Perinatal mortality in Japanese women diagnosed with gestational diabetes mellitus and diabetes mellitus.

AIMS: The objective of this study was to determine how many pregnant Japanese women with diabetes mellitus (DM)/gestational diabetes mellitus (GDM) experience perinatal mortality in the presence of fetal anomalies. METHODS: Our investigation included data from 205 secondary/tertiary obstetric facilities located widely in Japan. The Japan Ministry of Health, Labour and Welfare Vital Statistics of Japan was used for comparison. RESULTS: Of 237 941 women giving birth at 205 hospitals, 1796 (0.8%) and 13 037 (5.5%) had DM and GDM, respectively. The perinatal mortality rates (per 1000 births) wer

Long-Term Societal Costs After Births Before 24 Weeks of Gestation in Sweden.

AIM: Extremely preterm birth is associated with high morbidity and long-term support needs. This study estimated long-term medical and non-medical costs throughout childhood among children born before 24 weeks of gestation in Sweden. METHODS: This nationwide register-based study included 344 infants born before 24 weeks of gestation in Sweden between 2007 and 2018 who survived at least one year. Individual-level data from national health registers and social insurance records were linked to estimate costs of hospital care, outpatient care, prescribed drugs and parental support until the end o

Urbanisation and child health in resource poor settings with special reference to under-five mortality in Africa.

The health of children improved dramatically worldwide during the 20th century, although with major contrasts between developed and developing countries, and urban and rural areas. The quantitative evidence on urban child health from a broad historical and comparative perspective is briefly reviewed here. Before the sanitary revolution, urban mortality tended to be higher than rural mortality. However, after World War I, improvements in water, sanitation, hygiene, nutrition and child care resulted in lower urban child mortality in Europe. Despite a similar mortality decline, urban mortality in

[Against influenza in children, which actions?].

Each year, at any age, children are at risk of influenza illness during the epidemics. Children, especially those at school (attack rate close to 30%), have a major role in viral dissemination. Most of influenza illnesses occur in healthy children. Manifestations are typical, as in adults, in children over 5 years of age and less specific in younger children explaining misdiagnosis and underestimation in the youngest. Respiratory complications in outpatients include acute otitis media (close to 30% in children less than 3 years of age) and pulmonary disease (5 up to 10%). High fever 240 degree

A review of the literature on debriefing or non-directive counselling to prevent postpartum emotional distress.

BACKGROUND: childbirth generates powerful emotions and may lead to the development of symptoms of depression, anxiety, and trauma in some women. Debriefing and non-directive counselling have been used as early interventions to reduce the prevalence of depression and post-traumatic stress. METHODS: a review of the literature was conducted to describe the current state of knowledge on the effectiveness of a single debriefing session or non-directive counselling session to reduce depression and trauma symptoms in women following birth. FINDINGS: a total of three studies reported in four papers

AACAP Official Action. Summary of the practice parameters for child custody evaluation.

This summary is presented as a guide for clinicians evaluating the often delicate and complex issues surrounding a child custody dispute. The historical basis of child custody and the various judicial presumptions that have guided courts, as well as the differences between performing child custody evaluation and engaging in traditional clinical practice, are reviewed in the complete document. Issues that are common to all child custody disputes are presented, including continuity and quality of attachments, preference, parental alienation, special needs of children, education, gender issues, s

Clinical management of immigrants' immunization histories: a focus on Soviet health records and BCG.

Increasing numbers of immigrants from the former Soviet Union are settling in the United States each year, making it imperative for clinicians to know how to find and interpret immigrant children's immunization records. Records show that these children have usually received immunizations against tetanus, diphtheria, pertussis, poliomyelitis, measles, mumps and tuberculosis (BCG). They are occasionally vaccinated against influenza, smallpox and tularemia, but never against rubella, hepatitis B or H. influenzae meningitis. The Soviet immunization schedule differs significantly from the U.S. sche

Efficacy of delivery models in early intervention: Findings from Germany's nurse-family partnership on family and child welfare services and pediatric medical incidents in high-risk families.

BACKGROUND: This study examines the efficacy of two different delivery models of the ProKind intervention, a German adaptation of the Nurse-Family Partnership (NFP): The continuous model (CM) with visits exclusively delivered by NFP-trained midwives, and a tandem model (TM) with visits delivered by a midwife during pregnancy and a social worker postnatally. OBJECTIVE: To evaluate the effectiveness of different delivery models in the ProKind intervention on reducing surveillance by the child protection service (CPS), foster care placements, and pediatric medical incidents. Improving lifetime trajectories for vulnerable young children and families living with significant stress and social disadvantage: the early years education program randomised controlled trial.

BACKGROUND: Children who experience neglect and abuse are likely to have impaired brain development and entrenched learning deficiencies. Early years interventions such as intensive education and care for these children are known to have the potential to increase their human capital. The Early Years Education Program (EYEP) is a new program offered by the Children's Protection Society (CPS) in Melbourne, Australia. EYEP is targeted at the needs of children who have been or are at risk of being abused or neglected. It has the dual focus of seeking to address the consequences of abuse and neglec

The diagnostic and clinical significance of café-au-lait macules.

Café-au-lait, also referred to as café-au-lait spots or café-au-lait macules, present as well-circumscribed, evenly pigmented macules and patches that range in size from 1 to 2 mm to greater than 20 cm in greatest diameter. Café-au-lait are common in children. Although most café-au-lait present as 1 or 2 spots in an otherwise healthy child, the presence of multiple café-au-lait, large segmental café-au-lait, associated facial dysmorphism, other cutaneous anomalies, or unusual findings on physical examination should suggest the possibility of an associated syndrome. While neurofibromatosis type

Clinical and dermoscopic characteristics of melanocytic nevi in Turkish children and their relationship with environmental and constitutional factors.

BACKGROUND/AIM: The number of melanocytic nevi strongly influences risk of melanoma. Researchers have therefore been prompted to study the epidemiology of nevi, particularly in children. Our aim was to determine the clinical and dermoscopic characteristics of melanocytic nevi in Turkish children and their relationship with environmental factors. METHODS: A total of 180 children were randomly included in the study. A survey was applied including age, sex, sunblock use, sunburn history, mother's dressing style, mother's education, and income. Dermoscopic patterns of nevi were noted. RESULTS: A

A model for educational simulation of neonatal cardiovascular pathophysiology.

Full-body patient simulators provide a technological basis for clinical education without risk to real patients. In a previous study, we described a model for educational simulation of infant cardiovascular physiology. Using essentially the same methodology, we derive a mathematical model for the cardiovascular system of a healthy 1-week-old neonate. Computer simulations of this model result in vital signs that are close to target hemodynamic variables. Simulated systemic arterial pressure waveform and left ventricular pressure-volume loop are realistic, and the system reacts appropriately to

Measurement of PCBs, DDE, and hexachlorobenzene in cord blood from infants born in towns adjacent to a PCB-contaminated waste site.

There are limited data on the concentrations of common contaminants--polychlorinated biphenyls (PCBs), dichlorodiphenyl dichloroethene (pp'-DDE) and hexachlorobenzene (HCB)--in umbilical cord blood. Cord blood provides the primary direct measure of prenatal exposure to these contaminants, the key determinant of PCBs' neurodevelopmental toxicities. The objective of this study was to characterize cord blood levels of PCBs, pp'-DDE, and HCB among 751 infants who were born between 1993 and 1998 to mothers residing adjacent to a PCB-contaminated harbor in New Bedford, Massachusetts, and for whom th

Homeless families and their children's health problems. A Utah urban experience.

Descriptive information was obtained about homeless families in the Intermountain West and their children's health care needs were defined to help professionals develop programs tailored to meet the unique needs of this population. We collected data during the well-child visits of 306 children in 161 families living at the Travelers Aid Society family shelter. The results reveal 2 types of homeless families, episodic and long term. Each requires different levels of assistance. This study's profile of mobile, predominantly white, 2-parent families with few children (mean of 1.2) differs conside

Infant morbidity in Harlem: a status report.

To the longstanding cycle of poverty, disease, and despair that has afflicted several generations of Harlem residents have now been added epidemics of crack abuse, AIDS, syphilis, and pervasive violence. The community's children have been especially vulnerable to these calamities. Harlem's traditional institutions of strength, aid, and comfort--its extended families, churches, the hospital, and the government--have become nearly overwhelmed by mounting social disorganization and an apparent lack of resources. It may be possible to combat such problems by re-examining some of the assumptions we

Excess Hispanic fetal-infant mortality in a midwestern community.

OBJECTIVE: We assessed excess fetal-infant mortality for Hispanic, non-Hispanic white, and non-Hispanic black populations in five contiguous counties of Missouri and Kansas. METHODS: We conducted a perinatal periods of risk (PPOR) assessment of fetal-infant mortality using electronic linked birth-death record files from 2001 through 2005. We generated an internal reference group in accordance with established PPOR protocol. We used Kitagawa analysis to determine whether excess deaths were due to birthweight distribution (a higher frequency of prematurity or growth retardation) or to higher mo
