Group A Streptococcus: A Re-Emerging Pathogen? Two Autopsy Cases of Deadly Infections in Previously Undiagnosed Young Adult Males.

Streptococcus pyogenes [group A strep (GAS)] is not a novel pathogen, but it is a newly emerging deadly threat. Recent data suggests that GAS, like many other contagions, saw a rise in infections in 2022, likely due to the withdrawal of COVID-19 precautions. GAS can present as a localized infection, namely tonsillopharyngitis, impetigo, cellulitis, and scarlet fever, or as an invasive infection such as meningitis, pneumonia, necrotizing fasciitis, and puerperal sepsis. Even though GAS tonsillopharyngitis is not typically fatal, it can be complicated by acute rheumatic fever if antibiotics are …

Paraspinal muscle abscesses in children - A case report and review of the literature.

BACKGROUND: Few pediatric cases with myositis and abscesses of the paraspinal muscles have been previously reported. CASE DESCRIPTION: We herein report on a 3-year-old patient who developed an epidural abscess in a paraspinal muscle abscess, after a complication of infectious impetigo due to atopic dermatitis. The child improved through the administration of an antibacterial drug. The median age of seven patients with a history of paraspinal muscle inflammation and muscle abscess was 12 years old (3-15-years-old), few of which had underlying diseases, with methicillin-sensitive Staphylococcus…

Pustular Psoriasis: From Pathophysiology to Treatment.

Pustular psoriasis (PP) is a clinicopathological entity encompassing different variants, i.e., acute generalized PP (GPP), PP of pregnancy (impetigo herpetiformis), annular (and circinate) PP, infantile/juvenile PP, palmoplantar PP/palmoplantar pustulosis, and acrodermatitis continua of Hallopeau (ACH), which have in common an eruption of superficial sterile pustules on an erythematous base. Unlike psoriasis vulgaris, in which a key role is played by the adaptive immune system and interleukin (IL)-17/IL-23 axis, PP seems to be characterized by an intense inflammatory response resulting from in…

Listeriolysin S, a novel peptide haemolysin associated with a subset of lineage I Listeria monocytogenes.

Streptolysin S (SLS) is a bacteriocin-like haemolytic and cytotoxic virulence factor that plays a key role in the virulence of Group A Streptococcus (GAS), the causative agent of pharyngitis, impetigo, necrotizing fasciitis and streptococcal toxic shock syndrome. Although it has long been thought that SLS and related peptides are produced by GAS and related streptococci only, there is evidence to suggest that a number of the most notorious Gram-positive pathogenic bacteria, including Listeria monocytogenes, Clostridium botulinum and Staphylococcus aureus, produce related peptides. The distribu…

A study of haemolytic streptococci isolated from outpatients in dermatological clinics.

A total of 44 patients suspected of streptococcal infections were studied in outpatient clinics in Tokyo during the one year from December 1988 to December 1989. Employing bacteriological culturing and serodiagnosis, the following results were obtained. 1) There were 9 cases of impetigo and 15 cases of erysipelas with typical clinical manifestations and age distributions. 2) It seemed that some of the skin infections were caused by group A streptococci whose M-types were different from those of upper respiratory infections typically occurring in Japan. 3) The type distribution of group A strep…

Accelerating the development of a group A Streptococcus vaccine: an urgent public health need.

Group A Streptococcus (GAS) infections cause substantial worldwide morbidity and mortality, mostly associated with suppurative complications such as pharyngitis, impetigo, and non-suppurative immune syndromes such as acute rheumatic fever, rheumatic heart disease, and acute post-streptococcal glomerulonephritis. Deaths occur mostly in children, adolescents, and young adults in particular pregnant women in low- and middle-income countries. GAS strains are highly variable, and a GAS vaccine would need to overcome the issue of multiple strains. Several approaches have been used multivalent vaccin…
