STUDY DESIGN: Cross-sectional laboratory study.
OBJECTIVE: To compare single-limb heel-rise performance and foot-ankle kinematics between persons with stage 2 adult-acquired flat foot deformity (AAFD) and healthy controls.
BACKGROUND: The inability to perform a single-limb heel rise is considered a positive functional diagnostic test for AAFD. However, which foot motions contribute to poor performance of this task are not known.
METHODS: Fifty individuals participated in this study, 20 with stage 2 AAFD (mean ± SD age, 57.6 ± 11.3 years), and 15 older participants (age, 56.8 ± 5.3 years) an

We report herein a retrospective study of 25 cases of ankle arthrodesis performed in 23 patients with rheumatoid arthritis (RA) using an intramedullary nail with fins, developed in 1994. Surgical treatment, postoperative management, and clinical evaluation are described. Clinical evaluation, at an average follow-up period of 7 years 1 month, was based on foot disease scores from the Japanese Orthopedic Association; we compared these scores pre- and postoperatively, and during follow-up. These parameters showed a significant difference between preoperation and the follow-up period. However, ins

BACKGROUND: Focal spasticity of the gastrocnemius-soleus muscles causes equinus gait in children with cerebral palsy (CP). Botulinum toxin type A (BTX-A), a neuromuscular blocking agent, reduces muscle tone/overactivity in dystonia, stroke, and CP.
OBJECTIVE: A prospective, open-label, multicenter clinical trial evaluated the long-term safety and efficacy of repeated intramuscular injections of BTX-A on equinus gait in CP children.
METHODS: Nine centers enrolled 207 children. BTX-A injections (4 U/Kg) were given approximately every 3 months (maximum dose 200 U per treatment). Outcome measure

BACKGROUND: Spring ligament complex (SLC) insufficiency is regarded as an important component of progressive collapsing foot deformity (PCFD). Reconstruction of this ligament complex is often favored over primary repair since the latter has not proven successful in chronic injuries. We present the clinical and radiographic outcomes of primary repair in PCFD performed using the navicular osteotomy technique which improves visualization and repair of SLC and compare the results with augmentation and reconstruction series reported in the literature.
METHODS: We retrospectively reviewed 17 PCFD o

PURPOSE: Development of valgus tibiotalar tilt is a significant complication after subtalar fusion for progressive collapsing foot deformity (PCFD) correction. However, its incidence and etiologic factors have not been extensively studied. The purpose of this study was to define the incidence of valgus tibiotalar tilt after subtalar fusion for PCFD reconstruction, and to determine predictors of this complication.
METHODS: This study included 59 patients who underwent PCFD reconstruction with subtalar fusion. Patients with tibiotalar tilt prior to surgery were excluded. On standard weightbeari

The aim of this study is to analyze patient-reported outcomes following this procedure as well as any demographics that may confer prognostic capability. A retrospective analysis was conducted of patients who underwent Triple Arthrodesis at our facility from 2014-2021. Patients were selected if they underwent an isolated triple arthrodesis. All cases included either a gastrocnemius recession versus a percutaneous tendo-achilles lengthening depending on the patient's Silverskiold examination. The electronic medical record was utilized to collect basic patient demographics, previous foot and ank

Over the past decade, Finite Element (FE) modelling has been used as a method to understand the internal stresses within the diabetic foot. Foot deformities such as hammer toe have been associated with increased risk of foot ulcers in diabetic patients. Hence the aim of this study is to investigate the influence of hammer toe deformity on internal stresses during walking. A 3D finite element model of the human foot was constructed based on capturing Magnetic Resonance Imaging (MRI) of a diabetic neuropathic volunteer exhibiting hammer toe. 3D gait measurements and a multi-body musculoskeletal …

BACKGROUND: Cavovarus foot is a common form of foot deformity in children, which is clinically characterized by an abnormal increase of the longitudinal arch of the foot, and it can be simultaneously complicated with forefoot pronation and varus, rearfoot varus, Achilles tendon contracture, or cock-up toe deformity. Muscle force imbalance is the primary cause of such deformity. Many diseases can lead to muscle force imbalance, such as tethered cord syndrome, cerebral palsy, Charcot-Marie-Tooth disease, and trauma. At present, many surgical treatments are available for cavovarus foot. For older

BACKGROUND: In children with cerebral palsy (CP), overactivity of the peroneus longus (PL) muscle is a major contributor to pes planovalgus. This retrospective study assessed whether abobotulinumtoxinA injections into a PL showing premature activity on electromyography (EMG) clinically improved foot morphology in children with CP.
METHODS: Study participants were <6 years old, had a diagnosis of CP, good functional abilities (Gross Motor Function Classification System level 1 or 2), equinovalgus (initial contact with the hallux or head of the first metatarsal) and overactive PL on EMG. The fo

OBJECTIVE: Equinovarus foot deformity following stroke or traumatic brain injury compromises walking capacity, interfering with activities of daily living. In soft-tissue surgery the imbalanced muscles responsible for the deviant position of the ankle and foot are lengthened, released and/or transferred. However, knowledge about the effectiveness of surgical correction is limited. The aim of the present study was to carry out a systematic review of the literature to assess the effects of surgical correction of equinovarus foot deformity in patients with stroke or traumatic brain injury.

Management of a child with club foot deformity that recurs after an apparently successful posteromedial soft tissue release remains a challenge. Revision surgery is often attempted, but this carries a high risk of significant scarring or neurovascular complications. We have treated 3 children (3 feet) with relapsed club foot and fixed deformities, using Botulinum toxin A (BTX-A) injection into muscle groups clinically thought to be responsible for recurrent deformity, followed by moulded plaster casts. Two cases have maintained a good foot position with an minimum follow-up of 18 months, and o

In order to investigate the shape and size differences in feet caused by daily footwear, a comparative study was conducted on foot morphology in two populations. The data from six measurements in general physique and 18 measurements in the feet and their contours were obtained from 34 Filipino women in Isabela Province and 40 Japanese women in Tokyo. Despite the fact that the Tokyo women had larger physique than the Isabela women, there were no significant differences in foot size between two groups. Both relative size of foot for general physique and intragroup deviation of foot proportion we

Six families with a total of 34 affected persons with the syndrome of tibial aplasia and ectrodactyly are reported. The spectrum of malformations is compared to that of 99 familial cases from the literature. The full-blown syndrome consists of bilateral aplasia of tibiae and split-hand/split-foot deformity. Additional malformations may be distal hypoplasia or bifurcation of femora, hypo- or aplasia of ulnae, and minor anomalies such as aplasia of patellae, hypoplastic big toes, postaxial and intermediate polydactyly in connection with split-hand deformity, and cup-shaped ears. INTRODUCTION: Foot deformities and amputations are parameters that have been studied as risk factors for diabetic foot ulceration (DFU). However, inclusion of "foot deformities" and "amputations" in a single, broad variable and with reference to the severity of these deformities, may better characterize subjects who are prone to develop DFU.
METHODS: The objective of the study was the examination of amputative and non-amputative foot deformities severity as risk factor for DFU in relation with the other established risk factors. A cross-sectional and case-control study was conducted from Octo…

BACKGROUND: Forefoot varus and medial column instability are important components of progressive collapsing foot deformity. Although medial column procedures are commonly performed to address these deformities, their necessity remains controversial.
PURPOSE: This study evaluated radiographic changes in medial column instability following double calcaneal osteotomy without additional medial column procedures.
STUDY DESIGN: Retrospective analysis.
METHODS: We retrospectively reviewed 44 feet in 31 patients with stage I progressive collapsing foot deformity who underwent double calcaneal osteo

Glycosylation of type 1 collagen fibers in diabetes mellitus results in contractures of the gastroc-soleus, tibialis posterior, peroneus longus muscle groups and plantar fascia, which may lead to progressive cavovarus deformity. The medial forefoot is reported to be the third most common location of chronic plantar diabetic foot ulceration (CPDFU), occurring because of increased plantar pressure secondary to deformity and loss of protective sensation. With high recurrence rates after nonoperative treatment with total contact casting and low patient adherence to orthotic footwear, surgical offl

BACKGROUND: Progressive collapsing foot deformity (PCFD) is characterized by multiplanar talar malalignment, including medial arch collapse. Although ankle arthrodesis is a well-established treatment for end-stage ankle osteoarthritis, its effect on concomitant PCFD-related parameters remains unclear. We observed that ankle arthrodesis appeared to result in radiographic changes in the medial column alignment, raising questions about whether talar stabilization might influence PCFD-related deformity.
QUESTIONS/PURPOSES: (1) Do patients with severe medial column collapse achieve greater radiogr
