We report a case of onychomycosis with melanonychia due to Candida parapsilosis alone. Candida parapsilosis is now identified in the great majority of candidal onychomycosis, mainly in association with other yeasts. The criteria allowing the distinction between invasion and colonization, the risk factors and the treatment of C. parapsilosis onychomycosis are discussed.

Two patients with keratosis punctata of the palmar creases are described. The association with ichthyosis vulgaris and other disorders of keratinization is discussed. In both cases, histopathology revealed a close relation between the keratotic plug and the sweat glands. The role of genetic factors and manual activity in the pathogenesis is discussed. Treatment with oral etretinate resulted in a good improvement in the first patient, but prolonged low-dose maintenance therapy was required to prevent recurrence.

A case of pyoderma gangrenosum on the dorsal aspect of the right second and third fingers of a 50-year-old woman is reported. The patient had been wounded at the above-mentioned site by a cutter. This wound developed into a painful ulcer which enlarged rapidly. No bacterial infections were detected. Blastomycosis is an uncommon cause of infection in the hand. Fungal infection should be suspected in any case when expected improvement does not occur with appropriate antibiotic therapy and wound care. We describe a rare case of primary blastomycosis of the hand that progressed to severe soft tissue destruction and significant loss of hand function.

Lichen striatus is a common childhood eruption, but only rare reports link cutaneous manifestations and onychodystrophy. We report a case of lichen striatus and onychodystrophy in a 12-year-old girl, only the eighth such patient in the dermatologic literature. Nail changes may precede the rash of lichen striatus and should be considered in the differential diagnosis of isolated nail dystrophy in children.

Seven cases of fiberglass dermatitis among production operators in the electronics industry are reported. This was due to the repeated daily handling of printed circuit boards (PRCBs). The predominant complaint of the workers was itch of the lateral aspects of the fingers and finger webs. In six workers, unroofed vesicles, pinhead areas of postinflammatory hyperpigmentation, and excoriation marks were noted at these sites. Irritant hand eczema is a well-known risk in adults, who have suffered from severe atopic dermatitis during childhood. We therefore studied the involvement of the skin on the hands in 29 consecutive patients admitted for severe atopic dermatitis. Only 5 of the 29 patients had eczema in the palmar skin (p less than 0.01; sign test). There was no correlation with the existence of hyperlinearity, extent of disease, concomitant asthma, total serum IgE or work.

Pyoderma gangrenosum is infrequently encountered by the orthopedic surgeon. The appearance of progressive, painful bullae followed injury and infection in a 56-year-old man. The treatment consisted of a search for an associated underlying systemic disease in concert with high-dose steroids. The etiology is not known.

A 17-year-old boy developed an acral granulomatous dermatosis which resembled clinically acrodermatitis continua of Hallopeau. The histology showed dermal and subcutaneous abscess formation with granulomas. The lesions responded to systemic corticosteroid therapy, with residual atrophy and contractures of the fingers.
