Brainstem dysfunction in critically ill patients.

The brainstem conveys sensory and motor inputs between the spinal cord and the brain, and contains nuclei of the cranial nerves. It controls the sleep-wake cycle and vital functions via the ascending reticular activating system and the autonomic nuclei, respectively. Brainstem dysfunction may lead to sensory and motor deficits, cranial nerve palsies, impairment of consciousness, dysautonomia, and respiratory failure. The brainstem is prone to various primary and secondary insults, resulting in acute or chronic dysfunction. Of particular importance for characterizing brainstem dysfunction and i

Avulsion of globe following maxillofacial trauma.

Avulsion of the eye globe is a rare condition that is unexpected in maxillofacial trauma and results from severe trauma to the orbit and face. A search of the literature reveals very few reported cases. The present article reports a case of this singular, challenging condition and comments on several other related causes, pathophysiology mechanisms, risks, complications, and management.

[Communication with the relatives of a patient presumed brain dead].

BACKGROUND: The choice of wording in cases of suspected brain death is important. If brain death has not been proven by electrocerebral silence and by absence of spontaneous breathing in an apnoea test in a patient in intensive care, then words like 'brain dead', 'has died' and 'clinical brain death' should be avoided in conversations with the relatives of the patient. This is illustrated by three cases. CASES: The first patient was a 46-year-old woman, with thrombosis of the basilar artery; the second was a 26-year-old man who was resuscitated after a bilateral pneumothorax, but developed se

An unexpected outcome of cervical near-hanging injury. A case report.

Cervical near-hanging injuries are most frequent in the young adult population. However, the literature gives little guidance regarding diagnostic evaluation of these patients, although it is well known that their initial clinical presentation has limited prognostic value. This case report presents a patient who actually survived a suicidal near-hanging attempt and was later able to walk and talk to his physician. He died the next day due to carotid dissection and cerebral ischemia. In this case report we emphasize the importance of thorough radiological investigation in the accurate assessmen

[Minor head injury in children--proposed algorithm for clinical management].

AIM: Objective analysis of clinical and epidemiological aspects of minor head injuries in children with an attempt to elaborate a practical algorythm for diagnostic management. MATERIAL AND METHODS: All children aged 3 to 18 years with minor head injury treated in the years 2005-2006 in the Department of Paediatric Surgery and Traumatology in Grudziadz, were included in a prospective study. In all, 884 children were enrolled into this analysis. Their medical data were reviewed with an emphasis on circumstances of injury, clinical signs and symptoms, diagnostic and therapeutic management its e

Medical imaging and CNS infections.

West Nile virus, Creutzfeldt-Jakob disease and new-variant CJD are emergent infectious diseases that affect the brain. Although incidence of these infections is low compared with other CNS diseases such as viral or bacterial meningitis, the reasons for their emergence are complex. Scientists believe that unique combinations of genetic, environmental and social factors have caused these rare pathogens to become more common within human populations. Because the causes of brain infections are difficult to diagnose and treat, medical imaging is an important screening, diagnostic and monitoring too

Posterior Fossa Surgery for Stroke: Differences in Outcomes Between Cerebellar Hemorrhage and Infarcts.

BACKGROUND: Posterior fossa surgery is the established treatment for large cerebellar strokes with brainstem compression. Despite this, there is a paucity of data for long-term outcomes. METHODS: A retrospective analysis of patients who underwent posterior fossa surgery for cerebellar hemorrhages and infarcts was performed to compare their difference in 6-month outcomes and to identify factors that affect outcomes. Patients were dichotomized into groups with good outcomes (modified Rankin scale [mRS] score 0-3) or poor outcomes (mRS score 4-6). Sex, age, preoperative Glasgow Coma Scale score,…

Amphetamine toxicity masked by concomitant γ-hydroxybutyrate (GHB) ingestion.

We report a case of a young male with amphetamine toxicity initially obscured by concomitant use of gamma-hydroxybutyrate (GHB), and the sympathomimetic symptoms emerged after GHB's effects receded. A 24-year-old unconscious man presented to emergency department showed the following vital signs upon admission: blood pressure 136/58 mmHg; heart rate 79 bpm; SpO2 87% under ambient air; body temperature 36.1 °C; Glasgow Coma Scale score 3. The pupils were not dilated. Arterial blood gas test revealed respiratory acidosis (pH = 7.229, pCO2 = 64.4 mmHg, pO2 = 42.3 mmHg, HCO3 = 26.3 mmol/L). Intubat…
