Noninvasive ventilation with helmet versus control strategy in patients with acute respiratory failure: a systematic review and meta-analysis of controlled studies.

BACKGROUND: Noninvasive ventilation (NIV) has proved to be a useful technique for breathing support. However, complications, discomfort, and failure of NIV were commonly caused by the mask. Therefore, the helmet was developed to improve performance and reduce complications; however, there has been no conclusive results on its effect until now. Thus, we performed a systematic review and meta-analysis to investigate the effect of NIV with a helmet versus the control strategy in patients with acute respiratory failure (ARF). METHODS: We searched Cochrane Library, PubMed, Ovid, and Embase databas

Motorcycle Helmets: The Economic Burden of an Incomplete Helmet Law to Medical Care in the State of Connecticut.

The lack of a mandatory motorcycle helmet law leads to increased injury severity and increased health care costs. This study presents a financial model to estimate how the lack of a mandatory helmet law impacts the cost of health care in the state of Connecticut. The average cost to treat a helmeted rider and a nonhelmeted rider was $3,112 and $5,746 respectively (cost adjusted for year 2014). The total hospital treatment cost in the state of Connecticut from 2003 through 2012 was $73,106,197, with $51,508,804 attributed to nonhelmeted riders and $21,597,393 attributed to helmeted riders. The effect of head protection on the hearing of rugby players.

OBJECTIVE: Professional rugby players utilise various methods of head protection to prevent against the development of a pinna haematoma. This study tests the hypothesis that these measures, whilst preventing injury, decrease the wearers' hearing threshold and therefore their performance. DESIGN AND PARTICIPANTS: Eight patients had free field audiometry performed in a soundproof room, with warble tones. All patients were young men (mean 24.75 years (range 22-34)). No participant had ear symptomatology or a past history of ear surgery. Three separate audiological assessments were performed on

Characteristics of older motorcyclist crashes.

In the U.S. as well as other countries, the number of motorcyclists killed in traffic crashes has risen sharply over the past five years, due in part to the increased popularity of motorcycling among older riders. This paper examines trends in motorcyclist casualties and vehicle registrations from 1990-2002, based on national and state (North Carolina) motor vehicle crash and vehicle registration data. The data show similar patterns of increased fatalities that parallel a growth in motorcycle registrations. Whereas the number of motorcyclists ages 16-24 declined over the 13-year study period,

MRI is nondiagnostic in cervical spine imaging of the helmeted football player with shoulder pads.

BACKGROUND: It is currently recommended that helmet and shoulder pads remain in place during the initial clinical and radiographic evaluation of the helmeted athlete with a potential cervical spine injury. The objective of this prospectively designed, single-subject study was to determine whether MRI may play a role in the initial evaluation and management of the helmeted football player with a cervical spine injury. METHODS: One male athlete was fitted using equipment (football helmet [Riddell], shoulder pads [Douglas]) worn during the collegiate season at Lehigh University. Standard MRI usi

Eye injuries in Canadian amateur hockey.

Two studies, one retrospective (1972 to 1973) and one prospective (1974 to 1975), CONcerning eye injuries incurred by hockey players were conducted by the Canadian Ophthalmological Society with questionnaires to its members. Responses to the questionnaires were analyzed by age, type of injury, cause (i.e., hockey stick, puck, or other means), and results to visual acuity. The results were also designated by organized or unorganized participation. Almost 300 eye injuries were reported in each study. In the first study, 13.7% of the injured players became legally blind as a result of the injury;

Photoprotective habits of patients with cutaneous lupus erythematosus.

BACKGROUND: Previous studies have suggested that patients with cutaneous lupus erythematosus (CLE) are deficient in sunscreen use. Use of other photoprotective methods by patients with CLE has not been assessed to our knowledge. OBJECTIVE: We sought to identify demographic and clinical characteristics of patients with CLE who have the lowest overall sun-protection habits scores, and who are least likely to practice 5 individual photoprotective methods (ie, shade, sunscreen, long sleeves, hat, and sunglasses). METHODS: A total of 105 patients with CLE at the University of Texas Southwestern M

Factors affecting motorcycle helmet use: size selection, stability, and position.

OBJECTIVES: One of the main requirements of a protective helmet is to provide and maintain appropriate and adequate coverage to the head. A helmet that is poorly fitted or fastened may become displaced during normal use or even ejected during a crash. METHODS: Observations and measurements of head dimensions, helmet position, adjustment, and stability were made on 216 motorcyclists. Helmet details were recorded. Participants completed a questionnaire on helmet usability and their riding history. Helmet stability was assessed quasistatically. RESULTS: Differences between the dimensions of ISO

First prototyping of a dedicated PET system with the hemisphere detector arrangement.

A strong demand is expected for high-sensitivity, high-resolution and low-cost brain positron emission tomography (PET) imaging for early diagnosis of dementia, as well as for general neuroscience studies. Therefore, we have proposed novel geometries of a hemisphere detector arrangement for high-sensitivity brain imaging, in which an add-on detector at the chin position or neck position helps in sensitivity uniformity improvement. In this study, we developed the first prototype system for proof-of-concept using four-layer depth-of-interaction detectors, each of which consisted of 16  ×  16  ×

Do American Youth Football Players Intentionally Use Their Heads for High-Magnitude Impacts?

BACKGROUND: Concern for head injuries is widespread and has been reported by the media to be the number one cause of decreased participation in football among the American youth population. Identifying player mechanisms associated with intentional, or purposeful, head impacts should provide critical data for rule modifications, educational programs, and equipment design. PURPOSE: To investigate the frequency of intentional and unintentional head impacts and to examine the player mechanisms associated with intentional high-magnitude head impacts by comparing the impact mechanism distributions

Bone Anchored Hearing Aid With Headband Over Cranial Orthosis: A Pilot Study.

BackgroundCranial orthosis is a widely accepted treatment approach for moderate to severe deformational plagiocephaly. Custom-fit helmets molded tightly to the head are worn for 23 hours a day for several months2. This poses a challenge for children with concurrent conductive hearing loss who benefit from hearing devices, specifically bone anchored hearing aids (baha), that are intended to contact the skull directly1. We theorized the bone conducted signals could be transferred through the helmet to the baha device given its tight fit to the cranium.MethodsThis is a pilot study in which we pre

Development of new hard hat dimensions using user-centered design approach among oil palm harvesters.

BACKGROUND: Hard hats are among the personal protective equipment (PPE) used in many industries to reduce the impact of any falling object on the skull and also to prevent head and brain injuries. However, the practice of wearing a safety helmet during working hours is still low. This is due to the physical discomfort perceived by safety helmet users. OBJECTIVE: Given the unpopularity of the current hard hat, the general perception of workers concerning its use and its measurements are the determining factors in the development of a new hard hat. METHOD: A cross-sectional study was conducted

[Correction of pronounced plagiocephaly with orthosis : results of a prospective follow-up control].

For the treatment of pronounced deformational plagiocephaly in infants different therapeutic options are under discussion in the literature. This article presents a prospective observational study of 45 infants enrolled with distinct plagiocephaly and treated with a custom-made helmet orthosis. Treatment with the helmet orthosis was indicated by a difference in the skull diagonals of more than 0.5 cm or a cranial vault asymmetry (CVA) index > 3.5. The follow-up results were controlled with the help of a mobile spatial laser scanner and the CVA index and the skull deformity were measured. The a

Risk factors for injuries from in-line skating and the effectiveness of safety gear.

BACKGROUND: Of the estimated 22.5 million people participating in in-line skating in the United States in 1995, about 100,000 were sufficiently injured so as to require emergency department care. We investigated the effectiveness of wrist guards, elbow pads, knee pads, and helmets in preventing skating injuries. METHODS: We used data from the 91 hospital emergency departments participating in the National Electronic Injury Surveillance System, a national probability sample of randomly selected hospitals with 24-hour emergency departments. Injured in-line skaters who sought medical attention b

Randomized trial on sun safety education at ski and snowboard schools in western North America.

The purpose of this study was to evaluate the effectiveness of Go Sun Smart, a sun safety education program, directed to parents and children enrolled in ski and snowboard schools at high altitude resorts in western North America. Twenty-six ski resorts were paired and then randomly assigned to the intervention or control condition. Three hundred fifty-seven parents were interviewed about their children's sun safety behavior and exposure to the Go Sun Smart program. More parents at the intervention resorts reported that their child was wearing sunscreen than at the control resorts (OR 2.37, 95
