Deformable foot orthoses shift the functional roles of foot and ankle joints during walking gait.

Carbon fiber deformable foot orthoses (DFOs) with customizable stiffness may benefit clinical populations by allowing an individual approach to enhancing gait energetics. Prior research suggests that increasing DFO stiffness can decrease energy loss while enhancing energy return at the metatarsophalangeal (MTP) joint, making it more spring-like. However, it remains unclear whether increasing DFO stiffness enhances the mechanics of the midtarsal and ankle joints or only the MTP joint, especially during walking. This study investigated whether walking with increasingly stiff DFOs impacted mechan…

The infected diabetic foot: Risk factors for re-infection after treatment for diabetic foot osteomyelitis.

Our objective was to evaluate risk factors for re-infection in patients after treatment for diabetic foot osteomyelitis (OM). We used pooled patient level data from two RTCs that evaluated patients with diabetic foot infections. We evaluated 171 patients with OM. OM was confirmed with bone culture or histopathology. Data from the 12-month follow-up were used to determine clinical outcomes. Re-infection occurred in 47 (27.5%) patients. Risk factors for re-infection were Toe Brachial Index <0.40 (25.7% vs. 9.8%, p = 0.02), skin perfusion pressure <40 mmHg (6.3% vs. 5.9%, p = 0.04), wound healing…

Diabetic foot ulcer incidence and survival with improved diabetic foot services: an 18-year study.

AIMS: To ascertain the effects of improvements in diabetic foot services over 18 years on incidence of diabetic foot ulceration. We also compared survival time from first ulcer development with presence of neuropathy, peripheral vascular disease, age and healing. METHODS: Persons with new ulceration and those at high risk of ulcer development were referred to community podiatry from 1998. Their details were recorded, with verbal consent, on a central database. The effects of neuropathy, peripheral vascular disease, healing and age on survival were analysed by Cox proportional hazards ratios. …

Hand-foot syndrome (hand-foot skin reaction, palmar-plantar erythrodysesthesia): focus on sorafenib and sunitinib.

Hand-foot syndrome (HFS), also called hand-foot skin reaction, palmar-plantar erythrodysesthesia, acral erythema, and Burgdorf reaction, is a dose-limiting cutaneous toxicity of many chemotherapeutic agents. Recently, the multiple tyrosine kinase inhibitor class of novel targeted therapies, including sorafenib and sunitinib, has emerged as an important cause of HFS, with 10-28% of patients treated with sunitinib and 10-62% of patients treated with sorafenib reporting HFS. This review examines the epidemiology, clinical features, histopathology, pathogenesis models, prognostic implications, and…

The biomechanical effects of 3D-printed ankle-foot orthosis in individuals with unilateral foot drop.

Thermoplastic ankle-foot orthotics (AFOs) effectively manage foot drop but are time-intensive to fabricate, while 3D printing offers a faster, customizable alternative with promising early results. However, broader adoption requires further clinical evidence on biomechanical efficacy and user satisfaction, particularly for novel 3D-printed AFOs. This study compared the biomechanical effects of a 3D-printed ankle-foot orthosis (3D-AFO) with a standard thermoplastic AFO (S-AFO) during walking in individuals with unilateral foot drop and evaluated user satisfaction. Nine participants were assesse…

Foot Flexibility and Alignment Are Weakly Correlated with Multisegment Foot Kinematics During the Landing Task.

BACKGROUND: Flat feet show increased foot motion during dynamic tasks. Although the increased motion is one of the risk factors for sports-related injuries, few studies have investigated how foot flexibility affects foot kinematics during landing. This study aimed to investigate the relationship between foot alignment, foot flexibility, and multisegment foot kinematics during a double-leg landing task in healthy participants. METHODS: Thirty-eight healthy volunteers (50% female) participated in this study. Their mean ± SD age was 22.0 ± 1.3 years; height, 166.1 ± 8.9 cm; and mass, 57.5 ± 8.8 …

Effect of bar length on foot abduction and ankle dorsiflexion in Steenbeek foot abduction brace.

We investigated the impact of changes in bar length of Steenbeek foot abduction brace on foot and ankle range of motion in 150 children. The ankle dorsiflexion and foot abduction was measured without brace, with standard brace size (0) as depicted in Steenbeek manual and after variations in bar length (-2˝ to +2˝). The bar length (0) was also compared with shoulder width for Indian population. The Steenbeek foot abduction brace bar length in current use (11.53±1.2˝) was longer than shoulder size (8.14±1.18˝) with no true correlation. Steenbeek foot abduction brace usefulness was evident for fo…

Can amputation be prevented in diabetic foot? Interdisciplinary approach to diabetic foot: a case report.

Diabetic foot ulcers may result in loss of an extremity and may even lead to mortality. The use of comprehensive foot care programs which include early screening and evaluation of problems, foot care education, preventive therapy and referral to specialists has been shown to reduce amputation rates by 49-85 [percent]. A 51-year-old woman with Type 2 diabetes under surveillance for nine months was referred from the emergency department. She presented with an entirely inflamed right foot with ulcer covering two-thirds proximal of the foot for 30 days' duration. There was extensive edema as well …

Weight-bearing activity and foot parameters in Native Americans with diabetes with and without foot sensation.

Amputations from diabetic foot ulcers account for substantial health care costs and pain. At the point of neuropathy, it has been recommended that weight-bearing activities (WBA) be restricted to reduce the risk of ulcers. While regular WBA has been shown to control glycemia, stress from this exercise may lead to ulcers by various processes. This investigation compared Native Americans with diabetes, with or without foot sensation and who reported regular or no leisure-time WBA, with variables of significance for risk of diabetic foot ulcers. Analysis revealed that there were no differences in…
