Inflammation and Fibrosis in Perirenal Adipose Tissue of Patients With Aldosterone-Producing Adenoma.

The prevalence of primary aldosteronism is much higher than previously thought. Recent studies have shown that primary aldosteronism is related to a higher risk of cardiovascular events. However, the underlying mechanism is not yet clear. Here we investigate the characteristics, including inflammation, fibrosis, and adipokine expression, of adipose tissues from different deposits in patients with aldosterone-producing adenoma (APA). Inflammation and fibrosis changes were evaluated in perirenal and subcutaneous adipose tissues obtained from patients with APA (n = 16), normotension (NT; n = 10),

Comparison of dorsocervical with abdominal subcutaneous adipose tissue in patients with and without antiretroviral therapy-associated lipodystrophy.

OBJECTIVE: Combination antiretroviral therapy (cART) is associated with lipodystrophy, i.e., loss of subcutaneous adipose tissue in the abdomen, limbs, and face and its accumulation intra-abdominally. No fat is lost dorsocervically and it can even accumulate in this region (buffalo hump). It is unknown how preserved dorsocervical fat differs from abdominal subcutaneous fat in HIV-1-infected cART-treated patients with (cART+LD+) and without (cART+LD-) lipodystrophy. RESEARCH DESIGN AND METHODS: We used histology, microarray, PCR, and magnetic resonance imaging to compare dorsocervical and abdo

Polycystic ovary syndrome as a plausible evolutionary outcome of metabolic adaptation.

As a common endocrinopathy of reproductive-aged women, polycystic ovary syndrome (PCOS) is characterized by hyperandrogenism, oligo-anovulat … s in combination with reduced insulin sensitivity and preferential accumulation of highly-lipolytic intra-abdominal fat in vivo. This PCOS phenotype may be an evolutionary metabolic adaptation to balance energy storage with glucose availability and fatty acid oxidation for optimal energy use during reproduction. This review integrates fundamental endocrine-metabolic …

Endocrine-metabolic pattern and adipose tissue distribution.

The associations between cardiovascular disease (CVD), non-insulin-dependent diabetes mellitus (NIDDM) and abdominal fat distribution are well established. The most important adipose tissue depot in this context is probably the mass of intra-abdominal adipose tissue which has been found to be associated with CVD, NIDDM and their established metabolic risk factors. This type of adipose tissue distribution is also associated with multiple endocrine aberrations, probably comprising an increased responsiveness of the hypothalamo-adrenal axis and a parallel or secondary decreased activity of the hy

Chronic stress, combined with a high-fat/high-sugar diet, shifts sympathetic signaling toward neuropeptide Y and leads to obesity and the metabolic syndrome.

In response to stress, some people lose while others gain weight. This is believed to be due to either increased beta-adrenergic activation, the body's main fat-burning mechanism, or increased intake of sugar- and fat-rich "comfort foods." A high-fat, high-sugar (HFS) diet alone, however, cannot account for the epidemic of obesity, and chronic stress alone tends to lower adiposity in mice. Here we discuss how chronic stress, when combined with an HFS diet, leads to abdominal obesity by releasing a sympathetic neurotransmitter, neuropeptide Y (NPY), directly into the adipose tissue. In vitro, w

Superior Mesenteric Artery (SMA) Syndrome With Enterocutaneous Fistula in a Young Woman: A Rare Association.

Superior mesenteric artery (SMA) syndrome is also known as Wilkie's syndrome. Sometimes it is the cause of obstruction in the duodenum. In SMA syndrome, the acute angulation of the SMA against the abdominal aorta can prevent duodenal contents from draining into the jejunum (upper small intestine); hence inadequate intake of nutrition leads to weight loss and malnutrition. This is primarily attributed to the loss of intervening pad of mesenteric fat tissue due to various debilitating conditions. Abnormal connections between the intra-abdominal gastrointestinal tracts and skin over the abdomen a

Characterising metabolically healthy obesity in weight-discordant monozygotic twins.

AIMS/HYPOTHESIS: Not all obese individuals display the metabolic disturbances commonly associated with excess fat accumulation. Mechanisms maintaining this 'metabolically healthy obesity' (MHO) are as yet unknown. We aimed to study different fat depots and transcriptional pathways in subcutaneous adipose tissue (SAT) as related to the MHO phenomenon. METHODS: Sixteen rare young adult obesity-discordant monozygotic (MZ) twin pairs (intra-pair difference (∆) in BMI ≥ 3 kg/m(2)), aged 22.8-35.8 years, were examined for detailed characteristics of metabolic health (subcutaneous, intra-abdominal a

Chylous ascites: Case report of a rare presentation of blunt abdominal trauma.

INTRODUCTION: Chylous ascites is the accumulation of a milk-like peritoneal fluid rich in triglycerides, due to the presence of intestinal lymph in the abdominal cavity. The most common causes of chylous acites in adults are abdominal malignancy and cirrhosis. Very few cases of chylous ascites associated to blunt abdominal trauma have been published in the literature. CLINICAL CASE: A 27-year-old, female patient was admitted to the emergency department (ED) with abdominal pain due to a deceleration-type traffic accident. During surveillance the patient presented a progressive decrease in hemo

Acquired abdominal intercostal hernia: a case report and literature review.

Acquired abdominal intercostal hernia (AAIH) is a rare disease phenomenon where intra-abdominal contents reach the intercostal space directly from the peritoneal cavity through an acquired defect in the abdominal wall musculature and fascia. We discuss a case of a 51-year-old obese female who arrived to the emergency room with a painful swelling between her left 10th rib and 11th rib. She gave a history of a stab wound to the area 15 years earlier. A CT scan revealed a fat containing intercostal hernia with no diaphragmatic defect. An open operative approach with a hernia patch was used to rep

Ectopic fat distribution and its correlation with insulin resistance indices in non-obese individuals with type 2 diabetes.

BACKGROUND: This study examined the links between regional adiposity and insulin resistance in non-obese adults with type 2 diabetes. METHODS: This post-hoc analysis utilized data from a randomized controlled trial involving participants with type 2 diabetes who were recruited from a hospital in East China between December 2018 and January 2019. Pancreatic fat content (PFC), liver fat content (LFC), visceral adipose tissue (VAT), and subcutaneous adipose tissue (SAT) were quantified using the IDEAL-IQ water–fat separation technique. Insulin resistance was assessed using homeostasis model asses

A two-step scoring model incorporating visceral-to-subcutaneous fat ratio and systemic immunoinflammatory index for predicting cytokine release syndrome severity in patients with gastric cancer receiving Claudin18.2-targeted CAR-T cell therapy.

Cytokine release syndrome (CRS) greatly impacts survival in patients who undergo chimeric antigen receptor (CAR)-T cell therapy, and the identification of its determinants is still challenging. We analysed the impact of systemic immunoinflammatory index (SII) and body composition parameters derived from CT images on the severity of CRS in 45 patients with advanced gastric cancer treated with CLDN18.2-targeted CAR-T cells. The waist circumference, skeletal muscle index (SMI), skeletal muscle density (SMD), subcutaneous fat area (SFA), visceral fat area (VFA) and VFA-to-SFA ratio (VSR) on baseli

Sex- and depot-dependent differences in adipocyte morphology and gene expression in diet-induced obesity.

OBJECTIVE: Biological sex differences have been observed in the development of obesity-related metabolic comorbidities. The aim was to determine the sex-specific response to high-fat diet (HFD) consumption on metabolic profile and adipose tissue dysfunction in male and female Wistar rats. MATERIAL AND METHODS: 24 Wistar male and female rats were fed a normal chow diet or a HFD for 4 months. Body weight and food intake were periodically registered, and markers of the metabolic profile as well as the expression of inflammatory factors in visceral and subcutaneous adipose depots were measured.

A comparative study of software programmes for cross-sectional skeletal muscle and adipose tissue measurements on abdominal computed tomography scans of rectal cancer patients.

BACKGROUND: The association between body composition (e.g. sarcopenia or visceral obesity) and treatment outcomes, such as survival, using single-slice computed tomography (CT)-based measurements has recently been studied in various patient groups. These studies have been conducted with different software programmes, each with their specific characteristics, of which the inter-observer, intra-observer, and inter-software correlation are unknown. Therefore, a comparative study was performed. METHODS: Fifty abdominal CT scans were randomly selected from 50 different patients and independently a

Association between lipid accumulation product and visceral adiposity index and rheumatoid arthritis: NHANES 1999-2018.

BACKGROUND: While the Lipid Accumulation Product (LAP) and Visceral Adiposity Index (VAI) offer improved assessment of visceral fat distribution compared to traditional measures, their connection with rheumatoid arthritis (RA) has not been thoroughly explored. This research aimed to assess the links between LAP, VAI, and RA occurrence using nationally representative datasets. METHODS: This cross-sectional study analyzed information from the National Health and Nutrition Examination Survey (NHANES) covering the years 1999-2018. RA status was determined based on self-reported physician diagnosi

Triglyceride-glucose index and its combination with anthropometric obesity indicators are significant predictors of visceral obesity among the Newfoundland population.

Visceral obesity (VO) is a critical risk factor for metabolic disorders and cardiovascular diseases. Although DXA, CT, or MRI are reliable tools for measuring visceral adiposity, their routine use is limited by cost and accessibility. The triglyceride-glucose (TyG) index, a surrogate marker of insulin resistance, has shown potential as a cost-effective predictor of VO in type 2 diabetes mellitus (T2DM). However, its relationship with visceral adiposity in generally healthy populations remains unclear. A total of 2921 healthy adults (825 males, 2096 females) without serious metabolic, cardiovas

The association between visceral adiposity index and female stress urinary incontinence: the NHANES 2013 to 2018.

Obesity is an independent risk factor for stress urinary incontinence (SUI) in women. The visceral adiposity index (VAI) has recently been used as a reliable index to assess visceral fat accumulation and dysfunction. The objective of this research was to examine the correlation between VAI and SUI among adult females. Our study is a cross-sectional study derived from the National Health and Nutrition Examination Survey. A total of 5938 females were enrolled, and all participants had complete VAI and SUI data. We analyzed weighted multivariable logistic regression to assess the independent asso

External validation of a deep learning model for automatic segmentation of skeletal muscle and adipose tissue on abdominal CT images.

OBJECTIVES: Body composition assessment using CT images at the L3-level is increasingly applied in cancer research and has been shown to be strongly associated with long-term survival. Robust high-throughput automated segmentation is key to assess large patient cohorts and to support implementation of body composition analysis into routine clinical practice. We trained and externally validated a deep learning neural network (DLNN) to automatically segment L3-CT images. METHODS: Expert-drawn segmentations of visceral and subcutaneous adipose tissue (VAT/SAT) and skeletal muscle (SM) of L3-CT-i

Evaluation of the cushion effect in blunt abdominal trauma patients: A computerized analysis.

BACKGROUND: Obesity may serve as a protective factor in blunt abdominal trauma (BAT) patients due to a "cushion effect". In this study, we aim to use computed tomography (CT) scans to measure abdominal adiposity and its correlation with injury severity in BAT patients. METHODS: We conducted a retrospective analysis of male BAT patients who had undergone CT scans. Subcutaneous adipose tissue (SAT) and visceral adipose tissue (VAT) were manually measured and height-normalized for analysis at lumbar levels L2 and L3. Statistical methods were used to compare differences in adiposity between patie
