The efficacy of anal fistula plug in fistula-in-ano: a systematic review.

AIMS: To assess the efficacy of anal fistula plug (AFP) procedure for the treatment of fistula-in-ano especially the complex fistulas. METHOD: The database of PUBMED, MEDLINE, SCOPUS, EMBASE and COCHRANE LIBRARY for the period 1995-2009 was searched. A systematic analysis was carried to evaluate the success rate of AFP procedure in fistula-in-ano. RESULTS: A total of 25 studies were extracted and 12 (n = 317) were finally included in the systematic review. The follow-up period ranged from 3.5 to 12 months. The AFP procedure had a success rate (patient cure rate) ranging from 24% to 92%. In c…

Treatment of transsphincteric anal fistulas: are fistula plugs an acceptable alternative?

PURPOSE: Treatments for transsphincteric, cryptoglandular anal fistulas pose risks for high recurrence rates and impaired anal continence. Anal fistula plugs have gained popularity because of reports of success without compromising the anal sphincter. To examine the benefit of the anal fistula plug, we compared its success rate with a standard treatment for transsphincteric fistulas with similar indications, the transanal mucosal advancement flap. METHODS: We examined the outcomes of all patients with transsphincteric fistulas who underwent anal fistula plug repair from July 2005 to December …

[Use of fistula extraction in the treatment of non-ramified pararectal fistulas].

A method of fistula extraction was used in treatment of 42 patients with non-ramified pararectal fistulas. The duration of hospital stay of the patients was 8 days, that of their out-patient treatment--14 days. Within a follow-up period of from 3 mos to 2 yrs, 1 patient with intra-sphincteric fistula developed a disease recurrency.

Arteriovenous Fistula-Associated Ascites.

Dialysis-associated ascites (DAA) is a diagnosis of exclusion in end-stage kidney disease (ESKD). The prognosis is grim and typically obviates kidney transplantation. We report a case initially misdiagnosed as DAA, which was later correctly identified as cardiac ascites due to high-output heart failure (HOHF) resulting from an aneurysmal arteriovenous fistula (AVF). We propose the term "AVF-associated ascites" (AFAA) to describe this entity. The HOHF improved with AVF ligation with complete resolution of the ascites, thus making the patient eligible to receive a kidney transplant. This case em…

Tracheomalacia and Tracheoesophageal Fistula.

Tracheomalacia and tracheoesophageal fistula are two pathologies that are commonly linked. Comprehensive aerodigestive evaluation can inform decision making on the best treatment approach. Patient factors including tracheal characteristics and surrounding anatomy influence the surgical approach.

Colovesical Fistula and Amyloidosis.

Systemic amyloidosis results from an abnormal deposition of toxic insoluble beta-sheet fibrillar protein in extracellular tissues, causing damage to multiple organ systems. Amyloid proteins (e.g., transthyretin, light chains, and serum amyloid A) may infiltrate the mucosa or vascular structures, resulting in gastrointestinal manifestations, including bleeding and diarrhoea. We discuss a case of a 71-year-old male with systemic immunoglobulin G kappa amyloid light chain amyloidosis who developed a colovesical fistula related to amyloid deposition.

Treatment of Bronchopleural Fistula.

Bronchopleural fistula (BPF) is a challenging clinical condition characterized by an abnormal connection between the bronchial tree and the pleural space. This article aims to provide a thorough review of BPF, encompassing its etiologies, clinical manifestations, diagnostic approaches, and treatment options. Current advancements and future directions in BPF management will also be discussed.

Pancreatopleural fistula in childhood.

A boy in late childhood presented with recurrent right-sided pleural effusions, chest pain and respiratory distress, which temporarily impro … c resonance cholangiopancreatography confirmed bilateral (right greater than left) pancreatopleural fistulas. The patient was treated with nil per oral status, total parenteral nutrition and Octreotide. Over a period of 10 days, drainage decreased, the chest tube was removed, and the child was discharged.

Anal Abscess and Fistula.

Anorectal abscesses and fistulas are common surgical conditions. Most often these are idiopathic and caused by cryptoglandular infections. Diagnosis relies on focused history and physical examination findings as well as imaging studies to determine the extent of sphincter involvement and exact location. Treatment of an abscess is prompt surgical drainage. Surgical management of a fistula is focused on preservation of continence while achieving healing. Optimal surgical treatment is based on anatomy of the fistula tract. Surgical options include fistulotomy, seton placement, advancement flap, L…

Vesicovaginal Fistula Following Trauma.

Vesicovaginal Fistula (VVF) is an abnormal communication between the bladder and vagina. The major causes of vesicovaginal fistula in developing countries are associated with obstructed labour whereas in developed countries, vesicovaginal fistula results from pelvic surgeries. Rarely vesicovaginal fistula may result due to direct trauma to the pelvis. This is a case of 17 years female who presented with the history of fall from tree 5 years back, sustained genital injury and complained of involuntary leakage of urine one month after the incident. She underwent fistula repair with vaginal appro…

Cholecystocutaneous fistula after cholecystectomy.

Cholecystocutaneous fistula is an exceedingly rare type of external biliary fistula, where an abnormal connection forms between the gallbladder and the skin. Cholecystocutaneous fistula commonly develops in the setting of chronic calculus cholecystitis or following a previous surgical intervention involving the biliary tract. Patients with cholecystocutaneous fistula often present with systemic symptoms, such as fever, nausea, and vomiting, as well as localized symptoms in the right upper quadrant of the abdomen, where the external opening of the fistula is typically found. Ultrasonography, co…

[Iatrogenic femoral AV-fistula].

The common femoral artery is a widely used for access in endovascular interventions. Various complications, such as hematoma, pseudoaneurysm and AV-fistula (AVF), can arise from arterial punctures with estimated prevalence between 1-10%. AVF is a rare complication with prevalence p<1%. AVF can cause a hemodynamic change in the form of a arteriovenous shunt (AV-shunt). AV-shunts in the groin are usually small and asymptomatic but tend to be symptomatic with larger and persistent AVFs which can present with leg claudication or high outpute heart failure.

[Perianal Fistula: An Overview].

Perianal fistula is a common inflammatory condition in the general population and affects the area around the anal canal. Although most cases are benign, they cause serious morbidity and require surgical treatment due to a high risk of recurrence. MR imaging is a gold standard technique for the evaluation of perianal fistulas and provides accurate information on the anatomy of the anal canal, its relationship to the anal sphincter complex, accurate identification of secondary tracts or abscesses, and reporting associated complications. MR imaging can help monitor treatment effects and determin…
