AIM: The alpha interferon treatment criteria have not been established in children with chronic hepatitis B. We report the results of a prospective study.
METHODS: Between 1988-1992 14 children (2 girls and 12 boys) with chronic hepatitis B received 3 million U/m2 of interferon alpha three times a week for 6 months. All patients underwent a liver biopsy that showed a pattern of chronic active hepatitis. One patient had cirrhosis. We report a case of thyrotoxicosis in a male patient under alpha-interferon (INF-alpha) therapy. Data from the literature show that treatment with IFN-a alone or combined with interleukin-2 (IL-2) can induce dysthyroidism (hypothyroidism, hyperthyroidism, thyroiditis). The frequency of dysthyroidism under INF-alpha therapy ranges from 10 to 40% depending on the authors. During combined INF-alpha and IL-2 therapy these figures reach 20 to 91%. The efficacy of interferon alpha-2b in doses up to 12 x 10(6) IU three times weekly was studied in 21 patients with a metastatic carcinoid tumour. Of these 21 patients, 19 were evaluable for response. Patients were treated with escalating dosages of interferon alpha-2b: 3 x 10(6) IU, 6 x 10(6) IU and 12 x 10(6) IU. Both recombinant interferon alpha and deoxycoformycin (dCF) are effective in the treatment of hairy cell leukaemia. Twelve patients with documented, progressive hairy-cell leukaemia were treated with human lymphoblastoid interferon-alpha. All patients initially received a three-month course of interferon-alpha (3 x 10(6) U each day) which resulted in two complete remissions and 10 partial remissions. Clinical case A patient with chronic, painless, bilateral loss of vision, after significant intake of interferon (IFNα) and ribavirina due to liver transplant. Ocular fundus is normal. A suspected retrobulbar optic neuropathy is confirmed by a prolongation of the latency of the patient's visual evoked potential.
