Effects of serial casting in the treatment of flexion contractures of proximal interphalangeal joints in patients with rheumatoid arthritis and juvenile idiopathic arthritis: A retrospective study.

AIM: To analyze the effects of serial casting (SC) in the treatment of proximal interphalangeal (PIP) joint flexion contractures in patients with rheumatoid arthritis and juvenile idiopathic arthritis. STUDY DESIGN: Retrospective case-series. METHODS: The data of 18 patients treated with SC were obtained from their patient records. The angular changes in the finger joints were analyzed and compared statistically using t-tests. RESULTS: A total of 49 fingers were serially casted with plaster of Paris over a 14-year period. The SC resulted in significant (26.8°; p < 0.001) reduction in the PI

[Research on minimally invasive release treatment of stenosing tenosynovitis of flexor digitorum].

The minimally invasive release treatment of TCM Small Needle-Knife for the stenosing tenosynovitis of flexor digtorum-"trigger finger" has a more satisfied efficacy. In recent years, many clinicians use self-made small sharp scalpels, iris knives, small sickles, push shear knives, and other improved alternatives to instead of the traditional small needle-knives. Changing the original small needle-knife vertical stabbed cutting method, take a mini-incision, along the traveling direction of flexor tendon make a vertical hook cut, pick cut, straight push cut and any other cuts, completely cut the

Robust neural decoding for dexterous control of robotic hand kinematics.

BACKGROUND: Manual dexterity is a fundamental motor skill that allows us to perform complex daily tasks. Neuromuscular injuries, however, can lead to the loss of hand dexterity. Although numerous advanced assistive robotic hands have been developed, we still lack dexterous and continuous control of multiple degrees of freedom in real-time. In this study, we developed an efficient and robust neural decoding approach that can continuously decode intended finger dynamic movements for real-time control of a prosthetic hand. METHODS: High-density electromyogram (HD-EMG) signals were obtained from

Clinical significance of an extensor tendon anomaly to the little finger--a new finding.

A new anatomical variation of extensor digiti minimi (EDM) was discovered during our anatomical studies. The EDM usually gives one tendon to the little finger to extend the metacarpophalangeal joint (MPJ) independently from the other fingers. The EDM in our cadaver had three tendon slips. It supplied the MPJ of the little finger with two tendon slips. Along with the extensor digitorum communis (EDC), the EDM also had an additional slip to the ring finger MPJ extensor hood apparatus. The little finger also received a separate tendon from EDC.

A retrospective study of closed extension block pinning for mallet fractures: Analysis of predictors of postoperative range of motion.

BACKGROUND: Extension block pinning is a simple and reliable technique for mallet fractures, but poor results are sometimes obtained. The predictors of postoperative range of motion after extension block pinning of mallet fingers were investigated. METHODS: The outcomes for postoperative active motion of the distal interphalangeal (DIP) joint, such as flexion angle, extension loss, and total range of motion, were examined. Predictors such as age, gender, finger, fragment size, joint subluxation, the time from injury to operation, procedure, fixation angle, the time from operation to wire remo

The influence of Dupuytren's disease fingers contracture degree on surgical treatment outcome.

BACKGROUND/AIM: Dupuytren’s disease is a progressive disease of the palmar and digital fascial structures, with functional limitations. There are no clear recommendations about the optimal time of surgical repair, concerning the hand impairment. The aim of our study was to investigate the relation between finger’s contracture degree and success of surgical treatment of the Dupuytren's disease. METHODS: This prospective analysis included 60 patients operated on due to Dupuytren’s contracture. According to preoperative contracture degree of proximal interphalangeal (PIP) and metacarpophalangeal

Abducted, hyperextended small finger deformity of nonneurologic etiology.

The abduction and hyperextension deformity of the small finger is usually associated with ulnar nerve palsy. Six patients who sustained an abduction force to their small finger presented with the finger in an abducted and hyperextended position. All patients were neurologically intact and were unable to adduct or flex the finger at the metacarpophalangeal joint. Surgical findings in these patients included rupture of the radial sagittal band, collateral ligament, and junctura tendinum. Sequential division of these structures in cadaveric hands confirmed that all these tissues had to be deficie

Losing dexterity: patterns of impaired coordination of finger movements in musician's dystonia.

Extensive training can bring about highly-skilled action, but may also impair motor dexterity by producing involuntary movements and muscular cramping, as seen in focal dystonia (FD) and tremor. To elucidate the underlying neuroplastic mechanisms of FD, the present study addressed the organization of finger movements during piano performance in pianists suffering from the condition. Principal component (PC) analysis identified three patterns of fundamental joint coordination constituting finger movements in both patients and controls. The first two coordination patterns described less individu

Needle Aponeurotomy for Stage IV Dupuytren Contracture: A Wide-Awake First Step Approach in 204 Consecutive Rays.

SUMMARY: Severe forms of Dupuytren disease are difficult to treat. Surgical fasciectomy is often the first choice, despite its high complication rate. At times, amputation is recommended. The authors evaluated the efficacy of minimally invasive needle fasciotomy (needle aponeurotomy) as the first and only treatment for severe (stage IV) Dupuytren contracture using a retrospective chart review of a single surgeon's consecutive experience over 8 years. A total of 204 rays from 165 patients with severe Dupuytren disease with total passive extension digit contracture of 135 degrees or greater were

[Aims of hand therapy in treatment of rheumatoid hand].

When dealing with rheumatoid arthritis, the main issues of hand therapy are the treatment of hand and finger joints. We emphasize mobilisation of joints, muscle strengthening, correcting deformities by low temperature splints and joint protection. Further tasks of occupational therapy are: Informing patients about adaptive devices and training their use as well as educational programs for patients with reduced mobility of the upper limb. The most common surgery on the rheumatoid hand, that need postoperative treatment by a hand therapist are complete and limited arthrodesis of the wrist, surgi

Symptomatic Linburg-Comstock syndrome: a case report.

The authors report the case of a 39-year-old woman with pain in the right dominant hand after initiating a crafts course. Clinical examination revealed tenderness on the volar side of the thumb and wrist. Simultaneous flexion of the distal interphalangeal joint of the index finger with flexion of the interphalangeal joint of the thumb was observed. Operative procedure confirmed the presumed diagnosis of an anomalous connection between the flexor pollicis longus and the flexor digitorum profundus tendon to the index finger, first described in 1979 by Linburg and Comstock.

Ligamentous laxity and scar formation.

The character of scar tissue is relative to or influenced by ligamentous laxity. Patients who are able to passively hyperextend the terminal interphalangeal joint of the index finger to 45 degrees and beyond have a tendency to leave broad scars. This proved to be statistically significance (p less than .001). It is suggested that in female patients in particular, assessment of the passive hyperextensibility of the terminal interphalangeal joint of the index finger can be used preoperatively to determine whether the patient will be left with a cosmetically displeasing scar following any operati

Dynamic extension assist splinting of acute central slip lacerations.

The management of zone III extensor tendon injuries remains a challenging clinical problem for both the surgeon and therapist. Current treatment protocols include prolonged immobilization, controlled mobilization, or limited early dynamic splinting, but overall results are often less than optimal. This retrospective study describes an alternative treatment approach for acute central slip lacerations consisting of initial immobilization of the PIP joint in absolute 0 degrees extension for an average of 3.5 weeks followed by active flexion of the PIP joint in a finger-based dynamic extension ass

Hand splint for rheumatoid arthritis patients during gait training after joint replacement in lower extremity.

Rheumatoid arthritis patients have multiple joint problems, often making it difficult to use gait aids after a joint replacement in the leg. To address this problem, we designed a hand splint with a hook on the palmar side for use with parallel bars. Patients put these splints on both hands and they can walk holding the bars with the hooks. Best suited for using this splint are rheumatoid arthritis patients who are unable to hold parallel bars without marked pain in the hands or fingers, contracture of the wrist joint, or dislocation of finger joints. Several patients have tried this splint, w

The Reliability and Validity of Active Range of Motion Measurement for Fingers Using Three-Dimensional Motion Analysis.

The Fugl-Meyer Assessment (FMA) and Range of Motion (ROM) are well-known measures for evaluating finger motor function. However, the finger FMA has a narrow scoring range, and goniometer ROM measurements lack reliability. Therefore, we investigated ROM measurement using a 3D motion capture system (Ultraleap Stereo IR 170, Ultraleap Ltd., Bristol, UK, hereinafter referred to as Ultraleap). This study aimed to evaluate the intra- and inter-rater reliability of finger Active ROM (A-ROM) using Ultraleap, and its criterion-related validity compared to a goniometer, in stroke patients with upper lim

Treatment of advanced trigger finger by ulnar superficialis slip resection: Long-term outcome and predictive factors for poor prognosis.

The aim of this study was to evaluate the outcome of ulnar superficialis slip resection and to determine predictive factors for poor prognosis in patients with advanced trigger finger. Over a 5-year-period, 55 patients (58 fingers) were included. After surgery, two groups were identified: group 1, with complete extension or <10° extension deficit in the proximal interphalangeal (PIP) joint (n = 27 fingers/27 patients); and group 2, with ≥10° residual PIP extension deficit (n = 31 fingers/28 patients). Factors associated with PIP extension deficit were assessed on logistic regression. There was
