Staffing levels and the use of physical restraints in nursing homes: a multicenter study.

There is an unclear relation between staffing levels and the use of physical restraints in nursing homes (NHs). A survey design was used in 570 older adults (median age = 86; 77.2% women), living on 23 wards within seven NHs. Restraint use was high (50% of residents, of which 80% were restrained on a daily basis). Multivariate analysis was conducted at the level of the individual wards. Neither staff intensity nor staff mix was a determinant of restraint use. Bathing dependency, transfer difficulties, risk for falls, frequent restlessness/agitation, and depression were independent predictors o

Twice in need of care: a transcultural nursing analysis of elderly Mexican Americans.

In sum, elderly Mexican Americans residing in an urban area were identified as a population at high risk for chronic conditions associated with age and were also at risk for health problems related to migrant status, poverty-level incomes, and low levels of education and nutrition. They are in need of care, i.e. assistance, succorance, nurturance, stress alleviation, and support in a stressful urban environment in which they encounter barriers of language, transportation, eligibility requirements, and self-perceptions of inferior ethnic identify. The family and kin groups are most important bu

Establishment of nursing needs scale for elderly patients with urinary incontinence and its reliability and validity test.

OBJECTIVE: Urinary incontinence significantly impacts the quality of life of elderly patients, yet there is a lack of specific assessment tools for their nursing needs in the Chinese healthcare context. To develop a nursing needs scale for elderly patients with urinary incontinence and test its reliability and validity, aiming to provide a quantitative assessment tool for geriatric nursing practitioners and a scientific basis for quality-of-life interventions and standardized nursing for these patients, thus promoting the standardization of nursing practice. METHODS: A comprehensive approach …

Is donepezil therapy associated with reduced mortality in nursing home residents with dementia?

BACKGROUND: Since cholinesterase inhibitors (CEIs) were approved for use in mild to moderate Alzheimer's disease, the therapeutic efficacy of this class of medications has largely centered on demonstration of short-term improvement in cognition and global function. Later evidence suggested that the beneficial effects of CEIs might be sustainable for at least 3 years and that CEIs may have a disease-modifying effect on Alzheimer's disease. These broad-ranging, long-term effects may explain the recent finding that the use of a CEI among nursing home residents with dementia was associated with lo

From X to Z: generational differences in ageism among Iranian nurses.

OBJECTIVE: Ageism is a form of discrimination based on age. It is defined as negative attitudes, beliefs, or behaviors toward older individuals. In healthcare settings, ageism can lead to reduced care quality, patient dissatisfaction, and poorer health outcomes. One potential factor contributing to ageism among healthcare providers is generational differences. Nurses from different generational cohorts (X, Y, and Z) have differences in socialization, experience, and cultural context, which can lead to variation in ageism. This study aimed to investigate generational differences in ageism among

Profiles in safety: The Hospital From Home Program.

Although today's health care system has achieved unprecedented advances, geriatric adults remain vulnerable to iatrogenic complications associated with traditional hospitalization, including acute cognitive delirium, rapid functional decline, medical errors, and health care-associated infections. To improve patient safety, an organization initiated an acute hospital-at-home pilot program from June 2023 through February 2024. Building on the removal of prepandemic regulatory and reimbursement barriers, the pilot was formally expanded and rebranded as the Hospital From Home (HfH) Program. An eva

Emergency Medicine Management of Distal Radius and Ulna Fractures: Narrative Review.

IMPORTANCE: Distal radius fractures and distal ulna fractures are common orthopedic injuries treated in emergency departments (ED). Our goal in this narrative review was to present emergency physicians with a single source that covers the evaluation and treatment of distal radius fractures and distal ulna fractures across the lifetime of our patients. There are differences in the treatment algorithms for distal radius fractures between pediatric, young adult, and geriatric patients. OBSERVATIONS: Pediatric distal radius fractures are defined by their relation to the growth plate and are influ

AI-assisted PEG aftercare education for older adults: clinician-informed chatbot design (PEGAssist).

PURPOSE: To evaluate whether a geriatric-focused, ChatGPT-based chatbot (PEGAssist) provides clinically adequate and comprehensible guidance for percutaneous endoscopic gastrostomy (PEG) aftercare in older adults. We examined whether its answers met expert expectations for depth/clinical usefulness, clarity/actionability, and scientific accuracy, with emphasis on complication recognition and triage. METHODS: A multidisciplinary panel (geriatrics, nursing, surgery) independently rated chatbot responses to a curated set of common PEG aftercare questions spanning education, routine care, trouble

Evaluating the implementation process of a multicomponent perioperative intervention for prevention of postoperative functional decline in geriatric patients: a qualitative study.

OBJECTIVES: Geriatric patients are at increased risk of developing postoperative neurocognitive disorders, including delirium. Existing evidence-based perioperative interventions need to be implemented into routine care to improve postoperative outcomes. In this qualitative interview study, we wanted to collect stakeholder experiences to understand the implementation process of a multi-component intervention to prospectively facilitate future implementation. DESIGN: Descriptive qualitative evaluation research. SETTING: Single-centre at a German major urban academic hospital. PARTICIPANTS: 2

The cardiac care bridge program: design of a randomized trial of nurse-coordinated transitional care in older hospitalized cardiac patients at high risk of readmission and mortality.

BACKGROUND: After hospitalization for cardiac disease, older patients are at high risk of readmission and death. Although geriatric conditions increase this risk, treatment of older cardiac patients is limited to the management of cardiac diseases. The aim of this study is to investigate if unplanned hospital readmission and mortality can be reduced by the Cardiac Care Bridge transitional care program (CCB program) that integrates case management, disease management and home-based cardiac rehabilitation. METHODS: In a randomized trial on patient level, 500 eligible patients ≥ 70 years and at

Inclusion of medication-related fall risk in fall risk assessment tool in geriatric care units.

BACKGROUND: Falls are common undesirable events for older adults in institutions. Even though the patient's fall risk may be scored on admission, the medication-induced fall risk may be ignored. This study developed a preliminary categorization of fall-risk-increasing drugs (FRIDs) to be added as a risk factor to the existing fall risk assessment tool routinely used in geriatric care units. METHODS: Medication use data of older adults who had experienced at least one fall during a hospital ward or a nursing home stay within a 2-year study period were retrospectively collected from patient rec

Evaluation of musculoskeletal pain management practices in rural nursing homes compared with evidence-based criteria.

Chronic pain, mainly associated with musculoskeletal diagnoses, is inadequately and often inappropriately treated in nursing home residents. The purpose of this descriptive study is to identify the musculoskeletal diagnoses associated with pain and to compare pain management of a sample of nursing home residents with the 1998 evidence-based guideline proposed by the American Geriatrics Society (AGS). The sample consists of 215 residents from 13 rural Iowa nursing home homes. The residents answered a series of face-to-face questions that addressed the presence/absence of pain and completed the
