Unusual systemic signs in a dog with sterile neutrophilic-macrophagic lymphadenitis and nodular panniculitis.

A 7 mo old female English springer spaniel was presented with diarrhea, vomiting, apathy, and hyperthermia. Further examinations revealed generalized lymphadenomegaly consistent with sterile neutrophilic-macrophagic lymphadenitis and pulmonary involvement. Subcutaneous nodules developed one day after presentation. Histology was consistent with sterile idiopathic nodular panniculitis and vasculitis. No infectious organism was isolated. The dog responded to prednisolone, but relapsed during medication tapering. Cyclosporine had to be added to control the disease. No further relapse had occurred …

Vulval elephantiasis as a result of tubercular lymphadenitis: two case reports and a review of the literature.

INTRODUCTION: Elephantiasis as a result of chronic lymphedema is characterized by gross enlargement of the arms, legs or genitalia, and occurs due to a variety of obstructive diseases of the lymphatic system. Genital elephantiasis usually follows common filariasis and lymphogranuloma venereum. It may follow granuloma inguinale, carcinomas, lymph node dissection or irradiation and tuberculosis but this happens rarely. Vulval elephantiasis as a consequence of extensive lymph node destruction by tuberculosis is very rare. We present two very unusual cases of vulval elephantiasis due to tuberculou

Endobronchial extension of granulomatous lymphadenitis in an HIV-positive man with immune reconstitution syndrome.

Endobronchial granuloma is a rare manifestation of endobronchial tuberculosis (TB). This case report describes a patient with endobronchial … me with development of active TB is common in patients commencing highly active retroviral therapy. Lymphadenitis is the commonest manifestation of this, and 20% of patients are culture negative for mycobacteria. Endobronchial granulomata due to TB are rare and no specific endobronchial therapy is required in such disease.

Detection of opportunistic infections by low-density microarrays: a diagnostic approach for granulomatous lymphadenitis.

In molecular diagnosis of infectious diseases often more than 1 pathogen has to be considered. As a consequence, a series of labor-intensive and time-consuming polymerase chain reaction (PCR) approaches specific for different putative pathogens have to be carried out. To speed up diagnosis, we established a low-density microarray for simultaneous detection of diverse putative pathogens causing a disease such as granulomatous lymphadenitis. Nucleic acids from formalin-fixed, paraffin-embedded tissues of 68 patients with lymphadenitis were used for molecular diagnosis of individual pathogens by …

Fine-needle aspiration cytology in tuberculous lymphadenitis of patients with and without HIV infection.

A study of the cytologic features and role of fine-needle aspiration cytology (FNAC) in tuberculous lymphadenitis (TBL) of 21 patients with HIV (group 1) and 21 patients without HIV (group 2) infection was undertaken. Four cytologic patterns were observed, of which necrotizing lymphadenitis (42.9%) and necrotizing suppurative lymphadenitis (28.6%) were predominant in group 1 while necrotizing granulomatous lymphadenitis (47.7%) and granulomatous lymphadenitis (23.8%) were more common in group 2. No pattern was found specific for either group. Zeihl-Neelsen-stained cytology smears of group 1 sh…

Mesenteric panniculitis associated with abdominal tuberculous lymphadenitis: a case report and review of the literature.

Mesenteric panniculitis is a rare disease characterized by chronic non-specific inflammation of the mesenteric adipose tissue. The specific aetiology of the disease is previously unknown. A case diagnosed as mesenteric panniculitis is presented. The cause was biopsy-proved abdominal tuberculous lymphadenitis. To our knowledge, mesenteric panniculitis associated with tuberculosis infection has not been reported previously in the literature. Thus, we would like to present the first case and describe CT features of the disease.

Mycobacterial cervical lymphadenitis in children: clinical and laboratory factors of importance for differential diagnosis.

The role of epidemiological and clinical features for the early differential diagnosis between Mycobacterium tuberculosis (MTB) complex and non-tuberculous mycobacterial (NTM) cervical lymphadenitis in children was examined in this study. From 1982 to 1997, 24 MTB complex cases and 26 NTM cases were diagnosed, of which 75% and 25%, respectively, presented during the first half of the study period. Epidemiological and lymph-node features and anatomical areas were not helpful in the differential diagnosis between the 2 groups. Fulfilment of 2 out of 3 criteria (positive tuberculin skin test reac…

Splenic vein occlusion secondary to tuberculous lymphadenitis at the splenic hilum: report of a case.

We report a patient with splenic vein occlusion (SVO) secondary to tuberculosis. A 17-year-old male patient with mild epigastric pain and sp … lthough there were no other findings suggesting intestinal tuberculosis, it seemed that tuberculous lymphadenitis of the splenic hilum most likely caused the occlusion of the splenic vein. Because specific tests for tuberculosis were negative in both immunohistochemical staining for bacille Calmette-Guérin and polymerase chain reaction of DNA for Mycobacterium …

[Kikuchi-Fusimoto disease: the differential diagnosis of cervical lymphadenitis with recurrent attacks of fever].

HISTORY AND CLINICAL FINDINGS: A 49-year-old, otherwise healthy woman had recurrent fever and lymphadenopathy with leukopenia for nineteen years. Her symptoms prompted successless antibiotic therapy and extensive evaluations of fever of unknown origin. At admission there were several enlarged cervical lymph nodes and subfebrile temperatures. INVESTIGATIONS: Neither laboratory findings nor imaging did show an infectious, rheumatologic or hematologic cause of her symptoms. Histopathological examination of an lymph node biopsy revealed histiocytic, necrotizing lymphadenitis (Kikuchi-Fusimoto dis…

The value of naked eye examination of biopsied lymph nodes in the diagnosis of tuberculous lymphadenitis.

Tuberculous lymphadenitis is common in Central Africa, where diagnosis by histological examination of a biopsied node is often delayed. In the present study, the naked eye appearance of the cut surface of 306 consecutive biopsied lymph nodes was compared with the histological diagnosis. One hundred and eight-eight nodes showed tuberculosis on histology (including two with coexisting second pathology). One hundred and forty-eight (79%) cases of tuberculous lymphadenitis (including both with coexisting second pathology) showed noncaseating tuberculomata or caseation visible on naked eye examinat…

Kikuchi's histiocytic necrotizing lymphadenitis: an analysis of 108 cases with emphasis on differential diagnosis.

Kikuchi's necrotizing lymphadenitis has now become recognized in many parts of the world as a well-defined clinicopathologic entity with a remarkable predilection for cervical lymph nodes of young women. The morphologic features encountered in lymph nodes are distinctive and should enable pathologists to establish the diagnosis with confidence. Nonetheless, this analysis of 108 cases, encountered over a 15-year period, has emphasized the difficulty that many pathologists have in recognizing this disorder, and in particular, in distinguishing it from malignant lymphoma. Although in the vast maj…

Congenital tuberculosis presenting as acute cervical lymphadenitis in a 28-week preterm infant.

Congenital tuberculosis is a rare infection in infants with extremely low birthweight. We report the case of an infant born at 28 weeks' gestation who developed isolated, cervical lymphadenitis at age 84 days. Despite the infant's continuous hospitalisation in the neonatal intensive care unit and a negative maternal history, Mycobacterium tuberculosis was identified through culture and molecular testing of surgical drainage fluid. The diagnosis was substantially delayed due to several factors: a low index of clinical suspicion based on the infant's isolation from external sources, the presence…

Case report: Cervical suppurative lymphadenitis caused by burkholderia multivorans in a healthy child.

BACKGROUND: Cervical suppurative lymphadenitis in children is commonly caused by Staphylococcus aureus or Streptococcus pyogenes. However, cases caused by Burkholderia multivorans (BM) are rare. The clinical presentation lacks specificity, making it difficult for clinicians to recognize, which may delay diagnosis and treatment. CASE PRESENTATION: We report a case of a 5-year-old boy admitted with recurrent fever and neck swelling. Initial treatment with meropenem and linezolid was ineffective, and symptoms persisted after 24 days of conservative therapy. Aspiration of pus yielded negative cul…

Early differentiation of Kawasaki disease preceded by cervical lymphadenopathy from bacterial cervical lymphadenitis.

UNLABELLED: Node-first Kawasaki disease (NFKD) initially presents with fever and cervical lymphadenopathy mimicking bacterial cervical lymphadenitis (BCL). This study aimed to develop a scoring system to enable the early differentiation between NFKD and BCL. This single-center retrospective study included patients aged ≤ 15 years admitted between January 2011 and May 2024 who received initial antibiotic treatment for cervical lymphadenitis. Patients were categorized into NFKD and BCL groups according to the final diagnosis, and clinical and laboratory parameters at admission were analyzed usin…

A 28-Year Multicenter Cohort Study of Nontuberculous Mycobacterial Lymphadenitis in Children, Spain.

We describe the epidemiology, diagnosis, and management of nontuberculous mycobacterial lymphadenitis cases detailed in a 28-year (1996-2023) multicenter cohort from Spain. The case numbers remained stable during the initial prospective phase (2013-2020), but a sharp decline was observed during 2021-2022. Disease onset occurred during spring or June in 45.9% of cases. Mycobacterium avium complex (43.1%) and M. lentiflavum (39.9%) were the most common species detected. M. lentiflavum affected mostly younger children from central Spain. The most common treatment strategy was complete surgical re…
