Patients who cannot eat enough require alternatives to oral feeding. Tube feeding is one such method for patients with a functioning gut. The techniques for the placement of feeding tubes and diets for tube feeding have improved during the past 20 years. Comfortable thin-bore polyurethane tubes are replacing the thicker vinyl tubes. The performance of three 10F enteroclysis tubes with different tip designs (E. Merck Ltd) was assessed using double-contrast, gravity infusion enteroclysis, with methylcellulose solution as negative contrast. The authors describe a method of complex examination of patients with diseases of the external biliary tract with the aim of diagnosing cystic duct blocks. Intravenous cholangiography using billignost and albumin in 2:1 ratio proved highly effective. Of essential help in the diagnosis of cystic duct block was dynamic ultrasonic examination. Cystic duct block is a distinct sign of cholecystitis requiring operative intervention.

A four-lumen, 20-F surgical gastrojejunostomy tube designed to incorporate separate lumens for both jejunal feeding and gastric decompression was modified for guide-wire use. This modification permits radiologic rather than intraoperative tube placement. The tube was successfully placed in six patients. Cecal volvulus is an uncommon cause of intestinal obstruction. Operative management is the accepted form of treatment. We report a patient with cecal volvulus who was a poor surgical risk and was successfully treated by decompression and detorsion with the use of a decompression tube inserted through the colonoscope.

A Sengstaken-Blakemore tube was unable to be withdrawn 14 h after its initial insertion. Despite a number of recognized maneuvers, both gastric and esophageal balloons remained inflated leading to impaction of the tube. This uncommon complication along with other complications peculiar to the Sengstaken-Blakemore tube are described.

We have described a case of intracranial placement of a nasogastric tube in a patient with severe head trauma. This complication occurs as a result of either passage through a traumatic defect in the cribriform plate associated with a severe head injury or direct penetration through an intact cribriform plate by an improperly passed rigid nasogastric tube. In combative semicomatose or comatose patients, oral placement may be preferable to nasogastric placement of a gastric tube.

The continuous inside splinting of the small intestine was carried out in 82 children and adults, because of postoperative ileus, chronic recurrent ileus and diffuse peritonitis. Possibly the splinting had contributed in one case to the perforation of the paralytic small intestine with a fatal outcome. In another case there occurred a relapse of an ileus. All other patients have no complaints in connexion with the splinting operation. Control examinations take place. A "total folding" of the small intestine was not observed by the radioscopy.
