Identifying trusted local sources and predicting behavior change pathways according to COVID-19 vaccination status: Results of a 2022 statewide survey of Alaskan adults.

BACKGROUND: Low rates of COVID-19 vaccination remain a substantial public health challenge. Despite early successes, vaccinations of Alaskans trail the US average, drawing attention to the need for better-designed and targeted vaccine confidence interventions. Our objective was to assess levels of community trust and theory-driven predictors of vaccination status to inform the design of future programs. METHODS: We conducted a cross-sectional, telephone-based survey of 940 Alaskan adults between May and June 2022. Data were collected on vaccination status (including initial vaccination and re

Extravascular lung water and cardiac function assessed by echocardiography in healthy lowlanders during repeated very high-altitude exposure.

BACKGROUND: High-altitude pulmonary edema is associated with elevated systolic pulmonary artery pressure (sPAP) and increased extravascular lung water (EVLW). We investigated sPAP and EVLW during repeated exposures to high altitude (HA). METHODS: Healthy lowlanders underwent two identical 7-day HA-cycles, where subjects slept at 2900 m and spent 4-8 h daily at 5050 m, separated by a weeklong break at low altitude (LA). Echocardiography and EVLW by B-lines were measured at 520 m (baseline, LA1), on day one, two and six at 5050 m (HA1-3) and after descent (LA2). RESULTS: We included 21 subject

[Complex regional pain syndrome, CRPS].

Complex regional pain syndrome, CRPS, occurs with severe disabling pain, usually in the leg or hand, coupled with changes in pain perception, hyperesthesia and allodynia. There is as well, edema, changes in the color of the skin, trophic changes, and dystonia. The pain syndrome is often triggered by minor trauma. The pain perception is severe and out of context with the initial trauma. The syndrome is rare, occurring in a population-based study in the United States, with an incidence of only 5.5 per hundred thousand people per year. The incidence in Iceland, from the National Register of Disea

Community Health Workers' Concept and Understanding of Diabetes: A Qualitative Study.

OBJECTIVES: Community health workers (CHWs) fill in the insufficiency of health professionals in low-income countries. The CHWs' roles include health education of their constituents whose health they likewise take care. This study aimed to describe the concept and understanding of diabetes among CHWs in the Philippines. METHODS: Fifty female CHWs currently working in rural and urban areas participated in six focus group discussions with guidance from Kleinman's eight questions. With the written informed consent of the participants, discussions were recorded and transcribed by the Research Ass

Effects of diesel exhaust inhalation on cognitive performance in human volunteers: A randomized controlled crossover study.

BACKGROUND: Mounting evidence links exposure to traffic-related air pollution (TRAP) to impairment in cognitive functioning. OBJECTIVES: To determine if short-term, controlled exposure to diesel exhaust (DE) adversely affects one or more cognitive function domains. METHODS: We carried out a double-blinded crossover design with 28 healthy, adult volunteers. Volunteers were exposed to two conditions for 120 min each, on separate order-randomized occasions: filtered air (FA) and DE (300 µg/m3 PM2.5) at the Air Pollution Exposure Laboratory (APEL) at Vancouver General Hospital (VGH). Volunteers

A one health study on phylogenetics and risk of pathogenic intestinal parasites at a ranch in Inner Mongolia.

Cryptosporidium, Giardia duodenalis, and Enterocytozoon bieneusi are widespread zoonotic pathogens causing gastrointestinal diseases in humans and various animal species. Inner Mongolia, a major beef production region in China, there is a notable absence of comprehensive research on intestinal parasitism. Thus, timely and comprehensive diagnosis is essential to mitigate disease spread and minimize economic losses in the livestock industry. In this study, we collected fecal samples from cattle and humans, as well as soil and water samples, and all samples were tested for pathogenic intestinal p

Falls Risk, Circadian Rhythms and Melatonin: Current Perspectives.

Aging is associated with weakening of the circadian system. The circadian amplitude of most physiological variables is reduced, while the circadian phase becomes more labile and tends to occur earlier with advancing age. As the incidence of falls in older persons could follow circadian variations, a better understanding of conditions in which falls occur can lead to the implementation of countermeasures (such as adjusting the scheduling of hospital staff, or changing the timing of anti-hypertensive medication if falls are related to undesirable circadian patterns of blood pressure and/or heart

A Qualitative Study of Perceived Risk of Occupational Exposure to HIV and Use of Post Exposure Prophylaxis Services Among Health-Care Workers in Tanzania.

BACKGROUND: Occupational exposure to HIV continues to present a risk of HIV infections to health-care workers (HCWs) in low-income countries. Since 2005, policies in Tanzania have been in place to guide the implementation of HIV/AIDS post-exposure prophylaxis (PEP) interventions in the workplace. However, little is known about how frontline HCWs experience and view these interventions. This study aimed to explore how HCWs perceive their HIV infection risk and capture their experiences of workplace HIV/AIDS interventions. METHODS: A descriptive qualitative design was used. Medical and nonmedic

Comparison of heat strain recovery in different anti-heat stress clothing ensembles after work to exhaustion.

A hot environment combined with physically demanding tasks can subject workers to a higher risk of heat stress. A series of regulations and guidelines have been proposed to design appropriate anti-heat stress work uniform to reduce body heat strain. The present study aimed to examine heat strain recovery in different anti-heat stress clothing ensembles after work to exhaustion in the heat. 10 healthy males performed intermittent treadmill running/walking to exhaustion, followed by 30min passive recovery sitting in a climatic chamber, which simulated the hot and humid outdoor environment (34°C

Management of abnormal uterine bleeding in low- and high-resource settings: consideration of cultural issues.

In non industrialized countries the incidence of heavy menstrual bleeding (HMB) appears to be similar to that of industrialized countries, although data is scanty. In low-resource settings, women with abnormal uterine bleeding (AUB) often delay seeking medical care because of cultural beliefs that a heavy red menstrual bleed is healthy. Efforts to modify cultural issues are being considered. A detailed history and a meticulous examination are the important foundations of a definitive diagnosis and management in low-resource settings but are subject to time constraints and skill levels of the s

[Japanese version of the Short-Memory Questionnaire: memory evaluation in Alzheimer's disease].

BACKGROUND AND PURPOSE: Memory deficit is a common sign of Alzheimer's disease (AD), which appeared generally in the early stage of the disease. Therefore, evaluation of memory is important for management of patients and for clinical research of AD. The Short-Memory Questionnaire (SMQ), an easily administered, informant-based scale, which was developed by Koss et al. (1993), is a standardized, validated, and reliable tool to assess everyday memory problems. In the present study, we prepared a Japanese version of the SMQ and examined its reliability and validity in assessing AD patients. SUBJE

Efficacy of a smartphone app to improve mental health among emergency service workers: A randomised controlled trial.

BACKGROUND: Emergency service workers (ESWs) are routinely exposed to highly stressful and potentially traumatic events leading to high rates of psychological distress. Early intervention is vital to prevent chronic impairment and/or psychiatric disorders, with digital health innovations (e.g., smartphone apps) offering a potential means of scaling such intervention while overcoming barriers to help-seeking in this population. This study aims to evaluate the efficacy of an app designed to reduce psychological distress and related outcomes in ESWs. METHODS: We conducted a randomised controlled

Implementation of an efficacious intervention for high risk women in Mexico: protocol for a multi-site randomized trial with a parallel study of organizational factors.

BACKGROUND: Studies of implementation of efficacious human immunodeficiency virus (HIV) prevention interventions are rare, especially in resource-poor settings, but important, because they have the potential to increase the impact of interventions by improving uptake and sustainability. Few studies have focused on provider and organizational factors that may influence uptake and fidelity to core intervention components. Using a hybrid design, we will study the implementation of an efficacious intervention to reduce sexually transmitted infections (STIs) among female sex workers (FSWs) in 12 ci

Co-exposure to metals and polycyclic aromatic hydrocarbons, microRNA expression, and early health damage in coke oven workers.

BACKGROUND: All humans are now co-exposed to multiple toxic chemicals, among which metals and polycyclic aromatic hydrocarbons (PAHs) are of special concern as they are often present at high levels in various human environments. They can also induce similar early health damage, such as genetic damage, oxidative stress, and heart rate variability (HRV). Exposure to metals, PAHs, and their combined pollutants can alter microRNA (miRNA) expression patterns. OBJECTIVES: To explore the associations of metal-PAH co-exposure with miRNA expression, and of the associated miRNAs with early health damag

The Role of Sleep Banking in Reducing Cognitive and Motor Impairments from Subsequent Sleep Restriction: A Narrative Review.

Sleep banking, i.e., preemptively obtaining extra sleep prior to anticipated sleep loss, has been proposed as a strategy to reduce the cognitive and physiological consequences of sleep deprivation. However, our understanding remains incomplete regarding the effectiveness of preemptive sleep extension in enhancing resilience to sleep loss. A comprehensive literature search was conducted using PubMed, MEDLINE, and Embase for studies published between 2004 and 2025. Following a comprehensive literature search, we identified 12 studies meeting the inclusion criteria-seven primary experimental tria

Multimodal home-based rehabilitation intervention after discharge from inpatient geriatric rehabilitation (GeRas): study protocol for a multicenter randomized controlled trial.

BACKGROUND: Geriatric rehabilitation aims to maintain the functional reserves of older adults in order to optimize social participation and prevent disability. After discharge from inpatient geriatric rehabilitation, patients are at high risk for decreased physical capacity, increased vulnerability, and limitations in mobility. As a result, ageing in place becomes uncertain for a plethora of patients after discharge from geriatric rehabilitation and effective strategies to prevent physical decline are required. Collaboration between different health-care providers is essential to improve conti

How the war on drugs impacts social determinants of health beyond the criminal legal system.

There is a growing recognition in the fields of public health and medicine that social determinants of health (SDOH) play a key role in driving health inequities and disparities among various groups, such that a focus upon individual-level medical interventions will have limited effects without the consideration of the macro-level factors that dictate how effectively individuals can manage their health. While the health impacts of mass incarceration have been explored, less attention has been paid to how the "war on drugs" in the United States exacerbates many of the factors that negatively im

Knowledge Update on the Economic Evaluation of Pacemaker Telemonitoring Systems.

(1) Introduction: In the last two decades, telemedicine has been increasingly applied to telemonitoring (TM) of patients with pacemakers; however, presently, its growth has significantly accelerated because of the COVID-19 pandemic, which has pushed patients and healthcare workers alike to seek new ways to stay healthy with minimal physical contact. Therefore, the main objective of this study was to update the current knowledge on the differences in the medium-and long-term effectiveness of TM and conventional monitoring (CM) in relation to costs and health outcomes. (2) Methods: Three databas
