Gait deviations in patients with juvenile rheumatoid arthritis.

The purpose of this study was to describe quantitatively the gait patterns of patients with juvenile rheumatoid arthritis (JRA). Thirty children with JRA and 30 healthy children were evaluated using a computerized gait analysis system. Time-distance characteristics and joint angle excursions were studied. The subjects with JRA walked with significantly decreased velocity, cadence, and stride length. We found no significant difference in step width nor in the percentage of time spent in each phase of the gait cycle. The anterior pelvic tilt of subjects with JRA was significantly increased throu…

The study and correction of human gait by electrical stimulation.

To gain a better understanding of the functions that the calf and vastus muscles perform in the human walking gait the author systematically increased the contractions of these muscles separately and in combination by applying Functional Electrical Stimulation (FES) to them, during walking tests performed by a subject with nonpathological gait, and a patient with a hemiplegic gait. A four-channel stimulator was used with foot switch activated control systems, which accurately sequenced the FES pulses and timed them in relation to the footswitch contacts. In normal gait FES applied to the calf …

Clinical experience with the reciprocal gait orthosis in myelodysplasia.

Ambulation has always been difficult for patients with myelodysplasia; however, introduction of the reciprocal gait orthosis seemingly offered increased potential as an ambulatory aid. Review of 41 patients with myelodysplasia treated with the reciprocal gait orthosis revealed that 78% reached community or household ambulator status, a marked improvement over previous results with traditional hip-knee-ankle-foot orthoses.

CSF shunts for dementia, incontinence, and gait disturbance.

From a review of our experience in the past 4 years and of the literature generally, the following comments can be made about selecting patients with idiopathic NPH for a shunt procedure. (a) In the clinical presentation, either significant gait difficulty or the full triad of dementia, ataxia, and incontinence should be present. If dementia occurred first or is the major symptom, shunting may not improve the patient. (b) A CT scan with periventricular low density and/or small sulci along with expansion of the entire ventricular system (especially the temporal horns) is strongly associated wit…

Relationship of hyperextended gait pattern of chondromalacia patellae.

A specific hyperextended gait pattern is seen in patients with chondromalacia patellae. A new program designed to correct this gait impairment was evaluated by a clinical study. The findings of the study suggest a direct relationship between the amount of hyperextension and clinical severity of symptoms. When the patient's gait deviation is corrected, the symptoms of pain and effusion are alleviated.

Predictive Neuromechanical Simulation Explains Gait Biomechanics in Obesity.

Individuals with obesity exhibit gait adaptations including reduced early-stance knee flexion, altered muscle coordination, slower preferred walking speeds, and shorter step lengths. Although these features are well documented, the mechanisms by which obesity-related physiological changes produce these patterns and influence knee joint loading relevant to osteoarthritis (OA) remain unclear. This study used predictive neuromechanical simulation to examine how musculoskeletal changes and movement objectives interact to generate obesity-associated gait patterns and tibiofemoral loading. Predictiv…

Parkinsonian gait improvement through vibratory stride parameter feedback.

BACKGROUND: Hypokinetic gait increases morbidity and mortality of persons with Parkinson's disease and constitutes a major target symptom for therapy. Feedback on gait quality, e.g. by a physiotherapist, improves stride length and reduces shuffling. Wearable devices measuring gait parameters and providing feedback could improve parkinsonian gait during everyday life when no physiotherapist is available. Here, we report on the efficacy of a new device providing discreet vibratory feedback upon decreased stride length and increased shuffling. METHODS: 33 persons with Parkinson's disease, in two…

Spinopelvic parameters during gait revealed through biplane radiography.

BACKGROUND: Spinopelvic alignment impacts hip mechanics and influences hip morphologies in healthy populations. Pelvic incidence (PI) and sacral slope (SS) are spinopelvic parameters that influence pelvic orientation, but their impact on gait is less understood. The objectives of this study were to investigate the relationships of PI with dynamic SS and hip kinematics during gait, and to identify sex-based differences in PI and SS. METHODS: Synchronized biplane radiographs of the hip were collected from 24 participants during walking, and subject-specific 3D bone models were created from CT i…

Differential gait features across Parkinson's disease clinical subtypes.

BACKGROUND: Clinical subtypes in Parkinson's disease including non-motor manifestations may be more beneficial than subtypes based upon motor manifestations alone. Inclusion of gait metrics may help identity targets for rehabilitation and potentially predict development of non-motor symptoms for individuals with Parkinson's disease. This study aims to characterize gait differences across established multi-domain subtypes. METHODS: "Motor Only", "Psychiatric & Motor" and "Cognitive & Motor" clinical subtypes were established through motor, cognitive, and psychiatric assessment. Walking was ass…

IMU-Based quantitative assessment of stroke from gait.

Gait impairment, which is commonly observed in stroke survivors, underscores the imperative of rehabilitating walking function. Wearable inertial measurement units (IMUs) can capture gait parameters in stroke patients, becoming a promising tool for objective and quantifiable gait assessment. Optimal sensor placement for stroke assessment that involves optimal combinations of features (kinematics) is required to improve stroke assessment accuracy while reducing the number of sensors to achieve a convenient IMU scheme for both clinical and home assessment; however, previous studies lack comprehe…

Perceiving intimidation through kinematic cues in men's gait.

Male intrasexual competition has shaped the evolution of sexually dimorphic traits, influencing both physical and cognitive mechanisms for assessing physical dominance and formidability. While previous research has examined static visual cues such as height and upper body musculature to male formidability, the role of dynamic cues remains underexplored. We used 3D motion capture to investigate biomechanical markers that drive perceptions of male physical dominance. Fifty-two male participants' walking gaits were recorded, and their upper-body strength, anthropometric measures, and self-reporte…

Defining trunk kinematics associated with pregnant waddling gait.

A common gait pattern during pregnancy is waddling gait with increased step width, which is clinically identified by trunk sway, but the specific correlates to trunk sway remain understudied. Existing research has not kinematically validated the combination of movements as a unified waddling gait pattern. In this study we examined the relationships among trunk kinematic characteristics, step width, and balance control. Twenty-three pregnant individuals were assessed every four weeks between 18 and 34 weeks of gestation. Participants completed a quiet standing balance test and treadmill walking…

Does Lower-Limb Tendon Structure Influence Walking Gait?

BACKGROUND: Within the exploration of human gait, key focal points include the examination of functional rockers and the influential role of tendon behavior in the intricate stretch-shortening cycle. To date, the possible relationship between these two fundamental factors in the analysis of human gait has not been studied. Therefore, this study aimed to analyze the relationship between the morphology of the patellar and Achilles tendons and plantar fascia with respect to the duration of the rockers. METHODS: Thirty-nine healthy men (age: 28.42 ± 6.97 years; height: 173 ± 7.17 cm; weight: 67.7…

Challenges in Quantifying Heel-Lift During Spacesuit Gait.

INTRODUCTION: Heel-lift is a subjectively reported fit issue in planetary spacesuit boot prototypes that has not yet been quantified. Inertial measurement units (IMUs) could quantify heel-lift but are susceptible to integration drift. This work evaluates the use of IMUs and drift-correction algorithms, such as zero-velocity (ZVUs) and zero-position updates (ZPUs), to quantify heel-lift during spacesuited gait.METHODS: Data was originally collected by Fineman et al. in 2018 to assess lower body relative coordination in the spacesuit. IMUs were mounted on the spacesuit lower legs (SLLs) and spac…

Measuring gait speed to better identify prodromal dementia.

Slow gait speed has been shown to predict incident dementia and cognitive decline in older individuals. We aimed to summarize the evidence concerning the association of slow gait speed with cognitive decline and dementia, and discuss the possible shared pathways leading to cognitive and motor impairments, under the unifying hypothesis that body and mind are intimately connected. This is a scoping review supported by a systematic search of the literature, performed on PubMed and Web of Science. Longitudinal studies providing information on the role of gait speed in the prediction of cognitive d…
