Tympanotomy: exploratory "laparotomy" of the middle ear.

In some cases of conduction deafness or mixed deafness, direct inspection of the area believed to be involved is the only means by which diagnosis can be made with certainty. This can be done by a method of tympanotomy that is used for stapes mobilization. The necessary exposure is done with local anesthesia, is not painful, is well tolerated by the patient and requires only two days in hospital. Reparative procedures may be carried out as indicated.

Equivalent ear canal volumes in children pre- and post-tympanostomy tube insertion.

Pre- and postoperative equivalent ear canal volume measures were obtained from a group of 334 children ranging in age from 6 weeks to 6.7 years. The purpose of the study was to develop volumetric guidelines for the determination of tympanostomy tube patency. For children 4 years and older, almost no ambiguity existed in making this determination accurately. For younger children, the pre- and postoperative distributions overlap. A criterion value of greater than or equal to 1.0 cm3 as an indicator of a tympanic membrane perforation appears to yield the lowest possible error rate. When both pre-

A Comparison of 226- and 1000-Hz Probe Tone Tympanometry With Myringotomy Findings in Infants.

PURPOSE: This study aimed to verify the diagnostic value of tympanometry with 226- and 1000-Hz probe tones in infants by comparing tympanometry results with the gold standard of the middle ear state assessed by myringotomy. Furthermore, clinically useful predictors for false peaked tympanograms despite the presence of middle ear fluid should be identified. METHOD: Pre-operative 226- and 1000-Hz tympanograms were retrospectively compared with intraoperative findings of the tympanic cavity after myringotomy in 111 infants (217 ears) aged ≤ 12 months. In addition to the shape of tympanograms, de

Management of cholesterol granuloma due to eosinophilic otitis media using large ventilation tube.

A 46-year-old man who had been diagnosed with eosinophilic otitis media (EOM) and eosinophilic chronic rhinosinusitis was referred to our department. He suffered from bilateral earache, clogged ear sensation, and otorrhea associated with EOM. He had been treated with a myringotomy and a ventilation tube (VT) insertion. However, his symptoms did not improve, and the VT repeatedly fell out. We performed canal wall down mastoidectomy via a retro-auricular incision to remove the presumed cholesterol granuloma (CG) and a long-term VT insertion. The VT fell out repeatedly. Therefore, a large VT that

Temporal bone CT-based anatomical parameters associated with the development of cholesteatoma.

BACKGROUND: Cholesteatoma is caused by disorders of the middle ear ventilation that trigger a progressive series of events responsible for its formation. The aim of this study was to identify possible radiological CT-derived parameters predisposing to ventilation disorders and cholesteatoma. METHODS: In this retrospective study, patients diagnosed with cholesteatomatous chronic otitis media who underwent temporal bone CT and open tympanoplasty surgery have been included, as well as control patients with clinical examination negative for organic otological pathology who underwent temporal bone…

Acoustical Effects of Tympanostomy Tube Attachment to Human Tympanic Membrane.

PURPOSE: Several therapeutic approaches for hearing disorders involve attaching medical devices to the tympanic membrane. The attachment of these devices can change the mechanical and acoustical properties of the middle ear, affecting the middle-ear vibrations. The alteration of passive mechanical properties results from the mass, stiffness, and geometry of the attached device. Additionally, procedures like tympanostomy tube attachment create perforations on the tympanic membrane, altering both the mechanical and acoustical properties of the middle ear. This study examined the acoustical effec

Assessing Complication Risk of Pressure Equalizing Tube Placement in Children With Velocardiofacial Syndrome (22q11.2 Deletion Syndrome/DiGeorge Syndrome).

INTRODUCTION: Persistent tympanic membrane perforation is a known complication of pressure-equalizing (PE) tube insertion. Conductive hearing loss and otorrhea can necessitate surgical repair of these perforations. Long-term tympanostomy tube placement can increase the risk of these complications. Patients with velocardiofacial syndrome (VCFS) typically require prolonged PE tube placement and are thought to have higher risk of requiring additional otologic interventions after PE tube placement. To date, no work has established rates of post-PE tube complications requiring myringoplasty or tymp
