The immune response in SAPHO syndrome: deficiency, hyper- responsiveness, or both?

The pathophysiology of SAPHO syndrome still remains to be determined. However, like in other forms of spondylarthritides, this rare condition seems to result from the combination of genetic, environmental and immunological factors. Surely, SAPHO syndrome cannot be simply regarded as the adult form of the 'caricatural' DIRA (deficiency in interleukin-1 receptor antagonist) syndrome, although this purely genetic disease also causes multiple osteomyelitis and pustular rashes. An initial bacterial trigger, mainly represented by the cutaneous saprophyte Propionibacterium acnes, could take advantage

Novel acid-labile subunit ( IGFALS ) mutation p.T145K (c.434C>A) in a patient with ALS deficiency, normal stature and immunological dysfunction.

We report a novel missense mutation p.T145K in the insulin-like growth factor (IGF) acid-labile subunit (IGFALS) gene identified in a Turkish patient with normal growth, transient pancytopenic episodes and signs of immunological dysfunction. Because of recurrent cutaneous mycoses and absence of pubertal development until the age of 14.75 years we determined several endocrine parameters in order to rule out autoimmune-polyendocrine syndromes. Despite a normal height between the 25th and 50th percentile we found severely decreased IGF-1 and undetectably low IGFBP-3 levels. Laboratory signs of im

Complement component C6 deficiency in a Spanish family: implications for clinical and molecular diagnosis.

Complement component C6 deficiency is a genetic disease presenting as increased susceptibility to invasive Neisseria meningitidis infections. This disorder has rarely been diagnosed in the Spanish population. In this work we report the immunochemical and molecular characterization of complement C6 deficiency in a Spanish patient showing no detectable functional activity of either the classical or alternative complement pathways and reporting a history of several episodes of meningococcal meningitis. The levels of individual complement components C3, C4, C5, C7, C8 and C9 were within the normal

Severe Atopic Dermatitis: Clinical Confusion With Hyper IgE Syndrome.

Hyper IgE syndrome (HIES) is a rare primary immunodeficiency characterized by chronic eczema, recurrent staphylococcal infections on the skin and pulmonary system, and high serum IgE concentrations. The first clinical sign of HIES is eczema, usually in early infancy. Severe atopic dermatitis (AD) may also mimic HIES with findings of eczema, high serum immunoglobulin E levels, and eosinophilia. Therefore, differential diagnosis may be difficult. Here, we present a case with eczematoid skin rashes, asthma, elevated serum IgE levels, and skin infections that started in infancy and were followed f

Immunodeficiency states and associated rheumatic manifestations.

For immunodeficiency states, the past year has been marked with the discovery of a series of events at the gene level and cell-to-cell interaction, particularly in common variable immunodeficiency, severe combined immunodeficiency, IgA deficiency, and immunodeficiency associated with IgM hyperimmunoglobulinemia. Clinical expression, especially in the musculoskeletal system, as well as infectious complications and therapy, also have been reported.

Humoral immunodeficiency: a review.

Humoral immunodeficiency is a heterogeneous group of rare disorders. The availability of IVGG has meant significant improvement for many patients, but infections continue to occur even on Ig replacement therapy. This may be partially due to associated T cell defects, the inability of current therapy to provide antibody effectively to mucosal surfaces, or the difficulty of providing antibody of sufficiently high titer, affinity, or specificity during infection. A combination of maintenance Ig replacement coupled with increased amounts of Ig and appropriate antibiotics during specific infections

Goblet cell number in the nasal mucosa relates to cell-mediated immunity in patients with antibody deficiency syndromes.

Histopathological changes in the nasal mucosa were studied in biopsies from the inferior turbinate in 22 patients with common variable immunodeficiency (CVID) and 16 patients with selective IgA deficiency. Most patients with CVID had fewer goblet cells than the healthy controls (p less than 0.001), whereas patients with selective IgA deficiency often had manyfold increased numbers (p less than 0.001). The number of goblet cells showed a significant (p less than 0.001) relationship to the cell-mediated immune function. Patients with lowered cell-mediated immune function, as measured by in vivo

Detection of the carrier state in combined immunodeficiency disease associated with adenosine deaminase deficiency.

A large pedigree containing a child with severe combined immunodeficiency disease (CID) associated with adenosine deaminase (ADA) deficiency was investigated to ascertain if heterozygotes could be detected by measuring red cell ADA activity. 9 of 17 individuals in three generations who were at risk for being heterozygous had decreased red cell ADA activity. This genetic information establishes one form of CID as an autosomal recessive disorder. The identified heterozygote population had a mean ADA value of 19.2 U/g hemoglobin (0.95 confidence interval; 14.0 to 24.4 U/g hemoglobin), which was a
