An 8-year-old girl sustained closed fracture of the right ulna 10 weeks prior to presentation. She was taken to a traditional bone setter who applied a tight splint. The patient reported pain, but the splint was not removed. A week after application of the splint, a foul odor was detected and removal of the splint showed extensive exposure of the forearm bones. Above elbow amputation was rejected by the patient's parents when she was taken to hospital, where she was admitted for 8 weeks by a second traditional bone setter. Deliberate self-harm is common. It is usually by drug overdose or ingestion of other noxious substances, but self-harm by cutting or burning often comes to the attention of plastic surgeons. We report three variant cases involving insertion of paperclips, a ballpoint pen cartridge and sewing needles into the forearm. We discuss the management considerations of each case and emphasise the importance of actively addressing the underlying psychiatric problems for all instances of deliberate self-harm.

Variation in the venous pattern in the arm is common. In this study, a rare variant of the axillary vein and its association with the median cutaneous nerve of the forearm is described. In the axilla, the medial cutaneous nerve of the forearm penetrated the axillary vein, thereby creating two narrow venous channels at the site of passage. Such variations are important because a large number of diagnostic and therapeutic invasive procedures are carried out on veins. A possible mode of origin and the clinical importance of this variation are discussed.

Eleven children with forearm bowing fractures were followed up after 80 months. They were evaluated specifically for pain, function, and cosmesis. Remodeling was assessed by comparing radiographs. The mean age when the trauma occurred was 7 years 5 months. Forces longitudinal to the forearm axis are considered responsible for this injury. This paper presents the experiences of the thumb reconstruction with osteofasciocutaneous reverse flap (OFCR flap). In the period between 1987 and 2000 the OFCR flap was used in 15 patients. We tested the hypothesis that venous emptying serves as a stimulus for vasodilation in the human forearm. We compared the forearm blood flow (FBF; pulsed Doppler mean blood velocity and echo Doppler brachial artery diameter) response to temporary elevation of a resting forearm from below to above heart level when venous volume was allowed to drain versus when venous drainage was prevented by inflation of an upper arm cuff to approximately 30 mmHg. Ipsilateral fractures of the humerus and forearm are uncommon injuries in children. The incidence of compartment syndrome in association with these fractures is controversial. Reciprocal inhibition of the H-reflex in the forearm flexor muscles was studied in 11 patients with essential tremor and in 10 normal controls. Whereas patients and controls had a similar first, disynaptic phase of reciprocal inhibition, patients had a significantly reduced second phase. Patients with more severe functional impairment had more pronounced abnormalities of reciprocal inhibition. Abnormalities of reciprocal inhibition may play a role in the pathophysiology of essential tremor and probably arise from defective suprasegmental control.

Most arteriovenous malformations usually arise from pre-existing named vessels. We report an unusual variant of an arteriovenous malformation. An 18-year-old man presented with a painful swelling of the right forearm. Arteriograms suggested branches of the anterior interosseous artery were feeding the malformation. Operative findings however, revealed the presence of a persistent median artery, which was contributing branches to the swelling.

Displaced fractures of the radial head that require reduction or excision are often associated with longitudinal instability. Where possible (type II), these fractures should be stabilized with rigid internal fixation In cases in which the initial displacement reflects complete dissociation, supplemental stabilization and repair may be required. Particular attention must be paid to the posterolateral collateral ligament, the interosseous ligament of the forearm, and ligaments at the distal radioulnar joint.

Anomalies of the radial forearm flap have been reported; however, rarely does one encounter variations that jeopardize the vascular supply of the flap. In the reported case, an anomalous superficial vessel in the forearm supplied the skin island of the radial forearm flap. In addition, there were no perforating vessels from the radial artery.
