Traumatic lumbar fracture-dislocation is a rare condition. Congenital spinal deformities may facilitate the occurrence of this case. We diagnosed spina bifida occulta accompanied at the same level by a L2 anterior dislocation which resulted from a vehicle in-site traffic accident in a 35-year-old female patient. Posterior intervention was carried out during therapy, and reduction and stabilization were performed by means of a T12-L1 and L2-L3 trans-pedicular screw. A 93-year-old patient with an unstable fracture-dislocation of the glenohumeral joint and medical problems that precluded open surgery was first managed by an unsuccessful closed reduction. Temporary transarticular pin fixation then was used successfully, allowing the patient to return to early activities with minimal morbidity.

We report a displaced body of hamate fracture in the coronal plane with dislocation of the ring and little metacarpals and interposition of these metacarpals between the two hamate fragments. The diagnosis, radiographic studies and the treatment of this unusual injury are discussed.

In four missed posterior shoulder dislocations, a trial of closed reduction was successful, but severe instability required further therapy. The glenohumeral joints were immobilized by 3 to 4 percutaneously inserted threaded pins. Removal of the pins after 4 to 6 weeks was followed by physiotherapy. After a follow-up of 4 to 42 months, the functional results of this simple procedure are good and there is no recurrence. Bending of the ends of the pins, a very careful follow-up to deal with pin migration, and adequate prevention of subsequent convulsions in epileptic patients are mandatory.

An 18-year-old woman involved in a motor vehicle accident sustained a vertical shear fracture of the sacrum accompanied by unilateral dislocation of the L5-S1 joint. Evaluation of the patient's injuries was defined by computed axial tomography. Through a posterior midline incision, open reductions of the dislocated L5-S1 joint and of the sacral fracture were performed. Reduction was followed by anterior stabilization of the symphysis pubis. The patient remained neurologically intact after surgery.

Dislocation of the radial head in an adult is an uncommon injury. The mechanism of injury is considered to be due to hyperpronation of the forearm. Fracture-dislocations of the forearm are well described and involve both the radius and the ulna. A dislocation of the head of the radius with an ipsilateral fracture of the shaft of the radius seems not to have been reported; the mechanism of injury can only be postulated.

A 17-year-old boy sustained a complete fracture-dislocation of the lumbar spine. Immediately after the injury there was motor paralysis of the lower limbs; 17 hours post-injury, operative reduction and fixation were carried out. Nearly full neurological recovery was obtained.

A case of fracture-separation and dislocation of the proximal humeral epiphysis, a rare injury, is reported. The patient, a 32-month-old girl suspected of being an abused child, was reported to have fallen from her crib 2 days earlier. She had a painful, swollen, slightly ecchymotic left shoulder, which she held in the neutral position and refused to move. Superior dislocation of the fibular head associated with a fractured tibia occurred in four cases with a direct, severe trauma to the tibia. In one case there was permanent peroneal nerve damage. This injury is somewhat analogous to the fracture-dislocation of the Monteggia type in the forearm. To diagnose secondary dislocations of the fibula and treat the condition early, the knee joint should be carefully examined before and after X-ray of all fractures of the tibia.

Report of a new type of fracture dislocation of the wrist and the intercarpal joints, which has not been published until now. A perilunar dislocation was associated with fractures of the scaphoid and capitate bones as well as a fracture of the radialstyloid. Open reduction and internal fixation was followed by fracture healing and slight posttraumatic arthritis. Loss of function was minimal and working capacity not reduced.
