Biomechanical testing of zirconium dioxide osteosynthesis system for Le Fort I advancement osteotomy fixation.

The following work is the first evaluating the applicability of 3D printed zirconium dioxide ceramic miniplates and screws to stabilize maxillary segments following a Le-Fort I advancement surgery. Conventionally used titanium and individual fabricated zirconium dioxide miniplates were biomechanically tested and compared under an occlusal load of 120N and 500N using 3D finite element analysis. The overall model consisted of 295,477 elements. Under an occlusal load of 500N a safety factor before plastic deformation respectively crack of 2.13 for zirconium dioxide and 4.51 for titanium miniplate

Transmaxillary approach to the cranial base: an evaluation of 11 cases.

UNLABELLED: Surgical access to the skull base is always difficult, especially because of the noble anatomic structures present there. Maxillary osteotomy provides direct view to the clivus region and the neck spine, and it also bears less morbidity when compared to the many other accesses described in the literature. AIM: to assess 11 patients submitted to transmaxillary osteotomy, describing the surgical technique and postoperative results and complications. MATERIALS AND METHODS: A retrospective study involving eleven patients submitted to transmaxillary approach to the brainstem. We studi…

Use of the 'shark-fin' osteotome in separation of the pterygomaxillary junction in Le Fort I osteotomy: a clinical and computerized tomography study.

Le Fort I osteotomy fails in many cases to completely separate the pterygomaxillary junction and often results in fractures of the pterygoid bone and the tuberosity, which subsequently can cause complications. The objectives of this study were to describe the specifically developed Laster 'shark-fin' osteotome and to compare its use to other methods of pterygomaxillary dysjunction. Pterygomaxillary dysjunction was performed in 10 adult patients requiring Le Fort I osteotomy. In one randomly chosen side of the maxilla, the Obwegeser osteotome was used, while the Laster 'shark-fin' osteotome was

Le Fort I advancement osteotomies of 1 cm or more. How safe or stable?

We studied the results and stability of Le Fort I maxillary osteotomies with advancement of at least 1cm. We analysed prospective data from 32 consecutive patients who were operated on by a single surgeon between 1985 and 2007, and who had mean advancement of the maxilla of 14 mm (range 10-22 mm). Mean relapse for all cases antero-posteriorly was 1mm, 10% (range 0-4mm). Relapse was marginally greater in the 24 patients with a repaired cleft palate (11%) compared with those without a cleft (8%). The results were essentially stable at least one year after surgery and without significant complica

Biomechanical models to simulate consequences of maxillofacial surgery.

This paper presents the biomechanical finite element models that have been developed in the framework of the computer-assisted maxillofacial surgery. After a brief overview of the continuous elastic modelling method, two models are introduced and their use for computer-assisted applications discussed. The first model deals with orthognathic surgery and aims at predicting the facial consequences of maxillary and mandibular osteotomies. For this, a generic three-dimensional model of the face is automatically adapted to the morphology of the patient by the mean of elastic registration. Qualitativ

Trans oral approach to the nasopharynx and clivus using the Le Fort I osteotomy with midpalatal split.

The Le Fort I level osteotomy is a procedure well known to oral and maxillofacial surgeons, who routinely use it to correct midfacial skeletal deformities and alter the dental occlusion. This osteotomy can also be used as a maxillotomy for access to more superiorly and posteriorly situated structures. The downfracture technique provides the surgeon with a safe approach that allows visualization of the maxillary sinuses, nasal cavity, nasopharynx, base of the skull and upper cervical spine. This approach can also be combined with a midline lip split, mandibulotomy and glossotomy to give access

Supplemental Internal Wire Suspension in Orthognathic Surgery Fixation.

Internal wire suspensions were previously used in the management of maxillary trauma and orthognathic surgery, but currently the gold standard is rigid internal fixation with bone plates and screws. The authors report a case of bimaxillary osteotomy in a skeletal class III patient where a supplemental circum-zygomatic-mandibular suspension was done to stabilize the maxilla because of the instability after rigid internal fixation due to poor bone thickness.

Study of Anatomical Changes of the Inferior Nasal Passage After Le Fort I Osteotomy With Superior Repositioning.

The relationship between postoperative morphological changes in the inferior nasal cavity and inferior turbinate after Le Fort I osteotomy remains unclear. This study aimed to investigate how the bone volume of the inferior turbinate affects contact with the inferior nasal cavity of patients who underwent superior repositioning. We evaluated the 3-dimensional relationship between the anatomical changes in the inferior nasal passage before and after surgery in 51 patients who underwent Le Fort I osteotomy with an elevation of >4.0 mm in the first molar. The soft tissue and bone volumes of the i

Bioactive glass granules S53P4 in osteotomy Le Fort I.

We evaluated bioactive glass graft (S53P4) in patients undergoing Le Fort I osteotomy, with non-grafted patients as controls. Computed tomography facial scans of the 25 patients submitted for Le Fort I were divided into two groups: Group 1-S53P4 group and Group 2-without grafting. CT scans were analyzed in the immediate postoperative period (T1) and 6 months later (T2), for linear bone gap measurements, tomographic radiodensity and behavior of the maxillary sinus. A Kruskal-Wallis test on bone gap data adopted α significance levels (p ≤ 0.05). The Friedman test (p ≤ 0.05) was used to evaluate

Pterygo-Maxillary Fissure as a Landmark for Localization of Internal Maxillary Artery for Use in Extracranial-Intracranial Bypass.

BACKGROUND: Internal maxillary artery (IMax) is a relatively new donor vessel used in the extracranial-intracranial bypass surgery. However, unfamiliarity and relatively elaborate techniques of its harvest precluded its widespread use. OBJECTIVE: To present a simplified technique of IMax harvest based on constant anatomical landmarks without the need of extensive skull-base drilling while providing adequate space for proximal anastomosis. METHODS: Cadaveric dissection on 4 cadaveric heads (8 sides) was performed. Zygomatic osteotomy was performed and temporal muscle was dissected off the zyg

Prosthetic-surgical integration and robot-assisted implant placement for maxillary anterior fixed rehabilitation in the predoctoral clinic: A case report.

This clinical report describes a prosthetically directed execution to maxillary anterior rehabilitation in a patient with congenitally missing lateral incisors, retained primary molars, and anterior segment spacing. The definitive tooth morphology and eccentric guidance were established before surgery through a diagnostic wax-up, intraoral mock-up, and conservative direct bonding to confirm esthetic and functional parameters. Implant placement at sites #5 and #12 was then completed using robotic-assisted intervention, allowing versatile mesial-angulation correction of the osteotomies to avoid

Transmaxillary osteogenic distraction with intraoral tooth-borne distractors.

Midfacial retrusion and class III malocclusion in maxillary hypoplasia are frequent sequelae in patients with cleft lip and palate. Similar deformities are seen in craniofacial dysostosis surgically treated in infancy. Recurrences after orthognathic surgery are unpredictable even using rigid fixations. Distraction osteogenesis, using Le Fort I osteotomies with external distractors, is uncomfortable for the patient, and internal distractors require a second procedure for removing the retractor. A new model of distraction osteogenesis is presented. Transmaxillary osteogenic distraction, using to

Nasolabial aesthetics.

Orthognathic surgery, as it relates to the maxilla, attempts to correct underlying skeletal deformities as well as improve function. In addition, it has the potential to significantly alter the central aesthetic unit of the face, the nasolabial region. A key to achieving a good functional as well as aesthetic result involves both comprehensive surgical planning and an understanding of the effects that orthognathic surgery of the maxilla will have on the soft tissues within the nasolabial region. The LeFort I osteotomy is one of the most commonly performed techniques to correct maxillary dentof
