Drooping shoulder is a roentgenographic abnormality consisting in a discontinuity in the scapulo-humeral arch due to inferior subdislocation of the humeral head. Drooping shoulder is commonly seen in patients with injuries to the shoulder or neurological loss. Two cases of drooping shoulder due to staphylococcal arthritis (after local corticosteroid injections) are reported herein. The mechanism of the downward humeral displacement in septic arthritis is discussed. The other causes of drooping shoulder are reviewed. Precautions needed to avoid increasing the displacement during rehabilitation …

The study aimed to describe the changes in biomechanical properties of the supraspinatus tendon, deltoid muscle, and humeral head post arthroscopic rotator cuff repair using shear wave elastography. Shear wave velocity of the tendon, deltoid, and humeral head of 48 patients was measured at predetermined sites at 1 week, 6 weeks, 12 weeks, 6 months, and 12 months post repair. One-way ANOVA with Tukey's correction and Spearman's correlation were performed. Mean±SEM healing tendon stiffness, adjacent to tendon footprint, increased from 1 week (6.2±0.2 m/s) to 6 months (7.5±0.3 m/s) and 12 months …

BACKGROUND: Treatment of young, active patients with symptomatic glenohumeral osteoarthritis, excessive glenoid retroversion, and static posterior humeral subluxation is challenging. Correction of glenoid retroversion may lead to centric loading and perhaps recenter the humeral head. We describe the functional and radiologic outcomes after corrective osteotomy of the glenoid in this population of patients.
MATERIALS AND METHODS: In this retrospective study, we included 10 shoulders (8 patients) that were observed for a mean of 33.4 months (range, 24-52 months) after corrective osteotomy of th…

Osteonecrosis of the humeral head is an uncommon condition that occurs when bone vascularity is impaired. The causes underlying osteonecrosis of the humeral head seem to be multifactorial and can be traumatic or nontraumatic. Although clinical symptoms and causative factors affect the treatment of each patient, staging is the most objective criterion in deciding on the most appropriate treatment. Conservative treatment can be effective in stages I and II. Surgical intervention is advocated when symptoms are persistent and signs of collapse are evident. The surgical options include arthroscopic…

Electromyographic (EMG) activity of selected shoulder girdle muscles was analyzed during voluntary posterior subluxation of the glenohumeral joint in four subjects. Although there was a great deal of variation in the muscle activation patterns during subluxation, subjects either pushed the humeral head backwards with the anterior muscles (deltoid, biceps) or pulled the humeral head backwards with the infraspinatus and/or posterior deltoid. In all cases, inhibition of the scapular rotators and winging of the scapula accompanied this maneuver. The method used in this study is presented as an obj…

The radiographic manifestations of renal osteodystrophy may be articular as well as osseous. The latter are well recognized, i.e., subperiosteal and subchondral bony resorption. Recently attention has been directed to the occurrence of an erosive arthritis of the hands and wrists in hyperparathyroidism. The authors present six patients with humeral head erosions, all of whom were on chronic long-term hemodialysis. These intra-articular erosions occurred at the "bare" area of the humeral head and thus represent an erosive arthritis and therefore can be distinguished from the usual sites of subc…

It would stand to reason that, in shoulder instability patients with bipolar bone loss, the combination of a bone block procedure and a remplissage procedure would provide better results than each one alone. Why would this be the case? When performing these procedures in the lateral decubitus position for patients with critical bipolar bone loss, the humeral head is anteriorly and inferiorly subluxed. This is most likely due to the incompetent restraints when in traction. A bone block procedure alone doesn't necessarily reduce the glenohumeral center of rotation; rather, it increases the "jump…

BACKGROUND: This study assessed the therapeutic effectiveness of medical training therapy (MTT) in reducing the upward migration of the humeral head during shoulder abduction, focusing on enhancing the scapulothoracic muscles to improve shoulder joint stability. The primary outcome measure was the vertical displacement of the humeral head's center of rotation before and after treatment.
METHODS: From January 15, 2023 to January 3, 2024, 63 patients with rotator cuff tears were randomly assigned to either the experimental (n = 32) or control (n = 31) group. Additionally, 49 healthy volunteers …

In cases with complex glenoid bone defects, bony increased-offset reverse shoulder arthroplasty can restore glenoid bone stock and improve stability using autografts harvested from the humeral head. However, conventional grafting methods may be insufficient when glenoid defects or deformities are complex. We present a novel bracket-shaped bone grafting technique applied in a patient with advanced rheumatoid arthritis and significant glenoid erosion. The graft was sculpted from the resected humeral head to match the glenoid defect in a Lego block-like fashion, resulting in stable fixation and f…

Fractures of the proximal humerus are common, particularly seen in elderly, female patients. Using open reduction and internal fixation good clinical results can be achieved in general. But even today not every problem has been solved in the treatment of proximal humeral fractures. Varus displaced fractures are particularly challenging, especially when the medial column is destroyed. Anatomical reduction of the humeral head and medial bone contact are crucial for a good surgical outcome. Otherwise a secondary varus collapse and/or an implant failure are predictable. Further challenges are the …

BACKGROUND: Anterior shoulder pain after reverse shoulder arthroplasty (RSA) is not uncommon and may be due to humeral impingement against the conjoint tendon during internal rotation (IR). It is unknown what effect different implant designs and configurations have on conjoint tendon impingement. The purpose of this study was to investigate the influence of several RSA implant design parameters on conjoint tendon impingement during IR.
METHODS: Twelve upper extremity cadavers were dissected to visualize and digitize the path of the coracobrachialis using a tracking system. This data were tran

The etiology of joint tissue degeneration following rotator cuff tear remains unclear. Thus, the purpose of this study was to understand the timeline of protease activity in the soft tissues of the shoulder (tendon, muscle, and cartilage) that may lead to down-stream degeneration following rotator cuff tear. A well-established rat model involving suprascapular nerve denervation and supraspinatus/infraspinatus tendon transection was employed. Histological staining and/or micro-computed tomography (µCT) were used to observe structural damage in the supraspinatus tendon and muscle, humeral head c…

In 23 patients suffering from recurrent posttraumatic dislocation of the shoulder joint the derotation of the humeral head was performed as described by Weber. A description of the operative technique is given. As recommended by Weber an angled 1/2-tubular plate with 7 holes was used for stable osteosynthesis 17 times. After movement treatment removal of the osteo-synthesis-material was possible 6 to 8 months later. Because of high absence of recidivity with repair of full mobility and full strength the derotation of the humeral head is a treatment of choice in cases of recurrent posttraumatic…
