A heparin induced thrombocytopenia Type II (HIT) is a dangerous complication of heparin therapy. Bleeding, but above all serious thromboembolic complications, which may result in crippling disabilities or even death, can develop. Deep vein thrombosis of the right leg occurred in a 77-year-old woman after percutaneous cardiac catheterization via the right femoral vein, performed to assess mitral valve disease with atrial fibrillation. A method to induce microthrombi in small mesenteric arteries (30-40 microns) has been developed to study platelet reactions and to investigate antithrombotic drugs. This model was used to evaluate the effect of Ca-heparin. Mesenteric lesions are induced in the vascular system of Wistar rats with an argon laser. Heparin therapy in acute stroke is a controversial issue. It is uncertain, whether heparin has a therapeutic or preventive effect in the early phase of the stroke. We have recently developed a new device for continuously measuring SIPA. Using the device, we have investigated the effects of heparin on SIPA. Heparin itself enhanced SIPA in the absence of agonists in a dose-dependent manner, whereas heparinoids had minimal effects on SIPA. A case of white clot syndrome in a 61-year-old man is reported. The patient suffered simultaneous aortic and vena caval thromboses while receiving heparin therapy for pulmonary embolism. Thrombocytopenia was noted. Thrombectomies successfully relieved the thromboses. All patients who underwent pelvic lymphadenectomy for prostatic carcinoma over a five-year period were reviewed to evaluate the effect of minidose heparin prophylaxis on lymphocele formation and postoperative complications. A case is reported of a multiple trauma victim who developed severe vascular spasm whilst receiving heparin-dihydro-ergotamine thromboembolic prophylaxis which ultimately led to amputation of the right foot. Identification of high risk groups for this complication is attempted from a review of the literature. Treatment should be with intra-arterial or intravenous nitroprusside or nitrates. Little information is as yet available on the use of prostacyclin in this condition.

A retrospective study of 192 patients operated on by three members of the Edward Wilson Colon and Rectum Unit, Sydney Hospital, was carried out. All operations performed involved dissection within the pelvis. Prophylactic administration of low-dose heparin was used for 71 of these patients.
