Articular comminuted distal radius fractures are still a surgical challenge. The development of volar locking plate device allows a stable and anatomic restoration of the distal radius. We propose a modification of the classic anterior approach for articular fracture of the distal radius with ulnar impaction. By a single incision, we realize 2 windows to expose the medial and lateral column of the distal radius. This technique is indicated for complex articular distal radius fractures associated with an ulnar comminution. The dissection is extended superficially to the ulnar side of the carpal

OBJECTIVE: This study evaluates a proximally angulated cortical screw forming a triangular construct in lateral locking plates of comminuted intraarticular tibial plateau fractures.
METHODS: Sixteen composite tibiae were used for the fracture model and a complete articular fracture of the proximal tibia (AO 41.C2) was performed. A compression force was applied to the medial tibial plateau with an alternating, stepwise loading: 250 N-10 N-300 N-10 N-350 N-10 N-400 N. The relative motion between the different fragments of each sample was measured using a contactless, 3-dimensional digital image

We identified 11 women with a mean age of 74 years (65 to 81) who sustained comminuted distal radial and ulnar fractures and were treated by volar plating and slight shortening of the radius combined with a primary Sauvé-Kapandji procedure. At a mean of 46 months (16 to 58), union of distal radial fractures and arthrodesis of the distal radioulnar joint was seen in all patients. The mean shortening of the radius was 12 mm (5 to 18) compared to the contralateral side. Flexion and extension of the wrist was a mean of 54° and 50°, respectively, and the mean pronation and supination of the forearm

BACKGROUND: We have attempted to restore the arc of motion by considering trochlear-coronoid articulation as a complete circle during fixation of the coronoid, even for comminuted coronoid fractures with partial loss of articular cartilage (CCFPLAC), using various kinds of locking plates. Herein, we report the radiological and clinical outcomes after fixation of the basal-1 type of CCFPLAC (O'Driscoll classification) using our method.
METHODS: Thirty-one patients diagnosed with CCFPLAC were admitted between January 2012 and December 2020. Sixteen of these patients met the inclusion/exclusion

BACKGROUND: This study aims to evaluate the outcomes of using calcaneal plate in fixation of comminuted posterior wall (PW) acetabular fractures especially that have cranial (dome) or posterior extension (posterior column edge), and to evaluate its safety. To our knowledge, this is the first study that utilizes this off label implant technique in fixation of such fracture.
METHODS: Twenty-two patients enrolled in the study with a minimum follow up of one year. After reducing the PW fragments sequentially, calcaneal plate was applied, fixing its distal part at ischial tuberosity upper ends usi

Background and objectivesThe ability to maximize corpectomy cage endcap size and vertebral endplate coverage after corpectomy for lumbar burst fractures (L1-L5) is limited by the presence of lumbar nerve roots and the larger cross-sectional area of the lumbar endplates relative to the restrictive corridor for cage insertion. This work aims to provide details and clinical examples of a novel operative technique for 3-column reconstruction and stabilization of comminuted lumbar burst fractures.MethodsThrough a standard posterior midline approach and following posterior instrumentation and latera

Background Comminuted proximal humeral fractures involving the anatomical neck pose challenges in fixation stability and vascular preservation. The JUST UNIC™ (Evolutis, Briennon, France) bilboquet-inspired fixation system has been developed to address these limitations by providing metaphyseal stability with minimal periosteal disruption. This study assesses the clinical, functional, and radiological outcomes following JUST UNIC fixation in this fracture subset. Methods This retrospective single-centre case series included 25 patients with comminuted proximal humerus fractures involving the a

BACKGROUND: Fixation of radial head fractures with neck involvement presents challenges. It remains unclear whether the outcomes of head-to-neck screw fixation are similar to those of arthroplasty in this scenario. We conducted a retrospective cohort study to evaluate the clinical outcomes of these two methods for treating such fractures.
METHODS: Demographic data, fracture type, number of fragments, and concomitant injuries were recorded. Range of motion, Visual Analogue Scale for pain, Mayo Elbow Performance Score, and Quick-Disabilities of the Arm, Shoulder, and Hand at three-year follow-u

Bone adhesives provide remarkable clinical solutions in treating highly comminuted fractures that are difficult to perform surgery with metal fixation. However, no commercial bone adhesives exhibit high adhesion, strength, and osteogenic activity for instant and sustainable fixation in dynamic, wet humoral environments at weight-bearing sites. Here, phase engineering is employed to construct a homogeneous hard-soft biphasic bone adhesive (HB-PTN) with a sea urchin-inspired structure of phosphorylated polyglutamic acid (P-PGA) encapsulating tetracalcium phosphate (TTCP) (hard phase) and a visco

INTRODUCTION: Fractures of the distal femur with metaphyseal comminution and complete intra-articular involvement (AO/OTA classifications 33C2 and 33C3) present challenges for reduction and fixation. However, an optimal fixation method remains unknown. This study aimed to compare the clinical and radiographic outcomes of locking extramedullary plating (LEP) and retrograde intramedullary nailing (RIN) for complete distal femoral intra-articular fractures with metaphyseal comminution.
MATERIALS AND METHODS: Between January 2016 and May 2022, 80 patients (45 men and 35 women; average age, 56.7 y

One of the methods of repairing the damaged bone is the fabrication of porous scaffold using synergic methods like three-dimensional (3D) printing and freeze-drying technology. These techniques improve the damaged and fracture parts rapidly for better healing bone lesions using bioactive ceramic and polymer. This research, due to the need to increase the mechanical strength of 3D bone scaffolds for better mechanical performance. Akermanite bioceramic as a bioactive and calcium silicate bioceramic has been used besides the polymeric component. In this study, the porous scaffolds were designed u
