[Pott's puffy tumor].

Between the complications of frontal sinusitis orbital or intracranial are the most frequent encountered (meningitis, abscesses and empyemas). All are secondary to thrombophlebitis of veins communicating the intracranial cavity with the frontal sinus. Frontal osteomyelitis secondary to sinusitis, the so-called Pott's puffy tumor, is a much more rare aftermath in the antibiotic epoch. Pott's puffy tumor must be suspected in patients with frontal headache followed by frontal oedema. Concerning the diagnosis clinical suspicion is essential and must be settled throughout computerized tomography an…

[History of frontal sinus surgery and current view of the problem. Part 1].

Surgical treatment of inflammatory diseases of the frontal sinus is one of the biggest challenges of modern otorhinolaryngology. Close proximity of the frontal sinus and frontal sinus drainage pathways to the skull base, the orbit and the anterior ethmoid artery, great limitations with its visualization and instrumentation, and high risk of the frontal recess scarring cause difficulties in either endoscopic or external approaches to the frontal sinus. At the same time endoscopic approach to the frontal sinus is considered as preferred method of frontal sinusitis surgical treatment by majority …

Pott's puffy tumor treated by endoscopic frontal sinusotomy.

Pott's puffy tumor is a serious extracranial complication of frontal sinusitis. The formation of this entity is facilitated by the close anatomic relationship between the paranasal sinuses and the frontal bone. Furthermore, the rich diploic venous drainage of the region enhances the spread of the infection. We report on successful functional endoscopic frontal sinusotomy in a series of four cases of Pott's puffy tumor following acute frontal sinusitis, between the years 1994 and 1997. We emphasize the advantages of this approach over the external approaches as follows: the technique treats the…

[Unilateral frontal sinus aspergillosis: the combined endoscopic and mini-trephination approach].

OBJECTIVE: Aspergillosis is a common fungal infection of the paranasal sinuses. Localization in the frontal sinus is usually secondary to involvement of one of the other sinuses. Isolated frontal sinus aspergillosis is rare and only 5 cases are described in literature. CLINICAL CASE: We report a case of a patient with a frontal sinus aspergilloma associated with ipsilateral chronic ethmoidal and maxillary sinusitis, successfully treated with a combined endoscopic and mini-trephination approach "Lemoyne technique". DISCUSSION AND CONCLUSION: The endoscopic approach to the frontal sinus is con

Personal identification using the frontal sinus.

The frontal sinuses are known to be unique to each individual; however, no one has tested the independence of the frontal sinus traits to see if probability analysis through trait combination is a viable method of identifying an individual using the frontal sinuses. This research examines the feasibility of probability trait combination, based on criteria recommended in the literature, and examines two other methods of identification using the frontal sinuses: discrete trait combinations and superimposition pattern matching. This research finds that most sinus traits are dependent upon one ano

Sinus complications of frontal craniotomy.

Frontal sinusitis, or inflamatory complications of sinusitis, may occur as a consequence of frontal craniotomy when the sinus has been entered during the craniotomy. This may happen particularly during surgery to treat an aneurysm or to excise a tumor in the sellar area. These complications may also occur if the sinus is entered during routine craniotomy when the sinus is high or asymmetrical, or during reduction of frontal bone fractures. This presentation describes the etiology of such inflammatory complications and describes the approach to treatment of such complications, as well as outlin…

[Subdural empyema complicating sinusitis].

Paranasal sinusitis complicated by subdural empyema (SDE) in a 14-year-old boy is described. He presented with fever, headache and vomiting. X-rays revealed left maxillary and bilateral frontal sinusitis. While in the emergency room he had seizures; CT-scan showed an air leak adjacent to the right frontal sinus. Despite intravenous antibiotics, left hemiparesis developed and repeat CT showed interhemispheric SDE. Frontal craniotomy was performed and a large amount of purulent material was drained. Recovery followed 4 weeks of intravenous antibiotic treatment without neurologic sequelae. SDE is…

Computed tomographic assessment of a new nonsurgical sinus trephination technique using a medical bone marrow drill.

The objective of this study was to determine the feasibility of trephination of the frontal sinus and injection of antifungal cream using a medical bone marrow drill in dogs. Results were compared with frontal sinus trephination using a standard surgical technique. Bilateral trephination of the frontal sinuses was carried out in the heads of 11 cadavers using a medical bone marrow drill and a surgical bone chuck. The time taken to carry out the procedure using both techniques was compared. Before and after injection of antifungal cream into the frontal sinuses, computed-tomography (CT) scannin

[Acute sinusitis].

The sinusitis is the term, which describes inflammation process of mucous membrane of these sinuses. Generally inflammation spreads from the nose to the paranasal sinus, so rhinosinusitis is the common name of disease. In classification of sinusitis two aspects are important--duration of signs and symptoms and changes in mucous membrane of sinus. In acute sinusitis symptoms withdraw after treatment, and mucous membrane comes back to the normal state. The most frequent bacteria's responsible for sinusitis are aerobes and among them: Streptococcus pneumoniae, Haemophilus influenzae, Moraxella ca

The Horizon Sign and Frontal Bar: Two Topographic Landmarks to Confirm Endoscopic Frontal Sinusotomy.

Confirming a thorough dissection of the frontal sinus during endoscopic sinus surgery can be challenging, and some surgeons would benefit from reliable topographic landmark identification to ensure completion of this sinus dissection. We defined (1) the "horizon sign" as the curvilinear shadow of the posterior table cast superiorly upon the anterior table of the frontal sinus at the acute angle of their meeting point and (2) the "frontal bar" as a sagittal septation at the union of the anterior/posterior tables. A cadaveric study, followed by an intraoperative consecutive case series, was perf

Frontal sinus: is it a predictor for vertical malocclusions?

The aim of the present study was to evaluate 3D frontal sinus morphology in consideration with different vertical facial developments. The study was conducted with the pre-treatment CBCT images of 87 patients, who were divided into 3 groups according to the sum of the posterior angle. The frontal sinus measurements were performed on the axial, sagittal, and coronal sections of the CBCT images. Craniofacial measurements were performed on the lateral cephalograms, obtained from the CBCT scans. Normal distribution of the data was confirmed by the Kolmogorov-Smirnov test. ANOVA, followed by post h

Frontal sinus stents.

OBJECTIVES: The formation of scar tissue, synechiae, or osteogenesis in the narrow frontal outflow tract after instrumentation of the frontal sinus has led to attempts to enlarge the frontal duct or stenting. STUDY DESIGN: Prospective pilot study. RESULTS: Sixty-four Freeman frontal sinus stents were used to maintain patency in 46 patients. Stents were inserted using an endoscopic technique in 26 sinuses containing polyps, 20 with pansinusitis, and 18 cavities with stenosis of the frontal duct. External incisions with frontal sinusotomy were required to remove the stenosis and insert the ste

[Digital volume tomography in the diagnosis of the surgical anatomy of the frontal recess area].

Frontal recess anatomy can be very complex, with accessory cells, such as frontal bulla, agger nasi, suprabulla, and frontal ethmoidal (Kuhn) cells, which may lead to obstruction of the frontal sinus. Digital volume tomography of paranasal sinuses used in this investigation revealed the rate of types of development of various anatomic structures and their correlation with frontal sinusitis in patients with (a study) and without the latter (a control group).

Coexistence of frontal sinus hypoplasia with maxillary sinus hypoplasia: a radiological study.

The goal of this study was to determine whether frontal sinus hypoplasia coexists with maxillary sinus hypoplasia. Analyzing paranasal CT scans retrospectively, we included 86 patients who had a hypoplastic maxillary sinus at least on one side and 80 patients with bilateral normal maxillary sinuses (control group). We classified hypoplastic maxillary sinuses using the classification system previously defined by Bolger et al. (Otolaryngol Head Neck Surg 103(5):759-765, 1990). We classified the frontal sinuses as aplastic, hypoplastic, medium-sized, and hyperplastic; as previously defined by Gue

Does nasal nitric oxide come from the sinuses?

OBJECTIVE: The purpose of this study was to assess nitric oxide (NO) output by the nose and sinuses. METHOD: In one volunteer, the osteomeatal complex and sphenoethmoidal recess were occluded to isolate the nose from the sinuses. The antrum and frontal sinus were each punctured by two catheters and irrigated with air at constant flow. Nitric oxide output and its rate of accumulation in the absence of air flow were measured in each sinus and in the adjacent nasal cavity. RESULTS: Prior to ostial occlusion, NO output in the nose was 96 nL/min. It decreased by 12% after blockage of all of the o

Nasal and paranasal sinus endoscopy. A diagnostic and surgical approach to recurrent sinusitis.

Our endoscopic concept of the diagnosis and surgical treatment of recurrent sinusitis is based on Messerklinger's finding that almost all infections of the frontal and maxillary sinuses are rhinogenic. They are secondary to infection foci in their prechambers in the anterior ethmoid, especially in the ethmoidal infundibulum and the frontal recess, spreading from there to the dependent larger sinuses. Consequently, our functional endoscopic sinus surgery is aimed at these infection foci in the ethmoid, clearing mucosal contact areas, stenotic clefts and diseased cells. Ventilation and drainage

[The incidence and location of inflammatory paranasal sinus lesions in CT].

Several recent studies have labelled the anterior ethmoid as the main source of chronic and/or recurrent sinusitis in the entire paranasal sinus system. In an attempt to identify other possible patterns of developing sinusitis, 1,345 computed tomographies of neurological and neurosurgical patients with no previous history of paranasal sinusitis were evaluated. These scans were made between 1985 and 1989 in the Neurosurgical Clinic of the University of Freiburg. Seventy-five of these studies revealed evidence of inflammatory changes. In 57 scans maxillary sinus disease was evident, while in 46
