A 30-year-old man experienced ocular motility impairment and exophthalmos immediately after a total thyroidectomy for papillary thyroid carcinoma at an external hospital. Two months later, 131 I therapy exacerbated his ocular symptoms. He presented to our ophthalmology department for evaluation, where a 68 Ga-FAPI PET/MR scan revealed active thyroid eye disease (TED) and residual tumor lesions. Based on these findings and clinical evaluation, a multidisciplinary team formulated and implemented an effective treatment plan. Malignant peritoneal mesothelioma is a rare malignancy that may manifest as extraperitoneal metastasis. We describe the findings of 68 Ga-FAPI and 18 F-FDG PET/CT in a case of malignant peritoneal mesothelioma with widespread muscle metastasis in a butterfly-shaped distribution pattern. We found that FDG and FAPI uptake in the peritoneal tumor and its spread were inhomogeneous. Notably, 68 Ga-FAPI PET/CT more clearly showed relevant lesions compared with 18 F-FDG PET/CT.

Cancer-associated fibroblasts that overexpress fibroblast activation protein (FAP) are enriched in many epithelial carcinomas and in hematologic neoplasms. A 61-year-old man presented with pneumonia and myocardial infarction on a background of diabetes and end-stage renal failure. Methicillin-resistant Staphylococcus aureus bacteremia was confirmed in the setting of ongoing pyrexia. The patient was referred for a 67Ga study, which revealed Ga-avid active disease in the pericardial effusion. This was localized further on CT/MRI and coronary angiography to a dissecting right coronary artery aneurysm, which was treated with surgical repair and long-term antibiotic treatment.

An 8-year-old girl with hypergammaglobulinemia showed an abnormal 67gallium accumulation in the orbits and parotid glands. Although she did not have any subjective siccant complaints, reported typical histopathological and sialographic changes suggesting Sjögren's syndrome (SjS) were observed in the salivary glands. Gallium scintigram might be a valuable and non-invasive diagnostic tool in the diagnosis of children with SjS without sicca symptoms.

Data are limited on the localization of Ga-67 in primary or metastatic adrenal cortical carcinoma. We report the localization of Ga-67 to pathologically confirmed adrenal cortical carcinoma metastatic to the lung. A review of the literature revealed four patients have previously been reported to have metastatic adrenal cortical carcinoma detected on Ga-67 scan. Gallium imaging may be useful in the evaluation of patients with adrenal cortical carcinoma. SPECT imaging should further improve lesion resolution and localization.
