Different faces of frontal lobe epilepsy: The clinical, electrophysiologic, and imaging experience of a tertiary center.

OBJECTIVE: Frontal lobe epilepsy (FLE) is the second most common epilepsy among drug-resistant focal epilepsies. Semiologic and electrophysiologic features of FLE present some difficulties because frontal lobe seizures are brief, accompanied by complex motor activities and emotional signs. The rich connectivity of the frontal lobe with other areas leads to the rapid and widespread propagation of seizure activity, which contribute to the difficulty of evaluating the semiologic and EEG patterns of the seizure. In this study, we investigated semiologic, interictal, ictal, and postictal EEG charac…

Value Neglect: A Critical Role for Ventromedial Frontal Lobe in Learning the Value of Spatial Locations.

Whether you are a gazelle bounding to the richest tract of grassland or a return customer heading to the freshest farm stand at a crowded market, the ability to learn the value of spatial locations is important in adaptive behavior. The ventromedial frontal lobe (VMF) is implicated in value-based decisions between objects and in flexibly learning to choose between objects based on feedback. However, it is unclear if this region plays a material-general role in reward learning. Here, we tested whether VMF is necessary for learning the value of spatial locations. People with VMF damage were comp…

Improvement Of Frontal Lobe Dysfunction And White Matter Integrity By rTMS In Treatment-Resistant Depression.

AIM: The impairment experienced by many individuals with depression is closely related to the cognitive symptoms of the disorder. Repetitive transcranial magnetic stimulation (rTMS) is a noninvasive brain stimulation method providing a promising technique for improving cognitive symptoms in treatment-resistant depression (TRD). In the present study, we investigated whether a relationship exists between improvements in frontal lobe dysfunction induced by rTMS and improvement of white matter integrity revealed by diffusion tensor imaging (DTI) in TRD patients receiving rTMS treatment. METHODS: …

A Novel Approach to Prenatal Measurement of the Fetal Frontal Lobe Using Three-Dimensional Sonography.

OBJECTIVE: While prenatal 3D ultrasonography results in improved diagnostic accuracy, no data are available on biometric assessment of the fetal frontal lobe. This study was designed to assess feasibility of a standardized approach to biometric measurement of the fetal frontal lobe and to construct frontal lobe growth trajectories throughout gestation. STUDY DESIGN: A sonographic 3D volume set was obtained and measured in 101 patients between 16.1 and 33.7 gestational weeks. Measurements were obtained by two independent raters. To model the relationship between gestational age and each fronta…

Mind-Matter Interactions and the Frontal Lobes of the Brain: A Novel Neurobiological Model of Psi Inhibition.

CONTEXT: Despite a large literature on psi, which encompasses a range of experiences including putative telepathy (mind-mind connections), c … at may inhibit psi. DESIGN: We used mind-matter interactions to study psi in two participants with frontal lobe damage. The experimental task was to influence numerical output of a Random Event Generator translated into movement of an arrow on a computer screen to the right or left. Brain MRI was analyzed to determine frontal volume loss. RESULTS: The primar …

FDG-PET in the diagnosis of complex partial status epilepticus originating from the frontal lobe.

Complex partial status epilepticus of frontal origin can manifest as nonconvulsive behavioral symptoms that mimic psychiatric illness and, thus, may elude timely diagnosis. The diagnosis can be further delayed by absence of ictal activity on scalp electroencephalography when the ictal origin is orbitofrontal or mesial frontal. We describe the case of a 51-year-old woman with clinically subtle complex partial status epilepticus of left orbitofrontal origin, lacking any clear ictal pattern on the electroencephalogram, who was finally diagnosed using positron emission tomography with [(18)F]fluor

Legionnaires' disease with hypoperfusion in the cerebellum and frontal lobe on single photon emission computed tomography.

A 59-year-old man was admitted for further investigation of headache. Neurological examination revealed memory loss, disorientation, and bilateral intention tremor. Legionella pneumophila antigen was detected in the urine. Brain magnetic resonance diffusion-weighted images showed marked hyperintensity in the splenium of the corpus callosum without other abnormalities. Single photon emission CT with Tc-99m hexamethyl-propyleneamine oxime showed multi-focal hypoperfusion in the brain, involving mainly the cerebellum and frontal lobe. This is the first report demonstrating cerebellar and frontal …

[An apraxia of eyelid closure in association with frontal lobe atrophy in a patient with amyotrophic lateral sclerosis].

A 57 year-old woman with amyotrophic lateral sclerosis (ALS) and an apraxia of eyelid closure was reported. Her first symptom was muscle weakness in the right arm. Since neurological examination showed mainly upper motor neuron sign with neurogenic pattern in the needle electromyograms of the tongue and limb muscles, she was diagnosed as ALS. Two years and 6 months later from onset, she showed an inability to close her eyelids voluntarily or on command with normal reflex closure. MRI of the brain showed atrophy in the frontal lobe and anterior of the temporal lobe, where 123 I-IMP SPECT reveal…

MRI parcellation of the frontal lobe in boys with attention deficit hyperactivity disorder or Tourette syndrome.

Dysfunction of frontal-striatal-thalamic-frontal circuitry has been hypothesized to underlie both attention deficit hyperactivity disorder (ADHD) and Tourette syndrome (TS). Several research groups have therefore used anatomic magnetic resonance imaging (aMRI) to obtain volumetric measurements of subregions of the frontal lobe in these disorders. Most previous studies have relied on subparcellation methods that utilize callosal landmarks to derive subregions of the frontal lobe. In contrast, we present here an investigation of frontal lobe morphometry in ADHD and TS based on a reliable frontal…

Effect of COMT Val108/158 Met genotype on frontal lobe function and risk for schizophrenia.

Abnormalities of prefrontal cortical function are prominent features of schizophrenia and have been associated with genetic risk, suggesting that susceptibility genes for schizophrenia may impact on the molecular mechanisms of prefrontal function. A potential susceptibility mechanism involves regulation of prefrontal dopamine, which modulates the response of prefrontal neurons during working memory. We examined the relationship of a common functional polymorphism (Val(108/158) Met) in the catechol-O-methyltransferase (COMT) gene, which accounts for a 4-fold variation in enzyme activity and dop

[A case of crossed aphasia with echolalia after the resection of tumor in the right medial frontal lobe].

We report a right-handed woman, who developed a non-fluent aphasia after resection of astrocytoma (grade III) in the right medial frontal lobe. On admission to the rehabilitation department, neurological examination revealed mild left hemiparesis, hyperreflexia on the left side and grasp reflex on the left hand. Neuropsychologically she showed general inattention, non-fluent aphasia, acalculia, constructional disability, and mild buccofacial apraxia. No other apraxia, unilateral spatial neglect or extinction phenomena were observed. An MRI demonstrated resected areas in the right superior fron…

Quantification of pathological lesions in the frontal and temporal lobe of ten patients diagnosed with Pick's disease.

The densities of Pick bodies (PB), Pick cells (PC), senile plaques (SP) and neurofibrillary tangles (NFT) in the frontal and temporal lobe were determined in ten patients diagnosed with Pick's disease (PD). The density of PB was significantly higher in the dentate gyrus granule cells compared with the cortex and the CA sectors of the hippocampus. Within the hippocampus, the highest densities of PB were observed in sector CA1. PC were absent in the dentate gyrus and no significant differences in PC density were observed in the remaining brain regions. With the exception of two patients, the den

Frontal lobe shortening in second-trimester fetuses with trisomy 21: usefulness as a US marker.

PURPOSE: To determine whether the frontal lobe is disproportionately smaller than normal in second-trimester fetuses with Down syndrome by using prenatal ultrasonographic (US) measurements of the frontothalamic distance (FTD). MATERIALS AND METHODS: The FTD, measured from the inner table of the frontal bone to the posterior margin of the thalamus, was measured in 43 fetuses (mean gestational age, 17.2 weeks +/- 1.3 [standard deviation]; range, 15.0-20.4 weeks) with chromosomally proved trisomy 21 and in 160 chromosomally normal fetuses (mean gestational age, 17.1 weeks +/- 1.5; range, 14.5-22…

The sources of visual information to the primate frontal lobe: a novel role for the superior parietal lobule.

Reaching movements are performed in order to bring the hand to targets of interest. It is widely believed that the distributed cortical network underlying visual reaching transforms the information concerning the spatial location of the target into an appropriate motor command. Modern views decompose this process into sequences of coordinate transformations between informational domains. The set of cortical areas and pathways by which the information on target location is relayed from the visual areas of the occipital lobe to the motor areas of the frontal lobe have, so far, been poorly unders…

Inhibition and the control of behavior. From Gall to Freud via Phineas Gage and the frontal lobes.

In his On the Functions of the Brain and Each of Its Parts, Franz Joseph Gall proposed that significant behaviors resulted from certain inde … e similarly deficient. The lack is seen most clearly in the experimental and clinical literature on frontal lobe function, especially in relation to the kinds of changes seen in the Gage case. The deficiencies of sensory-motor physiology are also to be found in Freud's model of the mental apparatus and in his concept of inhibition via a lateral cathexis. In th …

Apraxia of the left limbs in a case of callosal disconnection: the contribution of medial frontal lobe damage.

A 40 year old right-handed woman suffered hemorrhage from an anterior cerebral artery aneurysm. CT scan showed destruction of the anterior two thirds of the corpus callosum, both cingulate gyri, and the white matter underlying the medial frontal lobes. The left limbs displayed apraxia on verbal command, in imitation, and in actual object use. There were antagonistic actions of both hands, and the patient accused her left hand of disobeying her. We interpret the leftsided apraxia as resulting from a lack of interhemispheric transfer combined with a deficit in ipsilateral motor control stemming …

[Frontal lobe syndromes in (pre-)senile dementias of the Alzheimer type: a correlative statistical study].

A group of gerontopsychiatric patients suffering from mild to severe dementia was studied. Clinical instruments were a psychological assessment system and a battery of tests. Structural changes of the brain were detected by computerized tomography. A number of correlations between clinical and morphological variables was found. The statistical analysis of most of the variables correlating with the CT determinants of the frontal lobe (Huckman-number, width of the frontal interhemispheric fissure) revealed plausibility consistent with the classical frontal lobe syndrome. The relations of the rem…

Automated Speech Analysis to Differentiate Frontal and Right Anterior Temporal Lobe Atrophy in Frontotemporal Dementia.

BACKGROUND AND OBJECTIVES: Frontotemporal dementia (FTD) includes behavioral-variant FTD (bvFTD) with predominant frontal atrophy and semantic behavioral-variant FTD (sbvFTD) with predominant right anterior temporal lobe (rATL) atrophy. These variants present diagnostic challenges because of overlapping symptoms and neuroanatomy. Accurate differentiation is crucial for clinical trial inclusion targeting TDP-43 proteinopathies. This study investigated whether automated speech analysis can distinguish between FTD-related rATL and frontal atrophy, potentially offering a noninvasive diagnostic too
