Eighty-one knees surgically treated and twenty-six conservatively treated for recurrent dislocation of the patella in seventy-eight patients were studied. The average follow-up on the surgically treated knees was eight years and on the conservatively treated knees, sixteen years. Osteoarthropathy in congenital sensory neuropathy with anhidrosis (CSNA) is not only a rare complication but also difficult to treat. Two patients with CSNA, a three-year-old boy and a 24-year-old woman, were treated as follows. The boy was affected in his left knee and left heel and required reduction of recurrent bilateral hip dislocations. The woman was treated by amputation of her left leg above the knee because of ischemic necrosis, secondary to knee dislocations.

Isolated lateral retinacular release was performed on 102 knees in 76 patients. Diagnoses included chondromalacia patellae, patellar compression syndrome, and subluxation and dislocation of the patella. We reviewed 41 knees after arthroscopic lateral release for recurrent dislocation of the patella at a mean follow-up of four years, and graded the results according to the criteria of Crosby and Insall (1976). BACKGROUND: Patellar dislocations are common, but superior-lateral dislocation with vertical axis rotation is rare and challenging to reduce.
CASE PRESENTATION: A woman in her 20s presented with left knee pain and deformity after minor trauma. Traditional reduction attempts failed due to a vertical rotational component. Reduction was achieved using a technique involving hip flexion, a modified varus-valgus force, and knee extension.
CONCLUSION: Emergency clinicians must recognize complex patellar dislocations and consider early orthopedic consultation to minimize complications.
