An avulsion fracture of the second metacarpal is a rare injury often resulting from resisted wrist hyperflexion, and there is no consensus on the optimal treatment. A review of the literature reveals 20 articles documenting 25 cases of similar injuries. Of these, nine cases were initially managed conservatively, while 16 were treated surgically. Among the nine conservative cases, five (55.6%) required late surgical intervention due to unsuccessful initial treatment. In contrast, none of the 16 surgically treated cases reported poor clinical outcomes. This case involves a 23-year-old male with

This is a case report of an 85-year-old woman with osteopenia who underwent olecranon avulsion fracture repair with supplemental triceps tendon repair following a fall on an outstretched arm. The initial procedure failed due to osteoporotic bone quality and an atraumatic disruption of the olecranon fracture fixation. The patient subsequently underwent further surgical intervention with an olecranon avulsion fracture excision and a novel triceps tendon repair technique using plate augmentation and fiber tape. Surgeons may consider this novel approach as an initial treatment for elderly patients

BACKGROUND: The purpose of this study was to determine whether tolerated weight-bearing in a hard-soled shoe was noninferior to the use of a short leg cast for the treatment of a fifth metatarsal base avulsion fracture, as assessed with use of a 100-mm visual analog scale (VAS) for pain at 6 months after the fracture.
METHODS: A total of 145 patients were assessed for eligibility. Of these, 96 patients were randomly assigned to either the hard-soled shoe group (46 patients) or the cast group (50 patients). The primary outcome measure was the mean difference on the 100-mm VAS between groups at

Avulsion of the tibial spine is functionally equivalent to rupture of the anterior cruciate ligament in an adolescent athlete. It therefore presents to general orthopaedists as well as a wide variety of orthopaedic subspecialty surgeons, including traumatology sports medicine, and pediatrics. Restoration of normal knee kinematics is dependent on anatomic reduction and fixation of the avulsed fragment. Because this injury is typically sustained by the skeletally immature patient, epiphyseal fixation is ideal to avoid physeal injury, which can lead to angular limb deformity. We present a case, t

PURPOSE: Current literature lacks recommendations regarding proper fixation of tibial tubercle avulsion fractures involving the proximal tibial epiphysis (Ogden fractures). Therefore, the aim of this study was to compare isolated apophyseal screw fixation and additional fixation techniques in Ogden fractures.
METHODS: Two different types of apoepiphyseal tibial tubercle avulsion fractures were created in 40 proximal tibiae according to the modified Ogden classification: (1) Ogden type IIIA and (2) Ogden type IV. The fractures were fixed with either isolated apophyseal screws or additionally w

This study presents the first documented case of healing in the deep-sea sharpnose sevengill shark (Heptranchias perlo), a poorly understood species in the Mediterranean Sea. A juvenile specimen that sustained a traumatic mandibular fracture with extensive soft-tissue avulsion, which likely resulted from de-hooking during a previous longline capture, was retrieved dead by a bottom trawler in the southern Adriatic Sea. Gross pathological and radiographic examinations confirmed that the injury remained isolated to the mandibular region, with no abnormalities detected in the splanchnocranium or t

INTRODUCTION: Post-traumatic avulsion of the globe is a rare and uncommon pathology with serious morpho-physiological, cosmetic and aesthetic implications. The association with complex fractures of the skull increases the morbidity and complexity of the case.
CASE REPORT: We present a rare case of FSNOE complex fracture associated with avulsion of the left globe, with complete sectioning of the optic nerve and loss of vision.
DISSCUSION: The prompt diagnosis and the immediate establishment of a correct multidisciplinary treatment, led to the favorable evolution of the case. Three-dimensional

RATIONALE: The anterior cruciate ligament (ACL) is relatively rarely injured in children and adolescents, accounting for just 0.5% of all ACL rips that occur. Avulsion fractures are more common in youngsters because their ligamentous structures are known to be stronger than their physical insertion sites. Tibial eminences have been reported to be the sites of most ACL avulsions, and both cartilaginous and osteochondral avulsion fractures have been observed, whereas the latter occurs more commonly. On the other hand, femoral osteochondral avulsion fractures of the ACL in children are uncommon,

Arthroscopic procedures in the posterior compartment of the knee are technically demanding and present an inherent risk of iatrogenic neurovascular injury because of the close proximity of the popliteal neurovascular bundle. This technical note aims to enhance surgical safety by describing a series of modifications termed the "3D" technique. The technique integrates 3 simple, practical, and reproducible modifications that improve visualization and tunnel accuracy while minimizing the risk of complications using only standard arthroscopic instruments.

BACKGROUND: Tibial spine avulsion fractures are a relatively common pediatric injury that can be managed with a variety of operative techniques. No consensus exists on the optimal method of fixation, especially in patients with rotatory instability who are at a greater risk of rerupture.
INDICATIONS: Dual suspensory all-suture constructs with physeal sparing modified Lemaire lateral extra-articular tenodesis (LET) is a novel technique that may offer excellent outcomes for the management of types 2 to 4 tibial spine fractures in patients with rotatory instability.
TECHNIQUE DESCRIPTION: A lat

BACKGROUND: Isolated avulsion fractures of the humeral lesser tuberosity in adolescents with open growth plates are rare and frequently overlooked due to subtle clinical and radiographic findings. Delayed diagnosis may result in persistent internal rotation weakness and functional impairment. This report aims to highlight the diagnostic challenges and clinical course of a markedly delayed presentation of this uncommon injury.
CASE PRESENTATION: A 19-year-old male presented with persistent weakness in shoulder internal rotation. He reported a traumatic shoulder injury at the age of 15 that had

BACKGROUND: Some children and adolescents can develop persistent pain and instability following inversion injuries of the ankle. In these cases, imaging exams could reveal small bone fragments distal to the lateral malleolus. For these patients, regular conservative treatment may not be successful, requiring additional management, which can include surgical treatment. This study aimed to present the short-to-midterm functional and clinical outcomes of a series of 12 pediatric and adolescent patients who underwent ligament repair surgery due to chronic instability associated with the presence o
