi

We describe a rare ankle fracture pattern called "the Bosworth fracture," originally described in 1947. A Bosworth fracture has been defined as a fixed posterior fracture-dislocation of the distal part of the fibula in which the proximal fibular shaft fragment locks behind posterior tibial tubercle. This fracture is frequently irreducible by closed means. Severe early and late morbidity, resulting from failure to identify and promptly treat this fracture pattern, have been reported. We describe a new radiographic sign seen on anterior-posterior mortise radiographic view showing cortical densit

A four-year-old male with cerebral palsy and spasticity, as a result of a non-accidental head injury sustained when he was two years old, died of pneumonia. Postmortem full body X-rays revealed fractures of varying ages of the left humerus and both femora, tibiae, and fibulae. This led to a thorough investigation of the case by the Office of the Chief Medical Examiner. Child abuse, accidents, metabolic bone disorders, other primary or secondary diseases of the bones, and pathological fractures were ruled out. The final diagnosis was spontaneous fractures secondary to osteopenia. The term spont

The incidence of paediatric fractures among winter sport athletes is not adequately studied. Our objective was to categorize fractures that occurred in paediatric skiers and snowboarders at a single ski resort. X-rays of 756 skiers/snowboarders aged 3-17 diagnosed with a fracture were categorized using the Salter-Harris (SH) classification. SH fractures were seen in 158 (21%) patients, with 123 (77%) being Type II. There were no significant differences between patients with a SH fracture and patients with a non-SH fracture for age, sex, snowboarding or skiing, mechanism of injury, terrain or t

Most of the anatomic variations of the extensor hallucis longus (EHL) muscle are related to the tendon of insertion. We show a double origin of the EHL from the medial aspect of the fibula and the lateral aspect of the tibia. A 27-year-old male with a double closed fracture of tibia and fibula showed an involuntary extension of the big toe during foot plantar flexion after surgery. A tendon fibrosis by the fixation plates could be the cause of the foot functional alteration. Interestingly, the anatomic variation described could be related to the postsurgical foot dysfunction, since when the fi

Eight cases of fatigue fracture were observed by the authors. In two patients, they presented in metatarsal bones; others were in the lower end of the tibia, in the shaft of the fibula, in the patella, at the upper end of the fibula, in the shaft to the tibia and in the neck of the femur. The main features of these fractues, which occurred in previously intact bones, are reviewed. The role of previous muscle imbalance, or metabolic disturbance, (by age or in pregnancy) is discussed. Good results were obtained by simple immobilization. Surgery may be indicated in the two situations: 1. To elimi

BACKGROUND: The effects of changes in distal tibiofibular joint (DTFJ) congruence on clinical and functional outcomes are unclear in patients operated on for ankle fractures. The present study aims to evaluate the relationship between changes in DTFJ congruence and clinical and functional outcomes in the short-term follow-up of the patients operated on for ankle fractures.
METHODS: In this study, hospital records of patients who were operated on for ankle fractures were retrospectively analyzed. The data of patients who underwent bilateral ankle computed tomography scans at least 18 months af

BACKGROUND: Tibial resection in children presents a unique reconstructive challenge because of technical demands, future growth potential, and repetitive loading of the lower extremity. Positioned in a tibial defect, the free fibula undergoes primary bone healing while maintaining intrinsic resistance to infection and the ability to hypertrophy. The authors' aim was to evaluate surgical and functional outcomes in this selected group of pediatric patients.
METHODS: This is a retrospective review of 13 consecutive pediatric oncology patients who underwent reconstruction of segmental tibial defe

Fibula osteoseptocutaneous flap has been widely used for oncologic bony reconstruction of both the mandible and maxilla. Early and late morbidities of the donor side such as leg weakness, ankle instability, limited ankle mobility, tibial stress fractures or incision area pain are well documented; however, there is a lack of information about the effects of fibula grafting on patient quality of life. To address this issue, a scoping literature search in the PubMed electronic database was performed to identify all relevant studies and reviews in the period between 2010 and 2022. The potential di

In a series of 104 open tibia fractures, six patients developed compartment syndromes involving all four compartments of the lower leg, four of which developed after initial debridement and reduction. The presence of an open tibia fracture with a displaced, comminuted, fibula fracture should suggest the possibility of a developing compartment syndrome. Clinical symptoms include: increasing muscle pain which can be exaggerated by stretching; loss of sensation; decrease in muscle strength; and palpably swollen compartments. Double fasciotomy incisions may lead to adequate decompression, but resu

A syndrome in very low birth weight premature infants weighing less than 1500 g is evidenced by developmental nutritional rickets and fractures at 75 days of age. In a review conducted over 42 months, 247 very low birth weight cases were identified. Rickets was diagnosed in 96 (39%) infants whose mean age was 50 days and fractures were diagnosed in 26 (10.5%) infants whose mean age was 75 days. These 26 infants experienced 98 fractures: 10 humerus, 13 radius, 8 ulna, 4 metacarpal, 3 clavicle, 54 ribs, 5 femur, and 1 fibula. Risk factors included hepatobiliary disease, total parenteral nutritio

The bone mineral content of the upper ends of the tibia and the fibula was measured in 27 patients with tibia shaft fracture. The bone mineral decreased rapidly after fracture, the loss continuing for about 5 months. Later, towards the end of the first year after the injury, there was a slow restoration of mineral but no return to initial values in most instances. Although the average maximum loss was approximately 45%, only 25% of the initial bone mineral was missing after one year.

In the treatment of proximal and distal tibial fractures, special attention has to be focused on the severity of tissue damage. Guidelines are given in the paper. (1) For tibial head fractures, stable internal fixation is usually necessary but should be delayed for 1 week. The best approach is lateral or ventral. The use of implants for buttressing has to be kept to a minimum. Concomitant ligamentous injuries are reconstructed, and damaged menisci should be preserved. (2) In the case of distal tibial fractures, those not involving the joint can be treated non-operatively. The internal fixation

Orthopedic surgery is not short of situations where there is controversy regarding optimum management. Treating ankle syndesmosis injuries is an example where practice varies widely and there are many questions that remain unsatisfactorily answered. When addressing the type of syndesmosis stabilization that is required it is essential to ascertain the extent of instability. Only then can a logical approach to restoring the ankle mortise be achieved. Fixation of fibula shaft fractures and posterior malleolus fractures can restore sufficient stability to render syndesmosis stabilization unnecess

BACKGROUND: Displaced tibial shaft fractures are common in adolescent patients, yet there is no standardized management strategy. We compared surgical fixation and closed reduction and casting (CRC) of these fractures to assess treatment outcomes and determine predictors of failure.
METHODS: We retrospectively reviewed all patients aged 12 to 18 who presented with a displaced tibial shaft fracture that required reduction over an 8-year period. Exclusion criteria included open fractures and lack of follow-up to radiographic union or to 6 months from the index procedure. Fractures were initiall

BACKGROUND: The reconstructive and rehabilitative management of large mandibular defects with basal continuity is challenging in many respects, especially in the vertical dimension. The free fibula flap is an under-utilised but efficient approach in this indication. The aim of this case series is to demonstrate its use and long-term success.
CASE PRESENTATION: Three cases are presented, where the patient had a large bone defect (at least 5 cm in length and 1 cm in the vertical dimension), but the continuity of the mandible was maintained. Two cases were related to pathological fracture and on

This old operation is rarely practiced and it appeared interesting to precise its technique and indications from a short series. In the majority of our cases, a valgus type osteotomy by medial graft addition was performed. One or more prismatic triangular graft are taken from the full thickness of the iliac crest. The respect of the fibula protects the periostal hinge on the other side of the surgical approach; so the graft under pressure will have rapid and constant consolidation. Tibial osteosynthesis is facultative. We can distinguish major indications of that type of osteotomies: supramall

The distal fibula plays a key role in ankle stability by forming the ankle mortise and serving as the attachment site for the lateral ligament complex. Traumatic loss of the distal fibula is extremely rare and is associated with ankle instability, valgus deformity, and eventual arthritis if not addressed. We report the case of a 34-year-old female who sustained a high-energy open injury with complete loss of the distal fibula, disruption of the lateral ligament complex, and a comminuted distal tibial fracture following a road traffic accident. Initial management included emergency debridement,
