Flexor tendon rupture in the palm of the hand is very infrequent. It usually occurs in association with an underlying pathological condition. We report a case of hand tendon rupture in a patient with osteogenesis imperfecta (OI). This disease is typically characterised by brittle bones but other tissues rich in collagen, such as tendons, may also be involved. The ruptured tendon was diagnosed one month post-injury and was treated by tendon graft. Six months later, tenolysis was performed and one year post-injury flexion was still limited at the distal interphalangeal joint. Few cases of tendon

INTRODUCTION: Volkmann's ischemic contracture (VIC) is a complex and variable flexion deformity of wrist and fingers resulting from fibrosis and contracture of flexor muscles of forearm. It is caused by ischemic injury to the deep tissues enclosed in the tight unyielding osteo-facial compartments secondary to neglected acute compartment syndrome. VIC may be associated with malunion or non-union of forearm fractures. To the best of our knowledge, this article is the first case report of VIC associated with atrophic non-union of ulna managed by shortening of forearm bones combined with transposi

Fight bite injuries of the hand are common presentations in A&E departments and usually result from a fist blow to the mouth. The authors report a case of a 24-year-old man who presented 6 weeks after an injury to his right wrist following an altercation. Radiographic examination and CT scans were in keeping with a tooth fragment embedded in the trapezoid. However, post excision histology subsequently revealed the lesion to be a solitary osteochondroma of the trapezoid. Osteochondromas are benign lesions of bony or cartilaginous origin and are usually found in the metaphyseal region of long bo

The purpose of this study was to assess the long-term clinical results and morphological changes after tendon ball arthroplasty for advanced Kienböck's disease. Twenty-six patients were reviewed, with a mean follow-up interval of 125 months (range 50-226). At follow-up, mean score on the Disabilities of the Arm, Shoulder, and Hand questionnaire was 7.7 and mean visual analogue scale score for pain was 1. Mean carpal height ratio was significantly reduced with respect to the pre-operative value. On magnetic resonance imaging scans, cartilage damage, synovitis, and erosive or oedematous changes

In this article, a case of examining an incomplete human skeleton found in a forest is presented. Based on the assessment of posttraumatic lesions in the skull bones, the cause of death of a man who had died 14 years prior to the examination was determined with high probability to be due to a lightning strike. Moreover, the rare pathological lesions within the skeleton, such as a healed fracture of the humerus and rheumatoid malformations within the hand, in reference to preserved medical records, have also allowed to identify the deceased. Most noteworthy points in this case are, however, ini

The chemical composition, i.e. ash and calcium contents and organic, hydroxyproline, and hexosamine contents of the acetone-dried diaphyseal cortical bone powder of the tibias and the femurs, was studied in adult rats after a) below-knee amputation and b) immobilisation of the one hind leg in a plaster cast. There were no significant differences in the chemical composition of the chemical composition of the bones of the treated extremity as compared with the corresponding bones from the non-treated control leg, either at different times after the amputation and immobilisation, respectively, or

Carcinoma of the lung is the most common lethal form of cancer in both men and women worldwide. Orthopedic manifestations of lung cancer frequently include bony metastasis, most commonly the vertebrae (42%), ribs (20%), and pelvis (18%). Acral metastatic disease is defined as metastasis distal to the elbow or the knee. Metastases to the bones of the hand are extremely rare. Only 0.1% of metastatic disease resulting from any type of carcinoma or sarcoma manifests as metastasis in the hand. There are only a few reports in the literature of soft-tissue or muscular metastasis to the hand from a ca

PURPOSE: Coverage of exposed functional structures such as tendons, bones, vessels or nerves at the dorsal and palmar surface of the hand requires thin, supple tissue to provide adequate range of motion and a satisfying aesthetic result. Free fascial flaps are possible alternatives to cutaneous, fasciocutaneous and muscle flaps. The purpose of this retrospective study was to evaluate the functional and aesthetic results after coverage of the hand with free serratus fascial flaps in our department.
METHOD AND CLINICAL MATERIAL: From 1994 to 2002, ten patients underwent free fascial flap covera

Multiple hereditary exostoses (MHE) are an autosomal dominant disorder, consisting of multiple cartilage capped bone tumour arising from the metaphysis of long tubular bones. Mutations are seen in Exostosin-1 and Exostosin-2 genes. We present a family of MHE for three generations. The index case was a 10-year-old male presented with multiple exostoses in hand, forearm, leg, right knee and chest. Family history revealed similar complaints in younger brother, father, paternal uncle, paternal aunt & her daughter and grandfather.

The scaphoid is the most commonly fractured of the eight carpal wrist bones. The mechanism of injury is usually from a fall on the outstretched hand. Initial radiographs may not identify a scaphoid fracture. A high index of suspicion given to these biomechanics and the physical findings will assist in the identification of this fracture. Certain fractures of the scaphoid are at high risk of avascular necrosis and nonunion requiring splinting and orthopedic referral of suspected fractures to assist in preventing such complications.

Closed dislocations of finger bones is a rare injury in children. Most often complications are localized in the metacarpophalangeal articulation of the first finger. The main method of treatment of fresh dislocations is a one-step closed hand reposition. In case of its failure an open setting of the dislocation should be performed along with liquidation of the cause of interposition. The method of choice of treatment of neglected dislocations of fingers in children is the application of a compression-distraction apparatus.

Aneurysmal bone cysts associated with tubular bones of the hand are rare and even rarer in the paediatric age-group. They are rapidly growing, destructive benign bone tumors. Multiple treatment modalities for aneurysmal bone cyst have been reported in the literature, but controversy exists regarding optimal treatment. We report a case of aneurysmal bone cyst of proximal phalanx of middle finger in a 2 year old child treated at our tertiary care hospital. The diagnosis was confirmed with pre-operative MRI, FNAC and post-operative histopathology. Curettage of the lesion and autologous bone graft

Twenty right metacarpal bones of Brazilian fetuses (ranging from 17 to 35 weeks post-conception) were studied by the allometric method to define the pattern of metacarpal growth. The metacarpal measurements analysed were their maximum length (ML), ossified shaft length (OSL) and midshaft breadth (MSB). These data were related to crown-rump length and the metacarpal weight to fetal weight. The increase in metacarpal measurements showed an allometric positive coefficient (kappa), highest in the OSL. The ML growth followed an axo-marginal pattern. The 3rd metacarpal showed the highest MSB kappa c

Previous studies of patients with lymphoid malignancies have suggested a relationship between survival and the presence of more than 40% hard-mirror cells in the bone marrow. This report represents the first detailed study of a patient with American Burkitt's lymphoma with numerous hand-mirror cells in the bone marrow during the course of the disease. The Burkitt cells were atypical because of their hand-mirror configuration in the bone marrow, the presence of cytoplasmic glycogen, the finding of an IgG surface marker, and a strongly positive acid phosphatase staining reaction. The presence of

Hand deformities in 2 wild-born pygmy chimpanzees were investigated through dissection, linear measurements, bone mineral analysis and X-rays. Weight of tissue (skin, muscle, bone) was determined for each body segment (hand, forearm, upper arm), permitting comparison between the abnormal and normal upper limbs. In one individual, the right and left forearms and hands differed in tissue weights and bone mineral content. In the other, the hands differed in tissue composition, but not the forearms.

The anterolateral thigh (ALT) fasciocutaneous flap has been well described for hand skin cover, however its use in its composite (multiple tissue) form incorporating bone, muscle and/or fascia has been less well described. We report the usefulness of the composite ALT flap in six complex hand trauma cases, four male and two female (age range 14-46 years). The palm of the hand was affected by injury in three cases, the dorsum in two cases, and the radial side in one case. The first web formed part of the injury in three cases. There was variable involvement of the tendons, nerves and bones. Fas

A review of neck and upper limb disorders caused by combined exposures to hand-transmitted vibration and ergonomic risk factors (repetitiveness, force, posture) suggested the following conclusions: (1) hand-transmitted vibration has a dominant role in the etiopathogenesis of Raynaud's phenomenon and various forms of peripheral neuropathy with sensory impairment (digital, multifocal) in users of vibrating tools; (2) vibration of low frequency and high amplitude from percussive tools concur, together with adverse ergonomic factors, to produce degenerative changes in the bones and joints of the u

Ulnar wrist pain is a frequent symptom with many possible traumatic and non-traumatic causes. The complex anatomy and biomechanics of the wrist with the ulnocarpal complex including the triangular fibrocartilaginous complex (TFCC), the distal radio-ulnar joint (DRUJ) and the ulnar carpal bones make the differentiation between the possible causes difficult. A precise clinical investigation and appropriate imaging studies are essential for detecting the most important differential diagnoses, the first therapeutical steps and an early and appropriate referral to the hand surgeon.
