Maternal mortality in southern Africa, 1980-1982. Part I. Pregnancy can be lethal.

Statistics regarding the number of births and maternal deaths during the 3-year period 1980-1982, received from 267 hospitals in southern Africa, are presented. Although 812 maternal deaths were recorded, sufficient information for adequate analysis of epidemiological factors (age and parity), cause of death and avoidable factors was available in only 737. The maternal mortality rate was 8.3/10,000. Six hundred and sixty deaths (89.6%) were classified as direct obstetric death. Two hundred and twenty-nine deaths (31%) occurred in women experiencing their first pregnancy. Avoidable factors coul…

Maternal mortality in the American University of Beirut Medical Center (AUBMC) 1971-1982.

Maternal deaths were reviewed at the American University of Beirut Medical Center (AUBMC) during an 11-year period, 1971-1982. There were 35,058 live births and 45 deaths making a maternal mortality rate of 128 per 100,000 live births. Hemorrhage, sepsis and toxemia were the main direct obstetric causes of death. The most important indirect causes were cerebrovascular accidents and heart disease. In this review, an analytic discussion of the direct and indirect causes of maternal death in Lebanon are presented and preventive measures are discussed.

Causes and contributory factors of maternal mortality: evidence from maternal and perinatal death surveillance and response in Ogun state, Southwest Nigeria.

BACKGROUND: Nigeria still ranks second globally in the number of maternal deaths. Most maternal death reviews in Nigeria are isolated research based reports from a single health facility. This study determined causes and contributory factors of maternal mortality in Ogun statefollowing a periodic State-widematernal and perinatal deaths surveillance and response (MPDSR) review. METHODS: We carried out a retrospective analysis of cases of maternal deaths notified (n = 77) and reviewed (n = 45) in health facilities in Ogun State from 2015 to 2016selected using total sampling method. Using the na…

Racial Disparities and the Use of Artificial Intelligence for Predicting Maternal Mortality: A Literature Review.

BACKGROUND: Maternal mortality remains a major global health challenge, disproportionately affecting black and Indigenous women. Hypertensive disorders of pregnancy and postpartum hemorrhage are the leading direct causes of maternal death. Artificial intelligence (AI) tools have emerged as potential strategies for predicting these complications, yet concerns persist about their equity and validation across racial groups. METHODS: A rapid review was conducted in five databases, PubMed, EMBASE, Web of Science, Scopus and LILACS, to synthesize recent evidence on the use of AI for preventing mate…

The benefits and risks of maternal RSV vaccination on mortality in South Africa: A modeling study.

BACKGROUND: Maternal respiratory syncytial virus (RSV) vaccine, RSV prefusion F protein vaccine (RSVpreF (Abrysvo)), was found to be safe and efficacious in the MATISSE trial. However, post-hoc stratified analyses identified an excess of preterm births in the intervention arm in two upper-middle-income countries, most prominently in South Africa. This study weighs the potential benefits and risks in mortality associated with maternal RSV vaccination in South Africa, assuming the increased risk of preterm births observed in the trial was caused by vaccination. METHODS AND FINDINGS: We compared

Transition in the Composition of Maternal Mortality in Korea: Implications for Extremely Low Fertility Societies.

BACKGROUND: We examined long-term trends in age-specific and cause-specific maternal mortality and assessed shifts in the relative contributions of direct versus indirect obstetric causes from 1983 to 2023 in Korea. METHODS: In this population-based observational study, we analyzed all registered maternal deaths (International Classification of Diseases-10 O00-O99) among women aged 15-49 years and corresponding live births in Republic of Korea (= South Korea) from 1983 through 2023. Annual age-specific maternal mortality ratios (MMRs) per 100,000 live births were calculated using data from th…

Gestational age, birthweight, and maternal factors associated with neonatal mortality in the United States.

BACKGROUND: Neonatal mortality remains a key indicator of maternal and infant health and continues to show substantial variation across demographic and clinical groups in the United States. A clearer understanding of nonlinear associations and effect modification may help characterize epidemiologic patterns of neonatal mortality and inform perinatal prevention efforts. METHODS: We conducted a population-based retrospective cohort study using the U.S. Linked Birth-Infant Death Database from 2015 to 2020. Neonatal mortality was defined as death occurring from day 0 through day 27 after live bir

Maternal coronary heart disease and mortality following hypertensive disorders of pregnancy and/or diabetes.

BACKGROUND: Pre-pregnancy hypertension (HTN), hypertensive disorders of pregnancy (HDP), and diabetes have been linked to increased risk of post-pregnancy coronary heart disease (CHD) and all-cause mortality, but few studies have investigated their cumulative impact. This study aimed to assess the potential relationship between pre-pregnancy HTN, HDP, and diabetes and their cumulative impact on maternal cardiovascular outcomes defined as incident CHD and all-cause mortality within 5 years of delivery and over the entire study period (up to 14 years after delivery). METHODS: This retrospective

Care of the Postpartum Patient in the Emergency Department: A Systematic Review with Implications for Maternal Mortality.

OBJECTIVE: Approximately one-third of maternal deaths occur postpartum. Little is known about the intersection between the postpartum period, emergency department (ED) use, and opportunities to reduce maternal mortality. The primary objectives of this systematic review are to explore the incidence of postpartum ED use, identify postpartum disease states that are evaluated in the ED, and summarize postpartum ED use by race/ethnicity and payor source. STUDY DESIGN: We searched PubMed, Embase, Cumulative Index of Nursing and Allied Health Literature (CINAHL), ClinicalTrials.gov, Cochrane CENTRAL…

Mode of delivery is an independent risk factor for maternal mortality: a case-control study.

BACKGROUND AND OBJECTIVES: In contrast to the global trend, the maternal mortality ratio (MMR) in the United States has increased in recent decades. During this time, the cesarean section rate has concurrently and steadily increased. Herein, we sought to determine whether the mode of delivery is an independent risk factor for maternal in-hospital mortality. MATERIALS AND METHODS: We conducted a retrospective, population-based, 1:1 matched, case-control study on all births recorded in the Health - Care Cost and Utilization Project - Nationwide Inpatient Sample between 2005 and 2014. We compare…

Maternal mortality predictors in women with hypertensive disorders of pregnancy: a retrospective cohort study.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are multisystem disorders unique to human pregnancy. They are becoming the leading causes of maternal mortality worldwide, with the majority of deaths occurring in low income countries. However, little is known about the predictors of maternal mortality in women with HDP. METHODS: A retrospective cohort study was conducted between 2008 and 2013 in three university teaching hospitals among 1015 women admitted with a diagnosis of HDP. Statistically significant associations were assessed by the hazard ratio (HR) with 95% confidence using the …
