Comparison of the McGrath® Series 5 and GlideScope® Ranger with the Macintosh laryngoscope by paramedics.

BACKGROUND: Out-of-hospital endotracheal intubation performed by paramedics using the Macintosh blade for direct laryngoscopy is associated with a high incidence of complications. The novel technique of video laryngoscopy has been shown to improve glottic view and intubation success in the operating room. The aim of this study was to compare glottic view, time of intubation and success rate of the McGrath® Series 5 and GlideScope® Ranger video laryngoscopes with the Macintosh laryngoscope by paramedics. METHODS: Thirty paramedics performed six intubations in a randomised order with all three

From mannequins to humans - are manual therapy motor skills transferable? A mixed-methods study.

BACKGROUND: Manual therapy, including spinal manipulative therapy (SMT), is a core component of chiropractic education. Simulation tools such as the Human Analogue Mannequin (HAM®) have been developed and coupled with force-sensing technology to support safe and structured training of SMT force-time characteristics. However, it remains unclear whether motor skills acquired using these tools are transferable to real-world scenarios. METHODS: This sequential explanatory mixed-methods observational study investigated whether chiropractic students, proficient at applying a set of pre-specified SMT

Effect of the Brief Instructional Video Intervention on the Quality of Cardiopulmonary Resuscitation.

Background: Chest compressions are the basis of cardiopulmonary resuscitation (CPR), and high-quality chest compressions can improve survival rate in patients with out-of-hospital cardiac arrest. Although many efforts have been made to improve the quality of CPR in inexperienced adults, the results are still not high, especially during emergencies. The primary purpose of this study is to investigate whether a brief instructional chest compression-only CPR video could improve chest compression quality in inexperienced adults. Methods: One hundred adults with no CPR experience (age: 20.28 ± 2.28

Experimental modelling of imposed upper airway obstruction in infants and children.

The interpretation of injuries to children and infants poses a number of difficulties to any medical practitioner involved in their care or tasked with the investigation their death. This includes differentiating accidental from non-accidental trauma and the consideration of medical factors making a child more prone to injury. Non-fatal but life-threatening upper airway obstruction is unfortunately a well-recognized pattern of abuse which may precede a fatal episode. In this experimental study, we aimed to model theoretical digit marks to the head using infant and young child resuscitation dum

A proclaimed accidental fall of an infant-an experimental case reconstruction study.

The differentiation between a non-accidental injury and injuries resulting from accidents, such as falls to the ground or onto various objects, is a challenge not only for forensic experts but also for all clinicians caring for children. The forensic-biomechanical analysis of accidents in infants aims at the assessment of the loading characteristics based on the reconstruction of the particular incident. It includes an evidence-based analytical comparison between the actual injuries presented and the injuries that should be expected as a result of the proclaimed accident. With the help of kine

Gum-Elastic Bougie Efficacy for Tracheal Intubation During Continuous Chest Compression in Infants-A Crossover Simulation Trial.

BACKGROUND: Recent guidelines for infant cardiopulmonary resuscitation emphasize that all rescuers should minimize interruption of chest compression, even for endotracheal intubation. OBJECTIVE: We compared the utility of application of a gum-elastic bougie (GEB) plus Miller laryngoscope (Mil) with the Mil alone during chest compression on an infant mannequin. METHODS: Sixteen anesthesiologists with more than 2 years of experience performed tracheal intubation on an infant mannequin using the Mil or Mil plus 6Fr GEB, with or without chest compression. Intubation success rate, intubation time

New Risk Curves for NHTSA's Brain Injury Criterion (BrIC): Derivations and Assessments.

The National Highway Traffic Safety Administration (NHTSA) recently published a Request for Comments regarding a potential upgrade to the US New Car Assessment Program (US NCAP) - a star-rating program pertaining to vehicle crashworthiness. Therein, NHTSA (a) cited two metrics for assessing head risk: Head Injury Criterion (HIC15) and Brain Injury Criterion (BrIC), and (b) proposed to conduct risk assessment via its risk curves for those metrics, but did not prescribe a specific method for applying them. Recent studies, however, have indicated that the NHTSA risk curves for BrIC significantly

A simulator-based study of in-flight auscultation.

INTRODUCTION: The use of a stethoscope is essential to the delivery of continuous, supportive en route care during aeromedical evacuations. We compared the capability of 2 stethoscopes (electronic, Litmann 3000; conventional, Litmann Cardiology III) at detecting pathologic heart and lung sounds, aboard a C135, a medical transport aircraft. METHODS: Sounds were mimicked using a mannequin-based simulator SimMan. Five practitioners examined the mannequin during a fly, with a variety of abnormalities as follows: crackles, wheezing, right and left lung silence, as well as systolic, diastolic, and

A transcervical chorionic villus sampling model for teaching.

We sought to create a transcervical chorionic villus sampling model for teaching that would mimic a lifelike model. A model was created using silicone resembling the maternal interface. A cervix with an endocervical canal able to accommodate a catheter and a vagina was also created. Tap water was used as the amniotic fluid. Chorionic villus sampling was accomplished using this model with the actual ultrasound machines and environment as in the real model. This simulator allowed placental placement in different locations to increase the difficulty level as well as angulations and catheter handl

Experimental methods to determine inhalability and personal sampler performance for aerosols in ultra-low windspeed environments.

Most previous experiments of aerosol inhalability as it relates to particle aerodynamic diameter were conducted in wind tunnels for windspeeds greater than 0.5 m s(-1). While that body of work was used to establish an inhalable aerosol convention, results from studies in calm air chambers (for essentially zero windspeed) are being discussed as the basis of a modified criterion. Meanwhile, however, information is lacking for windspeeds in the intermediate range, which--it so happens--pertain to most actual workplaces. With this in mind, we have developed a new experimental system to assess inha

Development and use of mechanical devices for simulation of seizure and hemorrhage in obstetrical team training.

INTRODUCTION: The leading causes of pregnancy-related death are embolism (20%), hemorrhage (17%), and pregnancy-induced hypertension (16%).The Obstetric and Mechanical Engineering Departments at Stanford University worked together to create inexpensive devices that were used in high fidelity simulations to replicate 2 of the leading causes of maternal mortality: hemorrhage and eclampsia (seizure). METHODS: The mechanisms were designed to behave as similarly as possible to a human patient. The engineering team designed the eclampsia mechanism to jostle the mannequin's head at a frequency and r

Continuous monitoring of particle emissions during showering.

Particle formation from showering may be attributed to dissolved mineral aerosols remaining after evaporation of micron-sized satellite droplets produced by the showerhead or from splashing of larger shower water droplets on surfaces. Duplicate continuous particle monitors measured particle size distributions in a ventilated residential bathroom under various showering conditions, using a full-size mannequin in the shower to simulate splashing effects during showering. Particle mass concentrations were estimated from measured shower particle number densities and used to develop emission factor

Laser scanning as a tool for assessment of HIV-related facial lipoatrophy: evaluation of accuracy and reproducibility.

BACKGROUND: Facial lipoatrophy (LA) is a common complication of highly active antiretroviral therapy (HAART). Research into causes and treatment of facial LA is hindered by the lack of an objective measurement tool. OBJECTIVE: To evaluate the accuracy and reproducibility of three-dimensional laser scanning (LS) for estimating cheek volume changes. METHODS: Paired laser scans were performed and the images superimposed using commercial software. The volume difference between images was computed within a circle of radius 25 mm placed in a standardized position over the cheek area. Accuracy was

Two third-year medical student-level laboratory shock exercises without large animals.

BACKGROUND: Historically, medical schools have taught principles of hemodynamic shock using large animal models. Such exercises are infrequent today due to the increasing aversion of students and the wider community to the use of large animals in teaching. Herein, we describe two alternative exercises that communicated basic science and clinical principles of shock effectively. METHODS: We developed two complementary, distinct single-afternoon laboratory exercises for third-year medical students. The first exercise (lab) demonstrated three principles: (1) in vitro cytokine-induced apoptosis (

The influence of behavior on freefall injury patterns: possible implications for forensic anthropological investigations.

Case studies of freefall injuries suggest that most falls from heights result in lower extremity, pelvic, and vertebral fractures. These injuries are largely a consequence of the fact that most falls are accidental with victims landing feet first. This study investigates whether human behavioral response affects body orientation at impact and whether the human body tends to align in a particular way as a result of physical laws. The investigation was undertaken by observing nine experimental falls of an anthropomorphic dummy from a height of 65 ft (9.8 m). In all nine falls, the dummy landed h
