BACKGROUND: Eosinophilic oesophagitis (EoE) is an increasingly common cause of dysphagia in both children and adults, as well as one of the most prevalent oesophageal diseases with a significant impact on physical health and quality of life. We have provided a single comprehensive guideline for both paediatric and adult gastroenterologists on current best practice for the evaluation and management of EoE.
METHODS: The Oesophageal Section of the British Society of Gastroenterology was commissioned by the Clinical Standards Service Committee to develop these guidelines. The Guideline Developmen…

BACKGROUND: Intussusception is the common acute abdomen in children with difficult clinical diagnosis. The routine ultrasound has recently been proposed as the initial diagnostic modality with high accuracy, but is not available for 24 h by gastroenterologists. We aimed to evaluate the validation of bedside ultrasound for intussusceptions performed by pediatric emergency physicians with ultrasound training during the night or holiday.
METHODS: A retrospective study was conducted in children with suspected intussusceptions when routine ultrasounds by gastroenterologists were not available over…

BACKGROUND AND OBJECTIVE: Overuse injuries of the hand, wrist, forearm, and shoulder are common among endoscopists and may be from repetitive pinching and gripping forces or awkward posturing. In this pilot study, we evaluated distal upper-extremity musculoskeletal load during colonoscopy (1) to confirm the feasibility of performing ergonomic measurements in endoscopists and (2) to identify tasks that may contribute to overuse injuries.
DESIGN AND SUBJECTS: Three experienced gastroenterologists were evaluated during 3 colonoscopies each.
SETTING: Veterans Affairs Medical Center, San Francisc…

Open-access endoscopy has recently gained popularity in referring patients for endoscopic procedures. Retrospective (looking into patients' medical files) and prospective studies (using 2 different questionnaires, and evaluating a selection system) were conducted, comparing the efficacy of open-access endoscopy for patients referred by either family practitioners or gastroenterologists. In the retrospective study, 673 patients (mean age 48.8 years, male-58%) underwent upper gastrointestinal endoscopy. The main indications for upper endoscopy were epigastric pain (71%), heartburn (18%) and vomi…

Anorectal manometry is one of the motility tests in children performed by pediatric gastroenterologist. It evaluates the motility function of anorectal tract. It is helpful for diagnosing children with constipation, rectal hypersensitivity, fecal incontinence, Hirschsprung disease, anal achalasia, and anorectal malformations. The most common indication to perform anorectal manometry is to diagnose Hirschsprung disease. It is a safe procedure. This paper discusses recent advances and reviews on anorectal motility disorders in children.

The management of GI hemorrhage has undergone tremendous evolution in recent decades. Once commonly managed by surgeons, the almost continuous introduction of new technologies and pharmacotherapies has dramatically improved clinicians' ability to identify and control sources of bleeding without surgery. Although a gastroenterologist can successfully manage most cases of GI hemorrhage endoscopically, surgical consultation remains an important consideration for the emergency physician in selected cases.

Many steps in the diagnostic workup of the patient with liver disease or surgical jaundice can be taken by the general practitioner with subsequent referral on to a gastroenterologist or surgeon to complete the diagnostic process. The principal objective in patients with parenchymal liver disease is to make an early diagnosis and initiate early treatment so that progression to cirrhosis and its complications can be avoided. Maintaining an awareness and knowledge of the various symptoms and signs of liver disease will aid in this objective.

BACKGROUND: Transoral incisionless fundoplication (TIF) is an FDA-approved endoscopic procedure for managing gastroesophageal reflux disease (GERD). Despite growing adoption and supporting evidence, significant variability exists in clinical practice. The study aimed to characterize current TIF practice patterns among U.S.-based gastroenterologists and surgeons.
METHODS: A 33-question web-based survey was distributed to 305 U.S. physicians that perform TIF. The survey assessed endoscopist demographics, pre-procedure evaluation, procedural techniques, and post-procedure care. Responses were co…

BACKGROUND AND AIMS: Inefficiencies in the endoscopy suite cause frustration for physicians, hospital administrators, staff, and patients. Turnover time (TOT), the time between one case ending and another beginning, is subjectively disproportionate between various team members. We aimed to define perceptions of TOT and target steps within the process to improve efficiency.
METHODS: This was a prospective cohort study at a tertiary center outpatient endoscopy unit. Phase I aimed to identify the TOT process components based on time stamps in the electronic medical record (N = 686). We defined g

INTRODUCTION AND IMPORTANCE: Rectovaginal fistula is a complication that may occur due to rectal injury during vaginal reconstructive surgery. To prevent these complications, the recognition of the injury is an important factor so that primary repair can be done. The primary repair can reduce the risk of complications such as fistula formation, and also reduce the physical and psychological impact on the patient.
CASE PRESENTATION: A 33-year-old woman, came with a chief complaint of fecal leakage from the vagina and abdominal pain three months before admission with a history of vaginal recons

Gastrointestinal (GI) endoscopy is performed to aid in diagnosing and treating several GI diseases. Although adult GI endoscopy has been applied in children, there are several differences regarding the technique and preparation. In this article, we review the preparation for pediatric GI endoscopy. In addition to specific considerations for each procedure, the preparation requires effective communication and collaboration from multidisciplinary health care providers, particularly gastroenterologists, endoscopic nurses, and anesthesia care teams. This article highlights key considerations for p…

Esophageal pH monitoring via wireless probes is used to evaluate chest pain and atypical symptoms and diagnose gastroesophageal reflux. These probes are commonly placed during esophagogastroduodenoscopy performed by gastroenterologists in an ambulatory anesthesia setting. Dislodgment and aspiration of these probes can cause morbidity, require surgical removal, and involve the anesthesia provider in prolonged emergency care. We present a case of a probe dislodgment where aspiration was avoided and describe how retrieval of this device is different from typical hypopharyngeal foreign body remova…
