Drug use treatment and harm reduction programs in Iran: A unique model of health in the most populated Persian Gulf country.

Because of the proximity of Persian Iran to Afghanistan, the main opium producer in the world, drug use especially opium use has a long history in Iran. Opium and its residues are the traditional drugs while heroin, heroin Kerack, norgesic, temgesic, and methamphetamine use and injection have emerged more recently. In recent decades, heroin smoking and injection have presented challenges to the Persian health policy makers to accept and develop the internationally-approved programs of drug use treatment and harm reduction. The current paper summarizes the overall picture of main drugs used and…

The Global Fund to Fight AIDS, Tuberculosis and Malaria's investments in harm reduction through the rounds-based funding model (2002-2014).

BACKGROUND: Harm reduction is an evidence-based, effective response to HIV transmission and other harms faced by people who inject drugs, and is explicitly supported by the Global Fund to Fight AIDS, Tuberculosis and Malaria. In spite of this, people who inject drugs continue to have poor and inequitable access to these services and face widespread stigma and discrimination. In 2013, the Global Fund launched a new funding model-signalling the end of the previous rounds-based model that had operated since its founding in 2002. This study updates previous analyses to assess Global Fund investmen…

A Contextual Comparison of Risk Behaviors Among Older Adult Drug Users and Harm Reduction in Suburban Versus Inner-City Social Environments.

Recent epidemiological data show that older adults comprise a growing age group of drug users and new AIDS cases in the United States. Prevention and intervention studies show that risk behaviors leading to HIV infection are increasing among older users, particularly among the socially vulnerable. Yet older adults remain an under-researched population of drug users and little is known about their risk behaviors. Our aim is to address this gap in knowledge on older users by comparing contextual factors that influence risk behaviors and harm reduction strategies practiced by older drug users liv…

Targeting HIV-related outcomes with intravenous drug users maintained on methadone: a randomized clinical trial of a harm reduction group therapy.

Methadone maintenance programs (MMP) have the potential to play an important role in reducing HIV risk, given the appropriate type and level of ancillary treatments. In this study, we investigated the efficacy of a 12-session harm reduction group intervention for injection drug users, based upon the Information-Motivation-Behavioral skills model of behavior change, that focused on reducing both drug and sex risk. Two hundred and twenty patients entering an MMP were randomized to receive either standard care (SC)-2 hours of counseling per month and a single-session risk reduction intervention-o…

Reduction in Exposure to Selected Harmful and Potentially Harmful Constituents Approaching Those Observed Upon Smoking Abstinence in Smokers Switching to the Menthol Tobacco Heating System 2.2 for 3 Months (Part 1).

INTRODUCTION: The Tobacco Heating System (THS) is a "heat-not-burn" tobacco product designed to generate significantly lower levels of harmful and potentially harmful constituents (HPHCs) and present lower risk of harm than cigarettes. This study assessed the exposure reduction to selected HPHCs in smokers switching to menthol Tobacco Heating System (mTHS) 2.2 compared with smokers continuing smoking menthol cigarettes (mCCs) and smoking abstinence (SA) for 5 days in a confined setting, followed by an 86-day ambulatory period. METHODS: A total of 160 healthy adult US smokers participated in t

A Scoping Review of the Emergence of Novel Synthetic Opioids in Australian Drug Markets: What Does This Mean for Harm Reduction Responses?

ISSUES: Rising synthetic opioid deaths in North America, and the emergence of nitazene harms has driven concern about novel synthetic opioids (NSO) in Australia. This scoping review aimed to map what is known about NSOs in Australia. APPROACH: Scoping review of peer-reviewed and grey literature on NSO detections and harms. CRITERIA: studies describing NSO market indicators and harms in Australia. KEY FINDINGS: Overall, 912 peer-reviewed studies were identified; 40 met criteria. Monitoring systems identified NSOs (e.g., β-U10, nitazenes) for the first time in Australia. Nitazene toxicity app

Examining Harm Reduction in a Housing First for Youth Program for Youth Experiencing Homelessness and Concurrent Disorders in a Small Canadian City.

Housing First for Youth (HF4Y) is a youth-focused adaptation of the well-established Housing First (HF) approach to housing and service provision for individuals experiencing homelessness. Given that youth homelessness is associated with an increased likelihood of substance use issues, a central tenet of the HF4Y framework is the use of a harm reduction approach to substance use. However, research on HF4Y has yet to examine how harm reduction is specifically being implemented in these settings. This study addresses this gap by examining how the principles and philosophies of harm reduction wer…

"Give me the reigns of taking care of myself with a home": Healing environments in an Indigenous-led alcohol harm reduction program.

BACKGROUND: Distinct from western Managed Alcohol Programs (MAPs), Indigenous-led alcohol harm reduction programs can be defined by both 'culture as healing' and decolonized harm reduction philosophies. We sought to explore experiences of Indigenous 'family members' (participants) in an Indigenous-led alcohol harm reduction program and culturally supportive housing to identify appropriate supports according to family member perspectives, and to inform delivery of the program. METHODS: Situated within an Indigenous-western research partnership, we completed semi-structured interviews with seve…

Estimating the impact of international funding reductions on harm reduction programmes for people who inject drugs: a comparative analysis of three high-burden countries: India, South Africa, and Nigeria.

Harm-reduction services for people who inject drugs (PWID) remain vulnerable due to political instability and reliance on external funding. As international donors reduce support, the sustainability of these programmes is increasingly uncertain. This study examines how different HIV financing structures influence the resilience of harm-reduction services in India, Nigeria, and South Africa.A retrospective comparative analysis was conducted using national programme reports, financial data, and global monitoring sources from 2014 to 2024. Trends in domestic and external funding were assessed alo

When weight-loss drugs reduce drinking: glucagon-like peptide-1 receptor agonists as phenotype-enriched harm-reduction therapy for alcohol use disorder.

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have attracted interest as potential treatments for alcohol use disorder (AUD) after patients receiving these agents for obesity or type 2 diabetes reported a reduced desire to drink. This editorial examines whether current evidence supports their classification as broadly applicable addiction treatments or as phenotype-enriched therapies. Randomized trials evaluating exenatide and semaglutide suggest a clinically relevant but endpoint-selective signal. Reductions have been most consistent in heavy-drinking days, drinks per drinking day, na

The effectiveness of housing-based interventions and harm reduction among adults experiencing homelessness: a systematic review of recent controlled evidence (2019-2025).

Homelessness is a major public health and social issue, and research on the effectiveness of housing and harm reduction interventions provides fragmented findings that are difficult to compare across studies. The aim of this systematic review was to assess the effects of housing programmes (including Housing First) and alcohol harm reduction interventions among adults experiencing homelessness. Six databases (PubMed, Embase, Scopus, Web of Science, PsycINFO, and the Cochrane Library) were searched, identifying 1385 records. After screening, 159 publications were assessed in full text, of which…

Integration of harm reduction principles and practices within specialty substance use treatment programs in New Jersey: A qualitative study of program leadership.

INTRODUCTION: Harm reduction is a philosophical approach to improve the health of people who use drugs (PWUD) that integrates risk reduction, evidence-based treatment, and person-centered care. Specialty substance use treatment programs have historically been siloed from, and often misaligned with, harm reduction principles, but this trend has begun to shift in recent years. This study explored the ways in which some specialty treatment settings are adopting harm reduction principles and practices. METHODS: We conducted qualitative interviews with leaders of 14 New Jersey specialty treatment …
