BACKGROUND: Distal humeral fractures of type C by AO are serious musculoskeletal injuries, affecting the function of the elbow and upper limb. The authors present the results of treatment of these injuries with the use of two LCP plates.
MATERIAL AND METHODS: We report on 25 patients aged 26-75 years who were operated on between 2008 and 2010. We examined 20 patients. Treatment results were evaluated basing on patient histories (Oxford Elbow Score), clinical examination (Mayo Elbow Score) and radiographs. We rated the percentage loss of movement in relation to the healthy limb.
RESULTS: Bony…

BACKGROUND: Radically different conclusions exist in the pediatric orthopaedic and vascular literature regarding the management of patients with a pink hand but no palpable radial pulse in association with a supracondylar humeral fracture.
METHODS: One thousand two hundred and ninety-seven consecutive, operatively treated supracondylar humeral fractures in patients presenting to a level-I pediatric trauma center from January 2003 through December 2007 were studied retrospectively. Clinical records were reviewed to determine vascular and neurological examination findings, Gartland classificati…

Operative treatment for neglected fracture of lateral humeral condyle (LHC) is difficult because of contracted muscle, fibrous tissue formation, and indistinct bony edges. Its success depends on the ability to preserve blood supply during the surgery. We retrospectively reviewed eight cases of neglected fracture of LHC in children treated with open reduction with selected multiple 'V' lengthening of common extensor muscle and internal fixation. The patients were between 3 and 8 years of age. The period of neglect was between 3 and 20 weeks. Four patients with displacement of more than 10 mm an

Although proximal humeral fractures are common diagnosis in emergency, cases of axillary artery injuries are rare. Authors present a patient, who underwent high energy trauma during a traffic accident like a pedestrian. Comminuted proximal humeral fracture was complicated with an occlusion of the third part of the axillary artery. There was necessary an urgent surgery, ostesynthesis with locking plate and reconstruction of the axillary artery by interposition of expanded polytetrafluoroethylene (ePTFE) graft. Postoperative course was uncomplicated, patient had no symptoms of ischaemia of extre…

OBJECTIVES: Proximal and diaphyseal humeral fractures are common especially in the elderly, presenting as a challenging problem due to their high complication rates following surgical treatment. In this prospective study, we evaluated the results of patients treated with the PHILOS (Proximal Humeral Internal Locking System) locking plate, a new technique recently developed by the AO/ASIF.
METHODS: Forty-one patients who were treated with the PHILOS plate for proximal humeral fractures were evaluated in two age groups. Group A included 24 patients (12 males, 12 females; mean age 47 years; rang…

Articular fractures of the distal part of the humerus represent a challenging therapeutic problem, because they require thorough understanding of the complex local anatomy, extensile approaches, and reduction and fixation of small fragments that mainly consist of subchondral bone and articular cartilage. This report presents two cases of a unique fracture pattern of the lateral humeral column with a fracture line in the coronal plane separating the capitellum with a substantial portion of the lateral trochlear ridge, and with a second fracture line in the sagittal plane separating the residual

In this prospective study we evaluated the functional and radiological results obtained in 62 patients who had been treated for fracture of the humeral head by internal fixation with a cloverleaf plate. They were examined postoperatively, after 14 weeks and after an average follow-up of 75 weeks. Nine patients (14.5%) had dislocated 2-part fractures, 36 had 3-part fractures (58%; additional dislocations of the greater or lesser tubercle), 13 patients (21%) had 4-part fractures and 4 (5.6%) had luxation fractures of the humeral head. Early functional physiotherapy was started on the third day a

The purpose of this study was to evaluate predictors of fracture-induced humeral head ischemia. Between February 1998 and December 2001, 100 intracapsular fractures of the proximal humerus, treated by open surgery, were included in a prospective surgical evaluation protocol (mean age, 60 years; minimum, 21 years; maximum, 88 years; 45 men; 57 right shoulders). Fracture morphology was assessed following a structured questionnaire and based on radiographic and intraoperative findings. Perfusion was assessed intraoperatively by observation of backflow after a borehole was drilled into the central

INTRODUCTION: Elbow fractures account for 7-9% of all fractures in childhood, while the supracondylar type is the most common (up to 70%). 84% of all supracondylar elbow fractures occur under the age of 10 years (most often between the age of 5 and 8). The aim of this study was to present the technique and results of treatment of supracondylar fractures with grade III fragment dislocation in pediatric population. Treatment included orthopedic repositioning and percutaneous pin fixation.
MATERIAL AND METHODS: Over the period 1991-2000, 255 patients were hospitalized due to supracondylar fractu

PURPOSE OF THE STUDY: Evaluate outcome of different techniques used for radial nerve repair after humeral shaft fractures. Identify situations where nerve exploration is required or inversely unneeded. Evaluate the risk of more systematic nerve exploration in case of radial nerve palsy.
MATERIAL AND METHODS: Over a 10 year period, we cared for 62 patients with humeral fractures associated with radial nerve palsy who were addressed directly to our unit or referred after prior care. There were 54 men (mean age 30 years, 17 - 66) and 8 women (mean age 40 years, 19 - 80). There were 40 fractures …

A comparison was made between 44 humeral fractures treated conservatively with functional bracing (Group 1) and 45 treated by operation with a locking nail (Group 2). Thirty-eight of the 44 patients in Group 1 (86%) and 22 of the 45 patients in Group 2 (47%) regained full movement of the shoulder joint. The functional end results were somewhat better in Group 1 although over 90% of patients in both groups were able to clasp their fingers behind their necks and to reach up their backs without restriction. Two patients developed a pseudarthrosis in Group 1, and 2 patients in Group 2 required ope…

Twenty-one patients, 11 male and nine female, with fracture of medial humeral epicondyle were treated. The mean age of the patients was 21 (range 8-52) years. The average initial displacement was 13 (range 3-24) mm, and four out of 21 patients had a dislocation of the elbow joint. Two patients were first treated conservatively, but later on operation for removal of the non-united fragment and reattachment of the ligaments and muscles was necessary. One patient was treated by primary excision of the fragment. Eighteen patients were treated by open reduction and internal fixation using self-rein

The value of arthrography in children's fractures is well established, but the accuracy of this technique has not been reported. We assessed the usefulness of arthrography by performing this test on a series of lateral humeral condyle fractures and, when possible, compared the arthrographic findings with the findings at operation to assess the accuracy of the test. Twenty-four consecutive patients were evaluated, and 13 patients underwent both arthrography and open reduction. We conclude that arthrography is a useful and accurate test when performed on children with lateral humeral condyle fra

In the time from 1972 through 1985 173 children with supracondylar fractures of the humerus were treated according to the fracture type with two upper arm plaster splints, nail extension from von Ekesparre and in special cases with open reduction and fixation with Kirschner wires-osteosynthese. As primary complications due to accidents we discovered 4% lesions of the arm nerves and in 1.5% disturbance of the blood circulation, of which 0.5% were due to vessel lesions. All nerve lesions could be treated conservatively, however, one child had to be operated because of ischemia of the lower arm a
