Lowering of intraocular pressure after phacoemulsification in primary open-angle and angle-closure glaucoma: Correlation with lens thickness.

PURPOSE: To assess anterior chamber configuration changes during phacoemulsification in primary angle-closure suspect (PACS/PAC) and primary open-angle glaucoma (POAG). METHODS: Prospective observational comparative study of anterior segment optical coherence tomography (AS-OCT) findings before and after phacoemulsification on three groups of patients (PACS/PAC, POAG, and controls). Data were collected over a period of 9 months. Main outcome measures included mean change in anterior chamber depth (ACD), angle opening distance (AOD), and trabecular iris space area (TISA). RESULTS: 153 patient

Iris and its relevance to angle closure disease: a review.

Glaucoma is a leading cause of irreversible visual impairment, and primary angle closure glaucoma (PACG) affects Asians disproportionately. Whereas advances in ocular imaging have identified several anatomical risk factors, our ability to predict PACG still requires considerable improvement. The iris plays a crucial role in the pathophysiology of angle closure disease, either through a mechanical or vascular mechanism. Irises of closed-angle eyes inhibit vastly different structural constituents as compared with those of open-angle eyes, thereby effecting variations in biomechanical properties

Solitary Iris Plasmacytoma With Anterior Chamber Crystalline Deposits.

PURPOSE: To report a case of solitary iris plasmacytoma successfully treated with ruthenium plaque radiotherapy. METHODS: A 44-year-old white woman presented with pain in the right eye and raised intraocular pressure. Her medical history included breast cancer treated 11 years earlier with lumpectomy, lymph node clearance, chemotherapy, and radiotherapy. On examination, the right iris had a tan-colored mass with crystalline deposits visible on the mass surface and on the corneal endothelium. The fundus and left eye were normal. An anterior segment ultrasound scan showed a mass with mixed inte

Congenital Glaucoma and CHARGE Syndrome: A Case Report.

PURPOSE: To report a rare case of congenital glaucoma in a patient with CHARGE syndrome, present gonioscopic photographs, and explore mechanisms of disease that may account for this association. PATIENTS AND METHODS: We describe a 35-week-old girl with previously diagnosed CHARGE syndrome who presented with corneal edema, buphthalmos, and elevated intraocular pressure in the left eye. She was subsequently diagnosed with congenital glaucoma and started on topical and oral therapy. RESULTS: Examination under anesthesia confirmed the above findings as well as bilateral abnormal angles with an a

[Anterior pole ischemia and carotid stenosis].

PURPOSE/METHODS: We present the case of a 69 year-old male patient who developed an anterior pole ischemia syndrome in his right eye. He did not have any personal history of diabetes or previous ocular pathology. Indirect ophthalmoscopy did not show any pathological finding. RESULTS/CONCLUSIONS: Carotid Duplex scanning and Nuclear Magnetic Resonance detected a severe carotid stenosis on the same side, which explained the case. We reviewed different forms of presentation of venous stasis syndrome in carotid stenosis and present this uncommon case of anterior segment ischemia syndrome without p

[The effect of Nd:YAG laser iridotomy on irido-corneal angle width assessed with ultrasound biomicroscopy in patients with primary open angle glaucoma].

The aim of the study was to evaluate irido-corneal angle width by use of ultrasound biomicroscopy in primary open angle glaucoma patients and to compare the results obtained before and 7 days after Nd:YAG laser iridotomy. The analysis was performed with the results obtained in 48 primary open angle glaucoma eyes of 48 patients, with the disease lasting 3 to 9 years. The mean age of the patients was 58.7 years. The mean irido-corneal angle width YXT increased by 80%, YXZ by 86% and the opening width increased by 64% 7 days after Nd:YAG laser iridotomy. Ultrabiomicroscopy seems to be a good meth

Congenital glaucoma associated with cutis marmorata telangiectatica congenita: two case reports.

Two cases of cutis marmorata telangiectatica congenita are described in association with nevus flammeus and congenital glaucoma. Approximately 65 cases have been reported and the association with nevus flammeus and congenital glaucoma has only been reported in one of those cases. Unlike the glaucoma associated with nevus flammeus of Sturge-Weber, this type of congenital glaucoma appears to be associated with an abnormal filtration angle rather than an increased episcleral venous pressure.

Effect of repeated anterior chamber perfusion on intraocular pressure and total outflow facility in the cynomolgus monkey.

Total outflow facility was determined by two-level constant pressure perfusion of the anterior chamber in surgically virgin, aniridic, and ciliary muscle disinserted cynomolgus monkey eyes 6 to 13 times at 1- to 2-month intervals over periods of 8 to 24 months. Facility decreased approximately 15 to 20% between consecutive thirds of the perfusion history, independent of eye type. The facility decreases were too large to be explained by decreased uveoscleral facility or pseudofacility, and were not mediated by the iris, ciliary muscle, or gonioscopically or ultrastructurally apparent chamber an

Biomicroscopic, gonioscopic and tomographic analysis of a Kayser-Fleischer ring: Case report.

The Kayser- Fleischer Ring (KFR) is a characteristic, though not pathognomonic, feature of Wilson's disease (WD). It arises from copper deposition in Descemet's membrane and may exhibit a range of hues, including yellow-green, dull red, bright green, or blue. We report the case of a 58-year-old female patient in whom, in addition to copper deposition, optical coherence tomography (OCT) revealed leucomatous alterations in the corneal stroma adjacent to the deposit. Early recognition of this clinical sign plays a crucial role in the diagnosis of WD, in which timely identification is essential to

[Primary anterior chamber angle closure: progression from suspect to glaucoma. Part 2. Predictors of primary angle closure].

To review the literature devoted to the search of predictors of primary angle closure (PAC) progression as an important link in the pathogenesis of primary angle-closure glaucoma. This part presents a cluster analysis, describes the mechanisms of PAC development, and considers the studies aimed at discovering the risk factors for the progression of primary angle closure suspect into true angle closure. The results of the analyzed literature are ambiguous, indicating the need for further research that would involve strict inclusion criteria, and a standard approach to defining the primary angle

The anterior and posterior biometric characteristics in primary angle-closure disease: Data based on anterior segment optical coherence tomography and swept-source optical coherence tomography.

PURPOSE: Obtaining a better understanding of the pathogenesis of primary angle-closure disease (PACD) still requires studies that provide measurements of anterior and posterior biometric characteristics together and that assess the relationship between them. METHODS: In total, 201 eyes were enrolled in this cross-sectional study: 50 normal controls, 49 primary angle-closure suspect (PACS), 38 primary angle closure (PAC), and 64 primary angle-closure glaucoma (PACG) eyes. The anterior and posterior structural features were measured by anterior segment optical coherence tomography and swept-sou

Changes in Iris Stiffness and Permeability in Primary Angle Closure Glaucoma.

PURPOSE: To evaluate the biomechanical properties of the iris by evaluating iris movement during pupil constriction and to compare such properties between healthy and primary angle-closure glaucoma (PACG) subjects. METHODS: A total of 140 subjects were recruited for this study. In a dark room, the anterior segments of one eye per subject were scanned using anterior segment optical coherence tomography imaging during induced pupil constriction with an external white light source of 1700 lux. Using a custom segmentation code, we automatically isolated the iris segments from the AS-OCT images, w
