Headache and immunological/autoimmune disorders: a comprehensive review of available epidemiological evidence with insights on potential underlying mechanisms.

Several lines of evidence support a role of the immune system in headache pathogenesis, with particular regard to migraine. Firstly, alterations in cytokine profile and in lymphocyte subsets have been reported in headache patients. Secondly, several genetic and environmental pathogenic factors seem to be frequently shared by headache and immunological/autoimmune diseases. Accordingly, immunological alterations in primary headaches, in particular in migraine, have been suggested to predispose some patients to the development of immunological and autoimmune diseases. On the other hand, pathogeni

Maternal immunological aspects of pregnancy success and failure.

Historically, auto- and alloimmune maternal immunological abnormalities have been proposed to account for pregnancy loss. However, most of these immunological theories have not fulfilled the criteria for causality. Autoimmunity involving antibodies to cardiolipin and possibly phosphatidylserine is the immunological factor best associated with recurrent abortion in a condition known as the antiphospholipid syndrome, which occurs in < 5% of women undergoing recurrent pregnancy losses. Dichotomous T helper cell 1 (Th1) and Th2 cytokine/growth factor responses to trophoblast or other antigens are

Imiquimod: a potential weapon against Dupuytren contracture.

Dupuytren disease is a proliferative fibroplasia of the subcutaneous palmar tissue, occurring in the form of nodular and cords. Evidence is certainly accumulating for raised levels in Dupuytren's tissue of growth factors known to stimulate fibroblasts, Interleukin-1, basic fibroblast growth factor, transforming growth factor-beta, prostaglandin-F2, prostaglandin-E2, platelet derived growth factor and connective tissue growth factor have been suggested to have a role. Immune modification of profibrotic cytokines would provide a novel means to treat dupuytren contracture. Imiquimod cream 5% (Ald

Non-immunologic allograft loss in pediatric kidney transplant recipients.

Non-immunologic risk factors are a major obstacle to realizing long-term improvements in kidney allograft survival. A standardized approach to assess donor quality has recently been introduced with the new kidney allocation system in the USA. Delayed graft function and surgical complications are important risk factors for both short- and long-term graft loss. Disease recurrence in the allograft remains a major cause of graft loss in those who fail to respond to therapy. Complications of over immunosuppression including opportunistic infections and malignancy continue to limit graft survival. A

Monomicrobial necrotizing fasciitis complicating pregnancy and puerperium.

BACKGROUND: Necrotizing fasciitis is an uncommon, rapidly progressive, life-threatening infection involving the subcutaneous tissue and fascia. Usually, it is a synergistic polymicrobic infection that occurs in patients with coexisting factors predisposing them to bacterial inoculation and the spread of infection. CASES: We report a monomicrobial variant of necrotizing fasciitis affecting three otherwise healthy pregnant or postpartum women. The necrotizing fasciitis involved either the lower extremity or the abdominal wall. The causative bacteria were Streptococcus pyogenes (two cases) and S

The age-dependent reaction of the periodontal tissues to dental plaque.

An increase in the prevalence and severity of periodontal disease as a function of aging has been reported in the literature. Here the literature considered relevant to the etiology of this phenomenon is summarized. The age-dependent reactivity of the periodontal tissues may be the result of several local and systemic factors, such as the characteristics of the inflammatory cell infiltrate, gingival morphology, periodontal tissues, dental plaque, gingival fluid exudate, health status of the individual, hormonal changes and immunologic reactions. In most cases, the presence of several factors i

The epidemiology of primary biliary cirrhosis: immunological problems.

Primary biliary cirrhosis (PBC) has received considerable attention as an autoimmune disease, where in addition to immunological mechanisms, environmental factors may have an important role. Therefore the epidemiological analysis of disease prevalence and progression in association with immunological studies has been one of the hot areas of PBC research during recent years. In this paper the epidemiological data on PBC and its immunological marker--anti-mitochondrial antibodies--have been presented from a comparative viewpoint.

Post-Sepsis Syndrome after Critical Illness: Epidemiology and Long-term Outcomes.

Survival after sepsis has improved over recent decades, resulting in a growing population of sepsis survivors. Survivorship after sepsis is frequently marked by significant long-term morbidity, including new functional dependence, cognitive impairment, anxiety, depression, post-traumatic stress symptoms, recurrent hospitalization, and excess long-term mortality. This article presents the current knowledge on the epidemiology of long-term outcomes among critically ill adult sepsis survivors, with particular emphasis on patient-centered outcomes, their prevalence, and contributing factors, and a

Sex Matters in Neuroinfectious Diseases.

Although sex and gender have a major impact on the susceptibility and immunologic response to infectious diseases, these factors are often neglected. Identifying the mechanisms underlying sex-based differences in infectious diseases will facilitate the rational design and implementation of preventive and therapeutic strategies that reduce risk and improve outcomes for women and men. In this article, we discuss two examples in neuroinfectious diseases of how sex matters: (1) the heightened risk of cerebrovascular disease in women living with HIV infection and (2) the implications of Zika virus

Gingival pustules and sterile diffuse sclerosing osteomyelitis as a feature of synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome.

Synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome represents the rare co-occurrence of sterile inflammatory osteoarticular disease in association with a variety of cutaneous manifestations. Oral involvement is uncommon. The etiology of SAPHO is complex and is likely the combined result of infectious, genetic, and immunologic factors. Due to diverse clinical presentations, SAPHO is difficult to diagnose. Here, we describe the case of a 74-year-old man, who had a history of SAPHO syndrome and presented with gingival pustules and sterile diffuse sclerosing osteomyelitis of the …

Allergic eye disease.

Ocular allergy is one of the most common conditions encountered by pediatricians and ophthalmologists and is characterized by bilateral injection with itching as the predominant symptom. Risk factors include history of atopy (asthma, eczema, seasonal allergies). Basic and clinical research have provided insight into the immunologic mechanisms, clinical presentation, differential diagnosis, and pharmacologicmanagement of this condition. New pharmacologic agents have improved the efficacy and safety of ocularallergy treatment. This article discusses the classification of ocular allergy diagnosis

Herpes Gestationis after Ovodonation: Is Placenta the only Target of the Immune Reaction?

Herpes Gestationis (HG) is an autoimmune disorder that rarely complicates pregnancy. Its pathogenesis has not been clarified yet, as it derives from a complex interplay between immunologic, genetic, and hormonal factors. We present a case of HG occurring in a surrogate mother whose pregnancy was achieved via egg donation and in vitro fertilisation (IVF). The disease appeared at 26 weeks and 6 days of gestation and subsequently worsened despite the administration of oral and topic corticosteroids. A healthy baby was delivered at 35 weeks and 2 days of gestation by elective cesarean section and

A case cluster demonstrating the relationship between HLA concordance and virologic and disease outcomes in human immunodeficiency virus infection.

We present a detailed analysis of sexual HIV transmission from one source partner to two recipients. The HLA haplotypes between the source partner and one recipient were very similar with 7 out of 8 HLA alleles from four loci (HLA A, B, C and DRB) shared, while the other recipient shared only one allele. The immunologic outcomes between the two recipients differed dramatically, despite the absence of apparent virologic differences in their inoculums. We suggest that non-viral factors, which might be related to differences in the HLA profile, played a role in determining different CD4+ T-cells

Sudden cardiac death in a young HIV-positive man on effective antiretroviral therapy.

We describe the case of a young HIV-positive man on effective HAART with excellent viro-immunological control who presented a massive cardiac infarction. Despite the presence of clinical risk factors for cardiovascular disease, the patient had normal arterial carotid IMT values, known to be strong predictors of atherosclerosis and stroke. Interestingly, parameters of T-cell activation (CD8+CD38+) were shown to increase just before the onset of myocardial infarction. As T-cell activation is known to mediate atherosclerosis, the authors suggest that surrogate immunologic markers should be identi

Iron loading and disease surveillance.

Iron is an oxidant as well as a nutrient for invading microbial and neoplastic cells. Excessive iron in specific tissues and cells (iron loading) promotes development of infection, neoplasia, cardiomyopathy, arthropathy, and various endocrine and possibly neurodegenerative disorders. To contain and detoxify the metal, hosts have evolved an iron withholding defense system, but the system can be compromised by numerous factors. An array of behavioral, medical, and immunologic methods are in place or in development to strengthen iron withholding. Routine screening for iron loading could provide v

Immunologic issues in anti-retroviral therapy.

A number of drugs acting at different stages of viral replication have been shown to be effective anti-HIV agents in the laboratory, and several have been found to be active in patients. It has become evident that inhibition of viral replication in HIV-infected patients will result in an improvement in their immune function. However, as Robert Yarchoan, Hiroaki Mitsuya and Samuel Broder point out, complete immunoreconstitution generally does not occur in patients with established AIDS using currently available therapies. It is important to understand the factors that limit immunologic improvem

Allergic rhinitis and conjunctivitis. Help for the weeping nose and eyes.

Both the eyes and the nose are in constant contact with the external environment and thus are common targets for immune-mediated disease. Progress in understanding the pathophysiology of allergic nasal and ocular disease has greatly enhanced the approach to therapy. Chronic rhinitis may be caused by a variety of immunologic and nonimmunologic mechanisms. Allergic and nonallergic factors often coexist, which complicates diagnosis and therapy. In external ocular disorders, both IgE- and IgG-mediated mechanisms may be involved. Elucidation of these mechanisms will be important in improving diagno
