Technique and clinical results of a new intramedullary support nail and plate system for fixation of 3- or 4- part proximal humeral fractures in older adults.

BACKGROUND: Internal fixation of complex proximal humeral fractures (PHF) with osteoporosis is associated with a high incidence of complications. This study introduces the technique and clinical results of a novel intramedullary support nail and plate system (ISNPs) for the internal fixation of 3- or 4- part PHF in older adults. The ISNPs combines the concept of intramedullary support and dynamic fixation into a locking plate fixation system that can be applied using a minimally invasive surgical approach. METHODS: A total of 46 consecutive patients diagnosed with 3- or 4-part PHF that met th

Immediate weight bearing as tolerated (WBAT) correlates with a decreased length of stay post intramedullary fixation for subtrochanteric fractures: a multicenter retrospective cohort study.

BACKGROUND: Subtrochanteric femur fractures associate with a relatively high complication rate and are traditionally treated operatively with a period of limited weight bearing. Transitioning from extramedullary to intramedullary implants, there are increasing biomechanical and clinical data to support early weight bearing. This multicenter retrospective study examines the effect of postoperative weight bearing as tolerated (WBAT) for subtrochanteric femur fractures. We hypothesize that WBAT will result in a decreased length of stay (LOS) without increasing the incidence of re-operation. METH

Comparison between external fixation and elastic stable intramedullary nailing for the treatment of femoral shaft fractures in children younger than 8 years of age.

The main objective of this study was to compare external fixation (EF) with elastic stable intramedullary nailing (ESIN) for the treatment of femoral shaft fractures in children aged 8 or younger. Fifteen children with femoral shaft fractures treated by EF and 23 children with femoral shaft fractures treated by ESIN were retrospectively reviewed. All patients were pain free at the last follow-up, with good ranges of motion in the hip and knee. Partial and full weight bearing occurred sooner in patients treated with EF than with ESIN, although more polytrauma patients were present in the ESIN g

[Intramedullary fixation of humerus shaft fractures. An analysis of complications of 2 implants with special reference to outcome after management with the unreamed humerus interlocking nail].

INTRODUCTION: Operative treatment of humeral shaft fractures has gained a new impetus with the development of intramedullary interlocking systems for the humerus. Because of the anatomical structure of the humeral marrow cavity, a regular jamming of nail into bone is not to be expected, so that the stability of these systems needs to be achieved through an interlocking mechanism. Among the commercially available interlocking nailing systems the unreamed humeral nail (UHN) and the Seidel-nail (SHN) can be regarded as the standard implants. DESIGN: In a retrospective study comparing two groups

Delayed Rupture of Extensor Tendons of Index Finger-An Unusual Complication after Distal Radius Fracture Fixation with Intramedullary nail: A Case Report and Review of Literature.

Delayed rupture of extensor tendons of hand can occur due to a variety of reasons. Post-traumatic attrition rupture of extensor pollicis longus (EPL) tendon following distal radius fractures is well known. We report an extremely rare case of isolated rupture of both the extensor digitorum communis (EDC) and extensor indicis proprius (EIP) tendons of index finger due to intramedullary nailing without EPL rupture.

Clinical Outcomes of Single Intramedullary K-Wire Fixation with Figure-of-Eight Suture Augmentation for Ulnar Styloid Base and Distal Ulnar Metaphyseal Fractures.

Background/Objectives: The optimal fixation method for ulnar styloid base fractures remains controversial. Conventional dual K-wire fixation may be technically challenging in certain fracture configurations, whereas hook plate fixation provides rigid fixation but requires additional implant placement and surgical exposure. The purpose of this study was to evaluate the clinical and radiographic outcomes of single intramedullary K-wire fixation combined with figure-of-eight suture augmentation for ulnar styloid base and distal ulnar metaphyseal fractures. Methods: We retrospectively reviewed 60

Comparative clinical outcomes of suprapatellar intramedullary nailing vs. minimally invasive plate osteosynthesis fixation for distal tibial fractures: a retrospective cohort study.

INTRODUCTION: Distal tibial fractures are challenging injuries due to limited soft tissue coverage and complex biomechanics. Suprapatellar intramedullary nailing (IMN) and minimally invasive plate osteosynthesis (MIPO) are widely used; however, their comparative effectiveness remains controversial. This study compared perioperative parameters, functional outcomes, radiographic results, and complications to inform surgical decision-making. METHODS: This retrospective cohort study included 70 patients with distal one-third tibial fractures treated with suprapatellar IMN (n = 35) or MIPO (n = 35

Retrograde intramedullary nailing versus locked plate fixation for distal femur fractures: A systematic review and meta-analysis of randomised controlled trials only.

BACKGROUND: The optimal fixation method for distal femur fractures remains contentious. Retrograde intramedullary nailing (RIN) and locking plate fixation (PLATE) are the two principal surgical options, yet their comparative effectiveness has not been definitively established. This systematic review and meta-analysis aimed to comprehensively compare the operative, radiological, and functional outcomes of RIN versus PLATE for distal femur fractures exclusively from randomised controlled trials (RCTs). METHODS: A systematic search of MEDLINE, Embase, Scopus and Web of Science was performed from

Intramedullary fixation of proximal humerus fractures: do locking bolts endanger the axillary nerve or the ascending branch of the anterior circumflex artery? A cadaveric study.

BACKGROUND: Proximal humerus fractures are one of the most common fractures. Intramedullary locked nailing is becoming a popular alternative treatment, especially for easier fracture patterns. Although axillary nerve injury has been reported, no study has compared the safety of the proximal locking options relative to the axillary nerve and the ascending branch of the anterior circumflex artery. METHOD: Six different commercially available proximal humeral nails were implanted in 30 shoulders of 18 cadavers. After fluoroscopically guided implantation the shoulders were carefully dissected and

Minimally invasive plate osteosynthesis, open reduction and plate fixation, and intramedullary nailing for humeral shaft fractures: A network meta-analysis of randomized controlled trials.

BACKGROUND: The optimal surgical technique for humeral shaft fractures remains debated, and earlier syntheses often pooled all plate constructs, disregarding the distinction between open reduction and plate fixation (ORPF) and minimally invasive plate osteosynthesis (MIPO). This network meta-analysis compared ORPF, MIPO, and intramedullary nailing (IMN), focusing on iatrogenic radial nerve injury. METHODS: We searched PubMed/MEDLINE, Embase, Cochrane CENTRAL, and Scopus from inception through December 2024, with the PubMed search updated through January 2026, following the PRISMA network meta

Exploring the Efficacy of Computer-Assisted Navigation in Improving Lag Screw Placement and Preventing Cut-Out in Intramedullary Nail Fixation of Femoral Fractures: A Meta-Analysis.

Femoral neck fractures are a common complication treated by orthopedic surgeons. Exploring the role of computer-assisted orthopedic programs in femoral fracture treatment is of particular interest given the technological advances in computer-assisted programs in the medical field. Notably, systems such as Stryker's Adaptive Positioning Technology (ADAPT) may allow for more precision in determining the tip-apex distance (TAD) when treating intertrochanter femur fractures. Such innovations hold the potential to reduce complication rates, including the incidence of lag screw cut-out, which could
