Associations among upper airway structure, body position, and obesity in skeletal Class I male patients with obstructive sleep apnea.

Interactions between upper airway structure and posture in relation to obesity were studied in a sample of 61 adult Class I skeletal type ma … ontrol subjects. No correlations were observed between the variables related to the position of the hyoid bone with BMI in either patients with OSA or control subjects after the change in posture. On the basis of these findings, we propose that when patients with OSA change their body position from upright to supine (1) the patient's neck is more extended, a …

Somnofluoroscopy, computed tomography, and cephalometry in the assessment of the airway in obstructive sleep apnoea.

BACKGROUND: Assessments of the upper airways in patients with the obstructive sleep apnoea syndrome are usually carried out on awake patient … the 10 patients with hypopharyngeal collapse shown by somnofluoroscopy having an inferiorly placed hyoid bone according to cephalometry (distance from the mandibular plane to the hyoid bone (MP-H distance) increased); the one patient with no hypopharyngeal collapse had a normal MP-H. By contrast, six of the 11 patients had a normal or supranormal hypopharyn …

Virtual autopsy in hanging.

The aim of the study was to compare postmortem computed tomographic (CT) imaging findings from a case of hanging with the autopsy findings. The CT showed a good match with autopsy findings especially for bone, soft tissue, and spinal injuries. The CT images of the injuries of the neck obtained by CT scan clearly showed the ligature mark, the hemorrhagic suffusion in the soft tissue (thickening of derma) and in the muscles (sternocleidomastoid) of the neck. Furthermore, CT 3-dimensional reconstruction showed brain edema, fracture of the left posterior horn of the hyoid bone, and a grade 1 retro…

Accidental ligature strangulation by an ironing machine: an unusual case.

In this paper, we present a case of a 53-year-old woman who had her headscarf catch on the cylinder ironing machine in the laundry of the hospital where she worked. The hospital workers found the woman dead with her head stuck to the ironing machine. After the death scene investigation and autopsy were completed, it was determined that the death occurred as a result of accidental ligature strangulation. Accidental ligature strangulation in which an article of clothing is caught in such an electrical machine and strangles the wearer is very rare. This case highlights the fact that these kinds o

The stylohyoid ligament in Hurler syndrome and related conditions: comparison with normal children.

The stylohyoid ligament is seen on lateral radiographs of the neck to be normally calcified to some extent in approximately 25% of all children between ages 1 1/2 and 15. When it appears, the ossification pattern is a relatively thin configuration in 90% of children. In 8 out of 9 children with Hurler syndrome, the stylohyoid was calcified, and was thicker than in normal children. This finding illustrates the nature of the stylohyoid ligament as a bone analogue, and its participation in the skeletal deformation pattern of dysostosis multiplex.

Erosive parapharyngeal space teratoma.

An erosive teratoma of the parapharyngeal space is described. Our patient at three months of age presented with an eight-week history of torticollis. Diagnosis was strongly suggested by CT scan which clearly demonstrated bony, fluid and intermediate densities suggestive of a teratoma. The tumor had eroded the petrous bone with inferior extension to the level of the hyoid cartilage. An angiogram revealed compromise of the internal carotid artery at the skull base. Surgical resection at five months removed the tumor from the right parapharyngeal space and included a portion of the petrous bone i

Comparison of Pharyngeal Airway between Mandibular Setback Surgery Patients (Skeletal Class III) and Nonsurgery Patients (Skeletal Classes I and II).

PURPOSE: We investigated the pharyngeal airway dimensions and their correlations in patients who underwent mandibular setback surgery versus … atients, respectively. No differences were observed in EOP between the three skeletal patterns. The hyoid bone of Class III patients was significantly anterior to that of Class I/II patients. Furthermore, UOP had a moderate negative correlation with soft palate length. Postoperatively, no significant difference (UOP, TOP, EOP, soft palate width, and hyoid bo …

Craniofacial features of adult obese obstructive sleep apnoea patients in relation to the obesity onset - A pilot study.

INTRODUCTION: Obesity and craniofacial structures are aetiologies of obstructive sleep apnoea (OSA). The effect of obesity onset on the cran … prognathic mandible, longer lower facial height, protrusive incisors, a more caudal position of the hyoid bone and a wider lower airway. The late-onset group (n = 21) had more proclined and protrusive upper incisors, a shallower overbite, a more inferiorly positioned hyoid bone and an obtuse craniocervical angle. The overall obese group showed a combination …

Adjunctive procedures to neck rejuvenation.

Rejuvenation of the neck often requires more than just a neck lift. Various steps and procedures exist to enhance the surgical technique or overall result. Fibrin sealants can be used to improve the recovery process and obviate the need for drain placement. Chin augmentation can be a critical part of creating a more refined neckline. Submandibular gland excision has been put forth as helpful to the overall aesthetic result. A low and anteriorly positioned hyoid bone creates an unattractive neckline that is difficult to treat. This article focuses on techniques beyond lifting and resurfacing th…

Clinical and ultrasonographic assessment of airway indices among non-pregnant, normotensive pregnant and pre-eclamptic patients: a prospective observational study.

BACKGROUND: Prediction of a difficult airway is of paramount importance for an anaesthesiologist. Various anatomical and physiological factors contribute to a difficult airway in pregnant females, especially those with pre-eclampsia. The aim of the study was to assess airway indices using both routinely used clinical methods and ultrasound. METHODS: Fifty-five non-pregnant females, 55 normotensive pregnant females and 55 females with pre-eclampsia were included in this prospective study. Clinical airway assessment was the modified Mallampati score, thyromental distance, hyomental distance, hy

Thyroglossal duct cyst masquerading as a laryngocele.

Thyroglossal duct cysts (TGDCs) are the most common congenital neck mass and often present in the paediatric population as a painless mass in the midline. A lateralised neck mass presenting for the first time in an adult may raise more concern for malignancy or a laryngocele. A 50-year-old man presented with an asymptomatic right level II neck mass adjacent to the thyroid cartilage. Preoperative CT revealed a cystic mass right of the midline with an intralaryngeal component. Intraoperatively, the lesion tracked towards the central hyoid bone; a Sistrunk procedure was performed. Postoperative p…

Alterations to Swallowing Physiology as the Result of Effortful Swallowing in Healthy Seniors.

A significant proportion of healthy seniors report difficulty swallowing, thought to result from age-related decline in muscle bulk/function. Effortful Swallowing (ES) is used both as a compensatory maneuver to improve pharyngeal propulsion/clearance and has been proposed as an exercise to improve pharyngeal strength. This study sought to quantify the immediate kinematic, temporal, and functional changes during an ES maneuver to quantify its exercise potential to combat age-related changes in swallowing. Videofluoroscopy data were collected from 44 healthy seniors (21 male) over 65 years old (

Temporal variability in the deglutition literature.

A literature review was conducted on temporal measures of swallowing in healthy individuals with the purpose of determining the degree of variability present in such measures within the literature. A total of 46 studies that met inclusion criteria were reviewed. The definitions and descriptive statistics for all reported temporal parameters were compiled for meta-analysis. In total, 119 different temporal parameters were found in the literature. The three most-frequently occurring durational measures were upper esophageal sphincter opening, laryngeal closure, and hyoid movement. The three most
