[Electronystagmography aspects of peripheral and central vestibular syndromes].

This report deals with what is to be known in applying ENG in the everyday office. From vestibular, visual and proprioceptive informations central neural processing evolve to stabilize images on the retina. ENG can perform an analysis of these systems, alone or interacting. Anatomical, neurophysiological backgrounds and experimental pathophysiology results are first described. The methodic rule of anamnesis is emphasized and the non instrumental examination is outlined. Concerning the technical aspects and the ENG achievement, some nonlinearities are stressed. Next, procedure of eye movements

Vestibulo-ocular disturbances in rats exposed to organic solvents.

The influence of different kinds of industrial solvents on the vestibular function of rats has been studied by recording nystagmus, induced by accelerated rotation. The effect was related to the blood levels of the solvents. One group of solvents, including halogenated saturated hydrocarbons like dichloromethane, caused depression of the vestibulo-oculomotor reflex (VOR). Another group, including benzene compounds like xylene, toluene, styrene and cumene and halogenated unsaturated hydrocarbons like trichloroethylene caused an excitation of the VOR. The most striking chemical similarity betwee

Clinical signs of visual-vestibular interaction.

Visual suppression of vestibulo-ocular reflexes (induced vestibular nystagmus) differs between normal subjects and patients with various neurological disorders. Abnormalities of VOR suppression were associated consistently with abnormalities of the visual and/or vesitibular oculomotor reflexes and in particular with abnormal smooth pursuit eye movements in the direction of abnormal suppression. Absent VOR suppression in a gaze position of, and in the same direction as, a spontaneous nystagmus was found exclusively in patients with spontaneous nystagmus of central nervous system origin. Floating labyrinth: A unique finding on CT scan.

We report a case of a 67-year-old female patient that presented to the emergency department with complete right facial paralysis, progressive hearing loss and chronic otorrhea. A unique finding on CT scan is reported: a "floating labyrinth" - cochlea, vestibule and semicircular canals extensively surrounded by soft tissue density material but with intact thinned otic capsule walls. A transotic approach was performed for removal of noncholesteatomatous inflammatory tissue; intravenous antibiotics and corticosteroids led to partial recovery of facial nerve function. A chronic suppurative otitis

Endolymphatic sac tumor located around semicircular canals.

We report a case of endolymphatic sac tumor (ELST). A 48-year-old female had recurrent and slowly progressive hearing loss, accompanied with dizziness like Meniere's disease. A tumor was located around the semicircular canals, and was detected on CT and MRI. The patient underwent total removal of the tumor using a transmastoid approach. Histopathological examinations agreed with features of an ELST. The tumor was highly suspected to have originated from the rugose portion of the endolymphatic sac or the endolymphatic duct, based on surgical and imaging studies. Structure of the membranous laby

Endolymphatic sac revision for recurrent intractable Meniere's disease.

Meniere's disease can be observed uniquely in revisions of the conservative surgical procedure for endolymphatic sac enhancement, which preserves the labyrinth, unlike destructive procedures. Genetic anatomic abnormalities in patients lead to malabsorption of endolymph and symptoms of Meniere's that are reversed by endolymphatic sac enhancement, but disabling symptoms eventually may recur in a few cases and require revisional surgery. We observe developing pathophysiologic conditions intraoperatively and have modified our techniques to accommodate the redeveloped pathogenesis we observe and av

Meniére's disease. Electron microscopy of the vestibular ganglion and end organs after ultrasound.

Material for electron microscopy study was obtained from a patient previously suffering from Meniere's disease and treated by ultrasound. It consisted of parts of the membranous labyrinth as well as the vestibular ganglion. The neuro-epithelium of the macula showed degenerative changes and there was marked vacuolation of the cytoplasm. The endothelial cells lining the semicircular canal showed gross vacuoles contained degenerating mitochondria and some debris which may reach the endolymph. The wall of the semi circular canal was oedematous and the basement membrane lifted off by oedematous flu

Visually induced dizziness.

PURPOSE OF REVIEW: Visually induced dizziness (VID) is a common phenomenon in vestibular disorders of both peripheral and central causes. This article provides a review of the most updated understandings of definition, pathophysiology, and treatment options. RECENT FINDINGS: The pathophysiology is complex and its severity or persistence may be related both to the underlying cause and heritable factors. Environmental and psychological factors may influence the degree of impact of VID on daily life function. Treatment is mostly empiric at this point but includes pharmacologic, desensitization,

Evolution of caloric responses during and between the attacks of Meniere's disease.

Caloric tests are useful for disease surveillance of Meniere's disease (MD), although the interpretation of caloric tests remains controversial during the attacks of MD. Therefore, we aimed to characterize the findings of caloric tests during the attacks of MD. We analyzed the results of bithermal caloric tests during and between the attacks in 43 patients with unilateral definite MD. Evaluation was performed during the irritative/recovery phase in 22 and during the paretic phase in 26 patients. During the irritative/recovery phase, less than half of patients (10/22, 45%) showed a caloric asym

[Case report: ipsilateral association of posterior and anterior canalolithiasis].

Benign paroxysmal positional vertigo (BPPV) is the most frequently occurring dizzy syndrome. The literature provides ample descriptions of the forms determined by canalo/cupulolithiasis of the posterior canal, the most common, as well as canalo/cupulolithiasis of the horizontal canal. Descriptions of the forms of canalo/cupulolithiasis of the anterior canal are, instead, very rare and, sometimes questioned: this is fundamentally due to its superior position, which makes it a much less likely site for the accumulation of debris, both from a conceptual and practical point of view. For these reas

Tinnitus. More can be done than most GPs think.

BACKGROUND: Tinnitus is the appreciation of sound arising from an internal source. The sound may be pulsating and arising from a vascular cord near the ear or it may arise from pathology in the inner ear. When pathology in the inner ear is the cause of the tinnitus, the tinnitus is nonpulsating, continuous and may have variable frequencies and intensity. OBJECTIVE: This article aims to outline the diagnostic features of tinnitus to determine causes and aggravators, and provide an overview of treatment. DISCUSSION: Assessment of tinnitus involves distinguishing objective from nonobjective tin

[Results of surgery of the endolymphatic sac].

Surgery of the endolymphatic sac is controversial. Some consider it a placebo and others consider it the surgical treatment of choice in Ménière's disease. We studied the medical records of 87 patients who underwent surgery between 1978 and 1996. Simple decompression was practiced in 89% and a House shunt in 27.6%. The 1-year results were improvement or recovery from vertigo in 65.4%, no improvement in 25%, and reoperation in 9.5%. Tinnitus improved in only 11.9% and hearing loss improved in 9.5% and worsened in 19%. We reviewed the cases of 50 patients with a 5-year follow-up(60%). Vertigo im

Experimental labyrinthine disorders and direction of nystagmus.

The purpose of this paper is to demonstrate the variation of the direction of nystagmus which is elicited from the peripheral labyrinth. The direction of nystagmus depends, among other things, on the nature of the stimulus. Depending on the degree of pathophysiological conditions in the labyrinth, atypical nystagmus which does not comply with the rule is occasionally elicited even though the same stimulus is given. Hyperfunctional stimulation to the labyrinth, such as allergy, causes nystagmus which beats to the affected side and also shows hyperactivity of the whole nerve action potential and
