Rolling Egg-Shaped Peritoneal Loose Body (PLB): A Diagnostic Dilemma for Surgeons and Radiologists.

A man in his mid-50s presented with heaviness in the right lower abdomen for the last three months. Ultrasonography (USG) showed an intra-abdominal mass in the right iliac fossa. Contrast-enhanced CT (CECT) revealed a well-defined circumscribed mass near the ileocaecal junction, with a hypodense center, surrounded by a hyperdense periphery and well-defined capsule. A lower midline laparotomy was performed. Intraoperatively, a white, firm, smooth ball-like mass was found, lying freely in the abdomen. Histopathology revealed fatty tissues surrounded by a calcified shell, which was further surrou

Finite Element Analysis Could Predict and Prevent a Pathological Femoral Shaft Fracture after En Bloc Resection of a Large Osteoid Osteoma.

Osteoid osteoma is a benign tumor. Approximately 20% of these tumors are located in the femur. The tumor primarily occurs in children and is treated by surgical excision or radiofrequency ablation. Recently, bone-tumor resection using three-dimensional (3D) intraoperative imaging with an O-arm in combination with a navigation system has been reported to be effective. However, there is a risk of postoperative fracture because of the weakening of the bone after drilling for tumor resection. A 12-year-old Japanese girl presented with an osteoid osteoma in the left femoral shaft, which resulted in

Indigenous Proximal Femoral Nails and Their Novel Complications.

Pertrochanteric fractures are one of the commonest fractures in the geriatric age group. Management of these fractures has been revolutionized by the use of proximal femoral nails (PFN), with a screw or a helical blade for fixation in the femoral head. Multiple complications like Z-effect, screw cut out, head penetration, varus collapse, and so on are known with poor technique in proximal femoral nails antirotation (PFNA). We present a case where an indigenous implant presented to us a novel problem of helical blade breakage inside the bone in situ. Removal of this blade needed an open approac

Multidetector computed tomography in preoperative planning for temporomandibular joint ankylosis: A pictorial review and proposed structured reporting format.

Ankylosis of the temporomandibular joint (TMJ) is a disabling disease resulting from fibrous or bony fusion of the mandibular condyle and the glenoid fossa. Early diagnosis and surgical treatment are essential to prevent facial deformity and other complications. Conventional radiography has limitations in demonstrating the true extent of ankylosis. It is important for surgeons to be aware of the size and degree of bony ankylosis in order to perform complete resection of the ankylotic mass. In addition, a detailed evaluation of the relationship with adjacent vital structures such as the interna

A rare case of mesenteric heterotopic pancreas presenting as an inflammatory mass.

Heterotopic pancreas (HP) is a rare condition, is commonly asymptomatic and found incidentally at autopsy or during abdominal surgery. HP is usually found in the upper gastrointestinal tract, particularly in the stomach, duodenum and jejunum. Mesenteric heterotopic pancreas (MHP) has been rarely described in the literature, and there has been no reported case under ten years of age. We report a unique case of a 7-year-old African boy who presented with clinical appendicitis and intraoperatively was found to have a necrotic nodular mass in the jejunal mesentery which required segmental small bo

Unilateral absence of pulmonary artery associated with contralateral lung cancer.

Unilateral absence of a pulmonary artery (UAPA) is a rare congenital cardiac malformation that is often associated with other cardiovascular deformities. Surgical repair of this rare condition is usually performed only on the abnormal lung. The occurrence of lung cancer in association with UAPA is even rarer and clinical experience is very limited. This report aims to describe a case of unilateral absence of right pulmonary artery that was complicated by primary carcinoma of the contralateral lung. A left lower lobectomy was performed despite the absence of the right pulmonary artery and repea

Controversies and advances in the management of congenital ptosis.

The management issues associated with pediatric ptosis begin with determining the etiology of the ptosis, and considering how the eyelid position affects the child's visual and psychosocial development. These ultimately determine if and when surgical management should be undertaken. Surgical challenges include the lack of intraoperative feedback regarding the dynamic eyelid height and contour under general anesthesia. When the eyelid elevators do not function or if there is little drive to lift the involved eyelid, obtaining good surgical outcomes can be extremely challenging. A plethora of su

Intraductal papillary mucinous neoplasm of the pancreas with a bifid pancreatic duct.

A bifid pancreatic duct presenting a major bifurcation in the main pancreatic duct is one of the anatomical variations of the pancreatic ducts. We encountered a 71-year-old female with a 5-cm-diameter branch duct intraductal papillary mucinous neoplasm of the pancreas in whom preoperative endoscopic retrograde pancreatography demonstrated an anomalous bifurcation of the main pancreatic duct at the body of the pancreas. We performed a distal pancreatectomy, instead of a middle pancreatectomy, with a cutting line at the downstream pancreas to the duct bifurcation point. Intraoperative ultrasonog

A too-long anterior process of the calcaneus: a report of 31 operated cases.

A too-long anterior process of the calcaneus is one of the causes of repeated sprained ankles or painful feet in the child or adolescent that is often ignored and misdiagnosed. Among 31 cases of adolescents having consulted for these symptoms, a too-long anterior process of the calcaneus was discovered on radiographs, and explored in detail with a computed tomography scan or an MRI. The decision for surgical resection was taken considering the discomfort expressed by these children. The intraoperative observation corroborated the radiological findings and permitted a better understanding of th

Case Report of Post-Traumatic Monoarticular Tuberculosis of the Knee in a Healthy Young Adult: Diagnostic and Therapeutic Challenges in a Non-Endemic Setting.

INTRODUCTION: Tuberculosis (TB) arthritis, particularly in the knee, is a rare condition with significant diagnostic challenges, especially in regions with low TB incidence. This case report presents the first documented instance of native knee TB arthritis in Ireland, highlighting the necessity for increased awareness and a multidisciplinary approach in diagnosing this uncommon disease. CASE REPORT: A 27-year-old healthy male, originally from India, and living in Ireland, presented with non-specific left knee pain for 3 weeks, initially diagnosed as a soft-tissue injury. After a subsequent t

Intragastric Nonabsorbable Sutures From Prior Endoscopic Sleeve Gastroplasty Discovered During Robotic Hiatal Hernia Repair: A Case Report.

Endoscopic sleeve gastroplasty (ESG) has emerged as a minimally invasive bariatric procedure that utilizes endoscopic suturing to reduce gastric volume. While generally considered safe, delayed complications related to retained suture material are incompletely characterized in the literature. We report a case of intragastric nonabsorbable sutures discovered during revisional hiatal hernia surgery in a patient with a remote history of prior foregut intervention. A 58-year-old woman with a history of hiatal hernia repair with fundoplication approximately 10 years prior presented for surgical eva

Hemodynamic recovery after restoration of sinus rhythm after complete atrioventricular block during off-pump coronary artery bypass surgery: a case report.

BACKGROUND: Off-pump coronary artery bypass (OPCAB) surgery is frequently chosen for high-risk patients to mitigate cardiopulmonary bypass-related complications. However, OPCAB is associated with significant intraoperative hemodynamic challenges, particularly in patients with impaired ventricular function and pre-existing conduction abnormalities. High-grade atrioventricular (AV) conduction disturbances during off-pump coronary artery bypass surgery are rare but can result in life-threatening hemodynamic collapse. CASE PRESENTATION: We report the case of a 75-year-old man with severe ischemic

Diagnostic Challenge of Sequential Appendicitis in a Patient With Appendiceal Duplication: A Case Report.

BACKGROUND Appendiceal duplication is an exceptionally rare congenital anomaly, with a reported prevalence of less than 0.003%, and remains poorly represented in the contemporary evidence-based surgical literature. Recent Cochrane reviews and PubMed-indexed meta-analyses (2024 onward) have refined diagnostic pathways and management strategies for acute appendicitis; however, congenital anatomical variants such as appendiceal duplication are largely excluded from these analyses. Among the described subtypes, type B2 duplication according to the Cave-Wallbridge classification is clinically signi

Persistent Left Superior Vena Cava: Incidence and Management in Patients Undergoing Repair of Partial Anomalous Pulmonary Venous Connection.

INTRODUCTION: A association between partial anomalous pulmonary venous connection (PAPVC) and systemic venous anomalies has been described in the literature, but the true incidence is yet to be elucidated. At our institution, we sought to find the incidence of a persistent left superior vena cava (PLSVC) in patients undergoing an operation for PAPVC. METHODS: A retrospective review of all pediatric and adult patients with PAPVC who underwent surgical repair from February 2006 to February 2016. All clinical, radiographic, and operative data were reviewed. RESULTS: Eighty-five patients underwe

The small abnormal parathyroid gland is increasingly common and heralds operative complexity.

BACKGROUND: Over decades, improvements in presymptomatic screening and awareness of surgical benefits have changed the presentation and management of primary hyperparathyroidism (PHPT). Unrecognized multiglandular disease (MGD) remains a major cause of operative failure. We hypothesized that during parathyroid surgery the initial finding of a mildly enlarged gland is now frequent and predicts both MGD and failure. METHODS: A prospective database was queried to examine the outcomes of initial exploration for sporadic PHPT using intraoperative PTH monitoring (IOPTH) over 15 years. All patients

Building a pediatric spasticity surgery program in a low-awareness setting: capacity creation, multidisciplinary integration, and early outcomes.

PURPOSE: Selective dorsal rhizotomy (SDR) and other spasticity-relieving procedures remain markedly underutilized in many regions of the world, not because of technical infeasibility but because of low awareness among referring specialties, the absence of structured referral pathways, and limited multidisciplinary integration. We describe the stepwise establishment of a pediatric spasticity surgery program in such a setting, with emphasis on capacity creation, stakeholder engagement, individualized treatment selection, and early clinical outcomes. METHODS: This program was developed at a tert

Isolated lesions of the lower subscapularis tendon: diagnosis and management.

PURPOSE: Isolated lesions of the caudal portion of the subscapularis (SSC) tendon are rare and have not been subject of major attention in the literature yet. However, these lesions can lead to tremendous impairment of shoulder function. The purpose of the present study was to raise clinician's awareness for these "hidden lesions" and to present the crucial steps for diagnosis and management. METHODS: Five patients (three men, two women; mean age 40) with traumatic isolated caudal SSC lesion were enrolled in the present study. After clinical and radiological examination, all patients underwen
