Mucocutaneous lymph node syndrome (Kawasaki's disease) is an acute febrile, exanthomatous illness in which massive lymphadenopathy may occur in the cervical area. This lymphadenopathy may mimic other conditions and present a diagnostic dilemma. We present a case report of a child whose initial clinical signs, symptoms, and laboratory findings were compatible with acute coalescent mastoiditis; however, the child was found after close observation to have Kawasaki's disease. The case demonstrates the similarity between the two diseases and how close observation can prevent unnecessary surgery. Wh…

Symptomatic otologic involvement by leukemic infiltration is unusual, most often occurring in the already-diagnosed leukemic patient as postauricular mass, acute hemorrhagic otitis media, mastoiditis, cranial neuropathy, vertigo, hearing loss, or leptomeningitis. We think ours is the first reported patient whose leukemia presented as atypical mastoiditis and facial paralysis due to granulocytic sarcoma (chloroma). At mastoidectomy, tan lobulated rubbery tumor filled the mastoid antrum and middle ear cleft. Complete remission and full return of facial nerve function was achieved with 2500 R loc…

BACKGROUND: Acute otitis media is among the most common infections in children. Mastoiditis is a rare complication of acute otitis media in developed countries. When overlooked, mastoiditis can lead to temporomandibular joint (TMJ) ankylosis, which can impair function and hinder normal development of the child.
METHODS: This report describes a new protocol for treating TMJ ankylosis in toddlers, using a modified surgical method and in-house 3D technology. It presents a multidisciplinary approach that includes maxillofacial surgeons, pediatric anesthesiologists, orthodontists, and physical the…

OBJECTIVE: To describe and analyze the microscopic morphometry of the mastoid segment of the human facial nerve in the region of the stylomastoid foramen, in different age groups of elderly individuals.
METHODS: Eighteen blocks of temporal bones from cadavers aged between 63 and 94 years old were used and divided into 3 age groups: 60-70, 71-80 and over 80-years-old. The nerve was collected, fixed in a glutaraldehyde and paraformaldehyde solution and then subjected to dehydration and resin embedding processes. Semi-thin cross-sectional sections (1 μm) were obtained using an ultramicrotome and…

Background: Ongoing healthcare and medical education reforms in Kazakhstan have been accompanied by persistent workforce shortages and reduced inpatient capacity in pediatric care. Therefore, this study aimed to assess and forecast selected healthcare system indicators using acute mastoiditis (AM) as a sentinel condition while also describing its clinical and epidemiological characteristics. Materials and Methods: This study combined an analysis of national healthcare and demographic statistics in Kazakhstan from 1998 to 2024 with a retrospective review of pediatric AM patients treated at a te…

BACKGROUND: There has been a recent increase in the incidence of paediatric acute mastoiditis (AM) with a global trend towards more conservative management strategies. Our aim was to evaluate the contemporary management of paediatric AM in Aotearoa New Zealand.
METHODOLOGY: A retrospective observational study including AM patients aged <18 years treated in five tertiary hospitals across Aotearoa New Zealand between 2014 and 2022.
RESULTS: There were 212 cases of AM with 104 cases (49%) of uncomplicated AM and 108 cases (51%) of complicated AM. Complications included 75 cases (35%) of sub-per…

INTRODUCTION: Acute mastoiditis is one of the most common complication of acute otitis media, and its clinical course may vary significantly among patients.
OBJECTIVE: To investigate the association between adenoid size and the severity of acute mastoiditis in children.
METHODS: This retrospective cohort study included children (<18 years) hospitalized with acute mastoiditis. The primary outcome was the association between disease severity-defined by the presence of intracranial complications or PICU admission - and the adenoid-to-nasopharyngeal ratio (ANR), measured on CT scans at three nas…

OBJECTIVE: To assess pediatric patients presenting with acute mastoiditis with and without intracranial complications (ICC) METHODS: A case series with retrospective review was performed on pediatric patients who presented to a pediatric tertiary care center with the diagnosis of mastoiditis from 2012 to 2020. All patients had a documented microbiology culture. The main factors compared among patients were bacteria isolated from cultures, antibiotic resistance, and occurrence of intracranial complications of mastoiditis.
RESULTS: 298 patients with mastoiditis were included in this study with …

Acute mastoiditis (AM) is a rare but potentially life-threatening condition in pediatric patients, particularly those with cochlear implants (CI) seem to be at higher risk for complications. The aim of this study is to propose a diagnostic and treatment algorithm by analyzing common symptoms, incidence, microbiology, and management of AM in pediatric CI patients. This systematic review includes 20 studies and four own cases, therefore 86 pediatric CI patients diagnosed with AM were enrolled. The research databases Pubmed, Cochrane Library, Cumulative Index to Nursing and Allied Health Literatu…

INTRODUCTION: Mastoiditis is a major complication of acute otitis media that, if left undiagnosed or untreated, can lead to meningitis. This case report shares the clinical course of a child with acute mastoiditis. The related discussion provides an overview of acute mastoiditis, including its presenting signs and symptoms, diagnostic testing, treatment, and outcome and prognosis.
CLINICAL FINDINGS AND DIAGNOSIS: A 9-year-old male patient presented with fever, symptoms of a viral upper respiratory infection, and tachycardia. His physical examination was remarkable for muscle hypotonia, asymme…

KEY CLINICAL MESSAGE: Austrian Syndrome classically consists of meningitis, endocarditis, and pneumonia due to Streptococcus pneumonia bacteremia. A literature review, however, does not show variants of this triad. Our case highlights a unique variant of Austrian Syndrome with mastoiditis, meningitis, and endocarditis which requires immediate recognition and treatment to prevent devastating patient outcomes.
ABSTRACT: Streptococcus pneumonia is responsible for more than 50% of all bacterial meningitis and has a case fatality rate of 22% in adults. In addition, Streptococcus pneumonia is also …

IgG4-related disease (IgG4-RD) is an inflammatory condition characterised by infiltration of tissue by IgG4-positive plasma cells. This is the seventh reported case of IgG4-RD affecting the mastoid and informs clinicians in diagnosing patients affected by this rare condition.A woman in her 20s presented with unilateral otalgia, hearing loss and vertigo. She deteriorated despite antibiotic therapy and cross-sectional imaging revealed a destructive extra-axial lesion of the mastoid cells. Biopsy confirmed a diagnosis of IgG4-RD. She was successfully treated with prednisolone and azathioprine.Inf…

BACKGROUND: Acute mastoiditis (AM) is the most common complication of acute otitis media and primarily affects children under the age of two; current data on its prevalence in paediatric patients with cochlear implant (CI) are still scant. Proper management of AM in CI children is crucial in order to avoid the implications (financial and emotional) of an explant. Aim of this paper is to describe the cases of AM occurred among young patients with CI in follow up at our department, also in order to evaluate its prevalence, potential predisposing factors, clinical course and therapeutic strategie…

PURPOSE: This study aimed to investigate the variability of the retrotympanum in patients undergoing surgical treatment for cholesteatoma.
METHODS: We included 59 ears of patients undergoing middle ear surgery for cholesteatoma who had preoperative computed tomography scans. A retrospective analysis of the medical records was conducted. The sinus tympani (ST), subtympanic sinus (STS) and facial recess (FR) were classified into types A-C based on the relationship of their extension to the facial nerve. The mastoid and petrous apex were assessed and categorized as normal pneumatized or scleroti…
