Fixed-Dose Combination Therapy as a Strategy to Improve Antidiabetic Medication Adherence in People with Type 2 Diabetes: A Narrative Review.

Type 2 diabetes mellitus (T2DM) is a metabolic condition associated with increased morbidity, mortality and healthcare costs, with poor medication adherence to antidiabetic medications being a major barrier to optimal risk-factor control. Treatment complexity, due to polypharmacy and burdensome dosing regimens, is a key contributor to medication non-adherence in people with T2DM. Fixed-dose combination (FDC) therapy, in which two or more drugs are co-formulated into a single dosage, has emerged as a key strategy to reduce treatment burden, simplify dosing and potentially improve adherence in T

Increased DHEAS and Decreased Total Testosterone Serum Levels in a Subset of Men with Early-Onset Androgenetic Alopecia: Does a Male PCOS-Equivalent Exist?

BACKGROUND: Increased dehydroepiandrosterone sulfate (DHEAS) levels have been reported in men with early-onset (<35 years) androgenetic alopecia (AGA). It has been suggested that a male polycystic ovarian syndrome- (PCOS-) equivalent, defined as an endocrine syndrome with a metabolic background and a PCOS-like hormonal pattern, predisposing to type II diabetes mellitus (DM II), cardiovascular and prostate diseases later in life, may occur in at least a part of these men. The gonadal function, including sperm parameters and total testosterone (TT) levels, has been investigated in a low number o

Older bariatric surgery candidates: is there greater psychological risk than for young and midlife candidates?

BACKGROUND: Although severe obesity is dramatically increasing in older adults, many bariatric programs use age cutoffs due to concerns about greater perioperative morbidity and mortality risks. More recently, surgical outcomes have been reported in older adults. However, a paucity of data is available on the psychological risks of older bariatric candidates. Our objective is to examine psychiatric risk factors and weight loss outcomes in older (≥65 yr) versus midlife (40-55 yr) versus young adult (18-29 yr) patients. METHODS: Older, midlife, and young adults (n = 608) who underwent weight lo

[Bolus of methylprednisolone and Horton's disease/rhizomelic pseudo-polyarthritis. Preliminary results of a pilot study of treating the bolus with low doses of corticoids].

The aim of this study was to reduce the side effects of corticosteroid therapy responsible for high morbidity in giant cell arteritis. Nine patients were admitted consecutively for temporal arteritis (7 cases) or polymyalgia rheumatica (2 cases) without ocular involvement (mean age: 69.3 years; positive temporal biopsy: 6 cases). The following protocol was used: methylprednisolone, 500 mg/d, i.v., for 3 consecutive days, then low doses (an average of 22.5 mg/d) of prednisone or an equivalent drug. The mean length of follow-up was 15.1 months (range: 6-22 months). The mean lapse of time until t

Effects of methyl mercury on the in vivo release of dopamine and its acidic metabolites DOPAC and HVA from striatum of rats.

Mercury is a neurotoxic agent that produces different effects on the brain. In the present work, the effects of chronic doses of methyl mercury (MeHg) were studied on the dopaminergic system of the rat striatum, using microdialysis coupled to high-performance liquid chromatography in order to quantify the in vivo release of dopamine (DA) and its acidic metabolites DOPAC and HVA. The administration of an equivalent total dose of 6 mg/kg of MeHg induced significant increases in the striatal release of DA and/or its acidic metabolites, independently of the pattern of administration. These effects

Prehabilitation and functional recovery for colorectal cancer patients.

Cancer and its treatments are associated with functional decline that has impactful consequences on quality of life, and care continuum. Thus, optimizing perioperative functional capacity has been identified as a research and clinical priority in cancer care. The process of enhancing physical fitness before an operation to enable the patient to withstand the stress of surgery has been termed prehabilitation. Main elements are preoperative exercise, nutrition therapy, and anxiety-reduction techniques. Given the growing body of evidence on prehabilitation efficacy, this narrative review will sum

Nurse-Led Home-Based Mobile Health Cardiac Rehabilitation Program for Patients With Chronic Heart Failure: A Randomized Controlled Trial.

This 12-week randomized controlled trial evaluated a nurse-led mHealth intervention for patients with chronic heart failure, conceptually informed by Riegel's middle-range theory of self-care of chronic illness. The program integrated wearable activity tracking with weekly nurse-led behavioral coaching, reflecting the core self-care processes of monitoring, maintenance, and management. Compared with usual care, the intervention significantly improved daily step count, 6-minute walk distance, metabolic equivalents, and left ventricular ejection fraction. Findings highlight the effectiveness of …

[Target range of PTH in ESRD patients].

The normal bone turnover is important for ESRD patients in not only skeletal problem but also patients QOL and survival. The abnormal bone turnover often induces the renal osteodystrophy and ectopic calcification. The major regulative factor on bone turnover is PTH. We propose here the target zone of PTH in ESRD patients with skeletal resistance for PTH. We recommended the intact-PTH 100 - 150 pg/mL for normal Ca/Pi/bone metabolism. This range of intact-PTH is equivalent to C-PTH 4.0 - 6.2 ng/mL, HS-PTH 10.0 - 16.0 ng/mL, and whole-PTH 80 - 110 pg/mL. The maintenance of normacalcemia and normo

Enzyme histochemical studies on rat amygdala in fetal alcohol syndrome.

Histochemical studies were made of the activity of oxidative and hydrolytic enzymes in rat amygdala in FAS (Foetal Alcohol Syndrome). Ethanol in a dose of 9 g/kg/day was administered to rats during pregnancy and lactation in 6% aqueous solution as the only available liquid. The control rats received an equivalent diet in which ethanol was substituted by an equicaloric amount of sucrose. The offsprings were examined at the end of the 6th postnatal week. The activity of the lysosome and membrane enzymes, as well of some enzymes participating in the neurotransmission, was changed. A different dec

The effect of increases in physical activity on oral health and function in older adults in Singapore: A longitudinal causal inference study.

BACKGROUND: Physical activity may benefit oral health, but causal evidence is lacking. This study estimated the effect of modifying physical activity on self-rated oral health and masticatory function impairment in older adults using longitudinal modified treatment policies. METHODS: Data from three waves (2016-2024) of THE SIGNS Study, a nationally representative cohort of community-dwelling adults aged 60 years and above in Singapore (N = 3981), were analysed. Ten physical activity scenarios were emulated, including upward, downward, and stochastic shifts in weekly metabolic equivalent (MET…

Mogroside-Rich Monk Fruit Extract Improves Glycemic Control Without Promoting Additional Weight Gain Compared With Sucralose and Sucrose in High-Fat Diet-Induced Obese Mice.

A high-fat diet (HFD) with excessive sugar intake contributes to obesity and type 2 diabetes, leading to increased use of non-nutritive sweeteners (NNS). However, evidence on their metabolic effects remains inconsistent, especially between natural and synthetic NNS. This study compared the effects of a natural NNS, monk fruit extract (MFE), with sucrose and synthetic sucralose, at equivalent sweetness on body weight and glucose homeostasis in HFD-induced obese mice. Male C57BL/6 mice were fed an HFD for 8 weeks to induce obesity, then randomized to receive distilled water (control), sucrose (2

AI portal tract detection and characterisation for a regional analysis of steatosis and inflammation in MASLD, MASH and AIH.

AIMS: Annotation of liver biopsies for disease staging is increasingly aided by digital pathology; however, existing systems do not quantify inflammation and steatosis within an anatomical framework. We hypothesise that an artificial intelligence (AI) system that quantifies portal tracts (PT) and the anatomical distribution of steatotic vesicles and inflammatory cells will align with manual pathologist scoring and stratify liver diseases. METHODS: In this observational, cross-sectional study, digitised images of haematoxylin and eosin-stained specimens were pooled from four independent cohort
