Pediatric sialendoscopy in Asians: A preliminary report.

BACKGROUND/PURPOSE: The sialendoscopic approach in treating pediatric salivary gland disorders has been reported with great success through the years. Whereas this success has been widely reported in Caucasian populations, relatively little has been reported regarding the use of this procedure in pediatric patients in Asian countries. The purpose of this study is to report our preliminary experience in pediatric sialendoscopy. METHODS: The data from 20 patients (<18years old), who underwent sialendoscopy for obstructive sialoadenitis in the Department of Otorhinolaryngology of Mackay Memorial

Transurethral cystolithotripsy with holmium laser under local anesthesia in selected patients.

OBJECTIVES: To evaluate the feasibility and effectiveness of transurethral holmium:yttrium-aluminum-garnet (YAG) laser cystolithotripsy under local anesthesia in selected patients. METHODS: Thirteen consecutive male patients with large bladder calculi (3 cm or greater) caused by benign prostatic hyperplasia underwent transurethral cystolithotripsy using holmium:YAG laser under local anesthesia. The operation was performed with all the patients in the lithotomy position (except 3 with pelvic prosthesis). All patients underwent transurethral holmium laser cystolithotripsy (HLC) with a flexible

Simultaneous holmium laser enucleation of prostate and upper-tract endourologic stone procedures.

BACKGROUND AND PURPOSE: The holmium laser is a versatile urologic tool. Its unique cutting and coagulating properties allow multiple procedures, such as stone fragmentation and laser enucleation of the prostate (HoLEP), to be performed with a single set-up. This paper reviews our experience with simultaneous HoLEP and endourologic upper-tract stone procedures. PATIENTS AND METHODS: We retrospectively reviewed all 11 patients (12 renal units) treated with the combined approach of HoLEP and an upper-tract endourologic procedure for stone disease. All patients initially underwent HoLEP, followed

Ureteral calculi secondary to a gradually migrated acupuncture needle.

We herein presented a case of calculi secondary to a migrated acupuncture needle. A 74-year-old woman with a history of acupuncture therapy for lumbago was referred to our hospital for treatment of ureteral and renal pelvic calculi. Abdominal multi-detector computed tomography scans showed ipsilateral hydronephrosis and two calculi secondary to a migrated acupuncture needle. First, a percutaneous nephrolithotomy was performed to extract two calculi and fine needle fragments from the pelvis. Subsequently, residual needle fragments and calculi in the ureter were then removed by flexible transure

Migration of a metal clip into the urinary bladder.

Migration of metal clips into the urinary tract is rare. We present a case in which migration of a metal clip into the urinary bladder occurred after retropubic radical prostatectomy. A 75-year-old man, who had undergone retropubic radical prostatectomy three years before, presented with painful micturition and gross hematuria. Radiography and cystoscopy showed two vesical stones. As treatment for these stones, transurethral holmium laser lithotripsy was performed. One of the stones had formed around a metal clip that had presumably migrated into the urinary bladder. After removal of both ston

Equal-power Ho:YAG laser energy-frequency modulation during RIRS for renal pelvis stones: 0.3 J × 50 Hz vs. 1.5 J × 10 Hz.

To compare outcomes of two holmium: yttrium–aluminum–garnet (Ho:YAG) laser energy–frequency protocols delivered under equal average power during retrograde intrarenal surgery (RIRS) for renal pelvis stones. We retrospectively reviewed 250 adults undergoing unilateral RIRS for a single renal pelvis stone (10–20 mm) between January 2022 and November 2025. Patients were treated with Ho:YAG lithotripsy at the same average power (~ 15 W) using either 0.3 J × 50 Hz (n = 120) or 1.5 J × 10 Hz (n = 130). All cases were performed with a digital flexible ureterorenoscope and a suction-enabled ureteral a

Intraoperative Breakage and Endoscopic Retrieval of a Dismantled Triprong Forceps During Ureterorenoscopic Lithotripsy: A Rare Complication and Review of the Literature.

Ureterorenoscopic lithotripsy (URSL) has become the gold-standard procedure for management of ureteric calculi. Ancillary stone-retrieval instruments such as triprong and biprong forceps facilitate fragment extraction; however, mechanical failure of these devices during active use is a recognized, albeit rarely documented, intraoperative hazard. We describe a 68-year-old woman who underwent left-sided URSL for a 1 cm upper ureteric calculus at the L4 vertebral level. During stone-fragment retrieval, the distal prongs of a triprong forceps sheared off within the ureter. The retained fragment wa

[TRANSURETHRAL URETEROLITHOTRIPSY FOR UPPER URINARY TRACT STONE IN SMALL CHILDREN WEIGHNING AROUND 10KG].

Management strategy for upper urinary tract calculi in small children is still a matter controversial. We report successful management of ureteral stone with transurethral ureterolithotripsy (TUL) in 2 boys weighing around 10 kg. Case 1: A 2-year-old boy (78 cm in height, 9.6 kg in weight), who received hydrocortisone and fludrocortisone for the treatment of 21-hydroxylase deficiency, was referred to our hospital with a right 9-mm lower ureteral stone. For TUL, a 7.5 Fr rigid cystoscope was introduced into the ureter directly after dilation of the ureteral orifice. By using Holmium:YAG laser f

New techniques on the horizon: interventional radiology and interventional endoscopy of the urinary tract ('endourology').

PRACTICAL RELEVANCE: Interventional radiology and interventional endoscopy (IR/IE) uses contemporary imaging modalities, such as fluoroscopy and endoscopy, to perform diagnostic and therapeutic procedures in various body parts. The majority of IR/IE procedures currently undertaken in veterinary medicine pertain to the urinary tract, and this subspecialty has been termed 'endourology'. This technology treats diseases of the renal pelvis, ureter(s), bladder and urethra. In human medicine, endourology has overtaken traditional open urologic surgery in the past 20-30 years, and in veterinary medic

Therapy-resistant nephrolithiasis following renal artery coil embolization.

BACKGROUND: Transcatheter renal artery embolization is an effective and minimally invasive treatment option for acute renal bleeding. Early post-interventional complications include groin hematoma, incomplete embolization, coil misplacement and coil migration. Late complications are rare and mostly related to coil migration. CASE PRESENTATION: A 22-year-old woman with a history of recurrent stone disease and a lumbal meningomyelocele underwent bilateral open pyelolithotomy for bilateral staghorn calculi. Post-operatively, acute hemorrhage of the left kidney occurred and selective arterial coi

Prospective Analysis of a Complete Retrograde Ureteroscopic Technique with Holmium Laser Stent Cutting for Management of Encrusted Ureteral Stents.

Purpose: To propose and evaluate a new endoscopic technique using only a retrograde ureteroscopic approach for the removal of heavily encrusted ureteral stents. Methods: Data from 51 consecutive patients with encrusted and retained ureteral stents were prospectively collected. Description of the successive steps of surgery is detailed. The holmium yttrium aluminum garnet (Ho:YAG) laser properties offer the opportunity for fragmentation of stent-attached encrustation and the ability to cut the stent itself. Reducing the length of the stent is critical to creating space in the ureter and to allo

Harnessing magnetism: evaluation of safety, tolerance and feasibility of magnetic kidney stone retrieval in vivo in porcine models.

The primary objective of urolithiasis therapy is complete stone removal and highest stone-clearance rates possible to minimize recurrence. A novel approach that employs a magnetic suspension and a magnetic probe for the passive collection and removal of small residual fragments was developed. This study assessed the feasibility of this system in porcine models. Five female domestic pigs underwent retrograde intrarenal surgery under general anaesthesia to assess the new magnetic system. Pre-analysed human calculi were endoscopically inserted and comminuted using lithotripsy. The magnetic suspen

Modern management of obstructive salivary diseases.

Over the last fifteen years, increasing public demand for minimally-invasive surgery and recent technological advances have led to the development of a number of conservative options for the therapeutic management of obstructive salivary disorders such as calculi and duct stenosis. These include extracorporeal shock-wave lithotripsy, sialoendoscopy, laser intra-corporeal lithotripsy, interventional radiology, the video-assisted conservative surgical removal of parotid and sub-mandibular calculi and botulinum toxin therapy. Each of these techniques may be used as a single therapeutic modality o

Pulsed Thulium:Yag, Thulium Fiber, and Holmium:Yag Lasers in Mini-PCNL: A Three-Arm Comparative Cohort Study.

Background & Objectives: Laser lithotripsy is the standard energy source for mini-percutaneous nephrolithotomy (mini-PCNL). Although Holmium:YAG (Ho:YAG), Thulium Fiber Laser (TFL), and pulsed Thulium:YAG (p-Tm:YAG) are currently available for clinical use, direct comparisons among all three laser technologies are lacking. This study aimed to compare the efficacy, safety, and procedural performance of Ho:YAG, TFL, and p-Tm:YAG during mini-PCNL. The primary endpoint was the stone-free rate (SFR). Secondary outcomes included complications, operative time, lasing time, total energy delivered, and

Is ureteral stenting really necessary after ureteroscopic lithotripsy with balloon dilatation of ureteral orifice? A multi-institutional randomized controlled study.

PURPOSE: Our aim was to evaluate the role of balloon dilatation of the ureteral orifice on the decision to stent after ureteroscopy. MATERIALS AND METHODS: We prospectively enrolled 505 patients from two clinics, undergoing ureteroscopy (URS) for urolithiasis. Patients having balloon dilatation of the ureteral orifice and uncomplicated ureteroscopy were randomized to be either stented (n = 144) or nonstented (n = 142). Ureteroscopy was done with a 9.8 rigid ureteroscope. For dilatation of ureteral orifice, 18Fr-4 cm balloons were used (Uromax™, Boston Scientific, USA). Holmium laser or pneuma
